A comprehensive study of the sternoclavicular joints (SCG) of 120 autopsy cases comprising both sexes and all age groups has proved that this non-weight-bearing joint turns arthritic with increasing age. The degenerative lesions set on already from the 3rd decade; between 40 and 50 years of age, 84% of the joints are arthrotic, and over 70, all patients show an osteoarthrosis. was much less common in Black women than in White, and Heberden's nodes were seen in only 3 of 93 respondents who were over 65 years of age. Osteo-

of the metacarpophalangeal joints also, was less common in Black women than in White, but the Black men showed a much greater involvement than that seen in White men. As in White peoples, osteo-

of the metatarsophalangeal joint of the big toe was commoner in women than in men, but the over-all prevalence did not reach the high levels in Whites. Osteo-

of the knee was much commoner in the women than in the men; this was associated with a marked tendency to

of certain joints, such as the knee in women and the metacarpophalangeal joints in men, is even more common in Blacks than in Whites. The differences encountered can, in many instances, be attributed to differences in occupation and life style in the two populations; but genetic factors, may also play a part in the sense that certain predisposing disorders, such as congenital subluxation of the hip, appear to be far less common in Blacks than in Whites.

Generalized osteoarthrosis was found to be significantly more common in older males with high than with low diastolic blood pressure. The excess of osteoarthrosis in those with hypertension was mainly in the hips, knees, carpometacarpal and metacarpophalangeal joints, and was independent of
obesity
in the hypertensive group. It was not associated with a higher cholesterol or uric acid level in the serum. Radiological evidence of avascular necrosis was present in 36% of males with osteoarthrosis of the hips and diastolic blood pressure above 100 mmHg, in 20% with a diastolic pressure of 81-100 mmHg, but was found in none of those with osteoarthrosis and blood pressure of 80 mmHg or below. Only those with osteoarthrosis and a diastolic pressure above 100 mmHg had significantly more avascular necrosis that expected.
Osteoarthrosis
of the knee in female was more frequent in the hypertensive groups independent of
obesity
. It is concluded that vascular disorders are involved in this form of generalized osteoarthrosis.

Hardly any other disorder in this century has been subject to such strong changes--referring to epidemiology and symptomatology--as was gout. These changes were all linked to fundamental social upheavals. In the past 4 decades effective medicamentous possibilities of treating hyperuricaemia and gout have been added. The clinical picture of gout is being obscured by many and partly indiscriminate beginnings of treatment of innocuous or symptomless hyperuricaemias that were found by accident on the occasion of a medical checkup; attacks of gout and classical gout-specific morphological degenerations have become rare in spite of a still increasing number of hyperuricaemias among the general population. For these reasons epidemiological inquiries about gout lack any solid foundation. It is rather appropriate to speak only of "potential" gout, of a constellation which--if untreated--would probably mean manifest gout sometime. Because of the numerous accompanying diseases which usually attack patients with gout and which mostly occur combined with other disorders (such as
obesity
, lesions of the liver and kidneys, high blood pressure, disorders of the lipoprotein and carbohydrate metabolism), those patients' life expectancy--if they are not treated--is limited anyway. Most of these accompanying diseases can contribute to the development of an early severe atherosclerosis the consequences of which do lead to the death of most of the untreated "cases". Today gout as an

are largely unknown, with genetics, joint malalignment, overload or trauma,

, and aging as some of the known or suspected contributing factors. Even less well known is how these general factors are translated into disease mechanisms at the cell and tissue levels. However, it may be argued that degradation of cartilage matrix is a key event at some time in the development of
arthrosis
. During this process, fragments of matrix molecules and other chondrocyte products are released into the joint fluid and eventually into other body fluids. These molecules can be used as markers of cartilage metabolism to monitor joint disease. In addition, by identifying the proteases and the structure of the released matrix fragments, we may improve our understanding of the cellular mechanisms active in cartilage degradation. Such information offers improved diagnostic and prognostic tools for rational treatment aimed at retarding cartilage destruction in

in adults in the Congo. Affecting all socio-professional groups, it is diversely associated with

, alcoholism, hypertension and diabetes. Initial involvement affects the big toe. Oligo and polyarticular forms predominate because of the absence or delay in specific treatment. This series included 30 per cent of cases of chronic gout. Evidence of renal impairment was found in one third of patients. However, urate lithiasis was absent. Tophi were found preferentially over the elbows. Sickle cell disease was responsible for one case of tophaceous gout. In contrast with the results of studies undertaken before the 1980s, gout is seen to be a common condition in equatorial Africa.

The association between fat distribution, morbidity and subjective health was studied in 95 overweight adult men and 210 overweight adult women. Retrospective morbidity data were taken from a continuous morbidity registration made by general practitioners over a period of maximally 17 years. In addition information about subjective health and weight history was obtained from a self-administered questionnaire. Anthropometric measurements were taken and, on the basis of waist-hip and waist-thigh circumference ratios, subjects were classified into upper body segment

. It was found that, adjusted for age and body mass index, a high waist-thigh circumference ratio was a risk factor for hypertension and for gout or diabetes in women and

in men. A low waist-thigh ratio was associated with a high prevalence of varicose veins in women. The associations of waist-hips circumference ratio with morbidity were less pronounced, with the exception of hypertension in men. Information from the questionnaire revealed that persons with upper body segment

Some patients with radiologic findings of neurogenic
arthropathy
or multiple fractures do not exhibit overt neurologic signs. Results of nerve conduction velocity, computer-assisted sensory examination, periosteal nociception, and morphometric and graded teased-fiber evaluation of cutaneous nerves allowed us to recognize a mild neuropathic abnormality. Neurogenic arthropathy and subclinical neuropathy were also found in relatives. In three kinships, the underlying disorder was probably hereditary sensory neuropathy type 1 and in several others, it was recessively inherited sensory neuropathy. These arthropathies were often painful, and overt loss of superficial and deep pain sensation was not a prominent or necessary condition.
