There are many complications of a jejunoileal bypass for
obesity
. I report what is, to my knowledge, a previously unrecognized complication--granulomatous
colitis
and ileitis. Caution is urged in recommending this procedure for the treatment of
obesity
.

The results of the study have shown that the nutrition of rural population is characterized by excessive consumption of bread and baked products, by high content of phosphorus, magnesium and iron, low content of animal proteins, vegetable oils, calcium, vitamins A, ascorbic acid and riboflavin. The incidence of cardiovascular, respiratory and alimentary diseases in this group of population was rather high. The nutrition of students is characterized by excessive consumption of polysaccharides, vegetable oils, thiamine, niacin, ascorbic acid (in winter-spring period), and calcium. Diseases associated with nutrition disorders (
obesity
, hepatitis, cholecystitis,
colitis
) are most often recorded in this group of population.

15,000 adults in Azerbaijan have been surveyed. The programme of the survey included interviews with specially designed questionnaires consisting of questions related to working and living conditions, nutritional habits, X-ray and endoscopic examinations of the colon have been conducted. Referrals to polyclinics for colon diseases accounted for 6.0 per 100 inhabitants and after active detection among men--41.5 and among women--45.6 per 100 persons examined. The leading role in the incidence of chronic diseases of colon is played by hemorrhoid (31.0%), chronic
colitis
(33.6%), colon dyskinesia (13.5%). It has been found that the most significant predisposing causes of colon diseases were: bacillary dysentery, different types of neuroses, milk indigestion, gynaecological and prostatic diseases, and
obesity

The authors report three observations of patients having suffered from arthralgias as an aftermath of intestinal by-pass for treatment of refractory
obesity
. They remind the main troubles cropping up after such surgical treatment and refer to the data already existing in the literature dealing with articular manifestations. They think this pathology is in keeping with arthritis already reported in inflammatory
colitis
and they suggest a similar pathogeny.

After a brief reference on the so-called diseases of "civilisation" and on their correlations with a diet rich in refined high caloric carbohydrate having poor gastric filling capacity and therefore lacking in fibre, the authors present their experience on a new product, MF/D/80, which is an association of cereal and acid fruit fibres. 25 patients (3 men and 22 women) were treated, 22 of them suffered from chronic constipation and 3 from
colitis
. The experiment was carried out without the use of control drugs. The aim of the research was that of verifying the efficacy and tolerability of the product. The efficacy was optimal in 88%, good in 4% and modest in 2 cases (8%) of the entire number of cases while the tolerability has been optimal in all the cases. The Authors conclude the retaining that MF/D/80 is a product that can be used with enormous advantages in chronic constipation for its efficacy, optimal tolerability and for its mechanism of action. The Authors finally underline its usefullness as an "alimentary integrator" in all those diseases (
obesity
, diabetes, atherosclerosis etc.) which among the various etiopathogenetic moments, also count in refined alimentation.

A 13-year-old girl with Prader-Willi syndrome was admitted to our hospital with an 18-month history of anal bleeding and mucus discharge on defecation. Physical examination revealed
obesity
, hypogonadism, hypotonia and hypomentia. On digital examination, a nodular mass was palpated on the right wall of the ampulla recti, which was suspected to be carcinoma on a barium enema study. Proctoscopic examination revealed a large, irregular ulceration with white slough at the base, surrounded by the nodular and lumpy mucosa. The lesion was excised by the abdomino-anal pull-through method. The resected specimen showed a lesion of large, shallow, irregular ulcer, 5.0 x 2.2 cm in size. Microscopic examination revealed obliterated lamina propria by fibroblasts and muscle fibers derived from the muscularis mucosae, and misplaced cystic dilated glands in the submucosa at the margin of the ulcer. The gross and microscopic appearances are identical to those of "solitary ulcer of the rectum" described by Madigan and others, and similar to those of "colitis cystica profunda" described by Goodall and others. According to these findings, this lesion was diagnosed as solitary ulcer of the rectum. In the present report, the relationship between solitary ulcer of the rectum and
colitis
cystica profunda was discussed.

