The use of estrogen assays in monitoring fetoplacental function was discussed. In early pregnancy estrogen plus chorionic gonadotropin and progesterone or pregnanediol levels give a picture of the hormonal events. Important reasons for these assays are suspected, threatened, missed, or recurrent abortion, mole pregnancy, error in dates, suspected early pregnancy, ectopic pregnancy, or the presence of an abdominal mass. Estriol assay is the best test for monitoring fetoplacental function in late pregnancy. It was found that the perinatal mortality rate was around 9% when estriol values were low compared with 1.8% in the normal estriol group. Falling estriol levels are a poor prognosis and rising values are a good prognosis. Cortisone and ampicillin can depress urinary estriol excretion. Mandelamine destroys estriol during the assay. Plasma estrogen assays are also available; however, further work is necessary to determine which fraction is most relevant in assessing fetal risk.
