We report 10 patients with primary hypoparathyroidism. Age at onset varied from 7 months to 52 years (mean 28); 7 were female. Diagnosis was established at a mean of 4.1 years after the appearance of clinical manifestations. Unexplained hypocalcemia (mean 5.3 mg/dl) and hyperphosphatemia (mean 6.4 mg/dl) were present in all patients. Prevalent symptoms included
tetany
(9 patients), seizures (5) and hypocalcemic cataracts (4). Clinical manifestations may be grouped into 5 types 1)
tetany
; 2) seizures; 3) other neurologic disorders (basal ganglia calcification, pseudotumor of the brain, ataxia,
nystagmus
, hypertonus, paresis); 4) disorders of the lens including fully developed cataracts and 5) skin alterations like psoriasis and others. Some of these run on acute course (seizures,
tetany
), others a subacute one (skin alterations) while others are rather chronic (cataract and other neurologic disorders). Seizures and electroencephalographic disorders predominate in younger patients while
tetany
is more prevalent in older subjects.

An electronystagmographic study of optokinetic reactivity is conducted in 50 'normal' adult subjects and 200 patients suffering from various diseases, all of whom presented an abnormal electronystagmogram. Optokinetic reactivity is evoked by a sudden acceleration of the optokinetic stimulus. It is considered normal when this acceleration leads to an increase in amplitude. At the same time, the frequency of the optokinetic
nystagmus
decreases. This type of response can be modified by various disorders. Recent cranial trauma and
tetany
are associated with an incomplete or inadequate optokinetic response; brain vascular diseases and disorders of central origin induce an absence of that response, while some cases of multiple sclerosis and neuro-allergies give rise to an apparently momentary, unstable response. Optokinetic reactivity thus represents an extension of the classical optokinetic test and yields valuable diagnostic clues.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is an autosomal recessive disorder caused by mutations in the CLDN16 or CLDN19 genes, encoding claudin-16 and claudin-19 in the thick ascending limb of Henle's loop. In patients with claudin-19 mutations, severe ocular involvement (macular coloboma, pigmentary retinitis,

, or visual loss) has been described. In this report, we presented a 12-year-old girl with rickets, polyuria, and polydipsia. She was the daughter of consanguineous parents, and she had a history of recurred hypocalcemic and hypomagnesemic
tetany
. On physical examination, bilateral horizontal
nystagmus
and severe myopia were detected. Laboratory examination revealed hypomagnesemia, hypocalcemia, hypercalciuria, nephrocalcinosis, and renal stone. A clinical diagnosis of FHHNC caused possibly by claudin-19 mutation was decided with the ocular findings. DNA analysis revealed a novel homozygous missense mutation c.241C>T in the CLDN19 gene. In conclusion, in a patient with hypomagnesemia, hypercalciuria, nephrocalcinosis, and ocular findings, a diagnosis of FHHNC caused by claudin-19 mutation should be considered. This is the first study of FHHNC in Chinese population. Our findings of the novel mutation c.241C>T in exon 2 add to the list of more than 16 mutations of CLDN19 gene reported.