A 15-year retrospective review was undertaken to evaluate the operative outcomes of patients with indeterminate
colitis
who were referred for rectal-sparing operations. Review of 95 consecutive patients operated for ulcerative colitis (UC) or indeterminate
colitis
(IC) revealed characteristics of IC in 13 patients. In the group as a whole, there were 45 females and 50 males; the average age was 33. A total of 64 patients had ileoanal pull-through (IAA). Analysis revealed that four of these patients had IC revealed by findings before operation in three patients and following the first stage of operation in one patient. Three of these four patients have subsequently required permanent ileostomy. Six patients who underwent IAA have subsequently demonstrated signs and symptoms of Crohn's disease (CD). All six have subsequently required ileostomy. Overall 10 patients with CD underwent IAA, and nine have required permanent ileostomy. Fourteen patients had ileorectal anastomosis (IRA) for UC or IC. IRA was performed for patients with IC in nine cases, and five patients with UC elected this operative option. Indications for IRA in patients with UC included
obesity
, 2; mental retardation, 1; advanced age, 1; and patient preference, 1. Of the patients with IC who underwent IRA, two have subsequently shown signs and symptoms of Crohn's disease. Overall, 14 of 14 patients who had IRA still have functioning IRA. None has required ileostomy. The poor results in patients with UC or IC subsequently shown to have CD have caused us to change our operative approach in patients with any question in the diagnosis of UC.(ABSTRACT TRUNCATED AT 250 WORDS)

Restorative proctocolectomy is now the procedure of choice for patients suffering from ulcerative colitis (UC) and familial adenomatous polyposis coli (FAP). The majority of patients with UC require surgery for failure or development of side effects of medical therapy. Other indications include dysplasia, carcinoma and complications from either medical therapy or the disease, such as massive bleeding or perforation. In the adolescent patient, resection may be necessary owing to growth retardation. In patients with FAP the 100% risk of developing colorectal carcinoma can be avoided only by proctocolectomy. Crohn's colitis and carcinoma of the lower rectum represent absolute contraindications. A relative contraindication exists in indeterminate
colitis
, patients with poor sphincter function, mesenteric desmoids,
obesity
and an age above 65 years.

We describe in two brothers an apparently novel syndrome comprising
obesity
, congenital hypothyroidism, neonatal
colitis
, cardiac biventricular hypertrophy, craniosynostosis, and developmental delay. The first brother presented with neonatal
colitis
and congenital hypothyroidism and died at age 5 weeks of fulminant
colitis
. The second brother presented neonatally with the same condition, but survived and subsequently developed severe
obesity
, sagittal and coronal synostosis, and developmental delay. Both pregnancies had been complicated by hemolysis, elevated liver enzymes, and low platelets (HELLP syndrome). Exhaustive genetic and metabolic investigations have failed to provide a unifying pathogenesis. This unique combination of manifestations appears to represent a new syndrome with probable autosomal recessive or X-linked recessive inheritance.

Chronic inflammation and increased visceral adipose tissue (VAT) are key elements of the metabolic syndrome. Both are considered to play a pathogenic role in the development of liver steatosis and insulin resistance. The aim of the present study was to investigate the hypothesis that an inflamed intestine, induced both by diet and chemical irritation, could induce persistent inflammation in VAT. Female C57BL/6JOlaHsd mice were used. In study I, groups of mice (n = 6 per group) were given an
obesity
-inducing cafeteria diet (diet-induced
obesity
) or regular chow only (control) for 14 weeks. In study II,
colitis
in mice (n = 8) was induced by 3% dextran sulfate sodium in tap water for 5 days followed by 21 days of tap water alone. Healthy control mice (n = 8) had tap water only. At the end of the studies, all mice were killed; and blood and tissues were sampled and processed for analysis. Body weight of diet-induced obese mice was greatly increased, with evidence of systemic inflammation, insulin resistance, and liver steatosis. Tissue inflammation indexed by proinflammatory cytokine expression was recorded in liver, mesenteric fat, and proximal colon/distal ileum, but not in subcutaneous or perigonadal fat. In dextran sulfate sodium-induced
colitis
mice, mesenteric fat was even more inflamed than the colon, whereas a much milder inflammation was seen in liver and subcutaneous fat. The studies showed both diet- and
colitis
-initiated inflammation in mesenteric fat. Fat depots contiguous with intestine and their capacity for exaggerated inflammatory responses to conditions of impaired gut barrier function may account for the particularly pathogenic role of VAT in
obesity
-induced metabolic disorders.
