The malignant transformation of a plantar nevus is presented. Perilesional vitiligo developed concurrently with the onset of malignancy. The primary lesion was controlled surgically, but the patient succumbed to metastiatic melanoma within a year. The relation of perilesional vitiligo in malignant melanoma to the so-called halo nevus, and the immunologic significance of these discolorations is reviewed.

A large series of 211 Spitz nevi is reviewed. 30% of the lesions were from patients 20 years of age and over. The trunk and lower extremity were most commonly involved. There were no significant histologic differences between cases from adults and children. Features which may help in differentiating atypical Spitz nevi from malignant melanoma include the presence of some nevus cell maturity at the base, an absence of atypical mitoses, no significant upward epidermal spread and the nuclear chromatin pattern.

Failure to appreciate the variability in the natural course of choroidal nevi (benign melanomas) has caused the adoption of a misleading cytologic classification of malignant melanomas. This classification is not based on adequate clinical observation, reliable cytologic criteria for malignancy, or sufficient autopsy information. The available evidence suggests that spindle A tumors are benign and that spindel B tumors, although capable of local destruction of the eye, metastasize infrequently. The risk of observation of small melanocytic tumors must be weighed against the risk that early enucleation or other forms of therapy may not favorably affect prognosis. The clinical observations to date at the Bascom Palmer Eye Institute indicate that the risks of observation are low.

A case of unilateral nevus of Ota was associated with ipsilateral ocular melanocytosis and contralateral malignant melanoma of the CNS. The malignant neoplasm may have arisen from an area of bilateral leptomeningeal melanosis associated with unilateral nevus of Ota.

A man born with a unilateral epidermal naevus on the right side of the abdomen developed at puberty symmetrical benign acanthosis nigricans in the body folds. The symmetrical eruption later disappeared completely. The unilateral epidermal naevus and the symmetrical eruption showed identical histological features, i.e. those of acanthosis nigricans. At the age of 32 he developed a muco-epidermoid cancer of the left parotid gland. A decision whether the association of unilateral epidermal naevi with malignant tumours occurs with unusual frequency requires (a) a long follow-up of the patient with the unilateral naevus, (b) a differentiation of the unilateral eruption from unilateral malignant acanthosis nigricans and (c) statistical evidence.

This study was carried out as a step toward clarifying the hormone responsiveness of mammary tumors in the F1 hybrid mice of the GR/A and SHN strains. Mammary tumors in SHN mice are pregnancy-independent while mammary tumors of GR/A mice are pregnancy-dependent. Females of each strain were mated with males of the other strain to produce F1 hybrids. They were subjected to forced breeding without subsequent lactation. Only animals with palpable mammary tumors during 2nd and 4th pregnancies were used. Mice received sc injections of several hormones, singly or in combination, daily from 1 day after parturition, i.e., .5 mcg of estradiol benzoate (EB) and of 100 mcg human placental lactogen (HPL). The dose of progesterone (P) before parturition was 100 mcg and after parturition 1 mg. A group received grafts of isologous pituitary glands under kidney capsules (3 AP) during Days 12-34 of pregnancy. 1 day after parturition each mouse was given a single ip injection of 50 micro Ci tritiated-thymidine (tritiated-TdR; 5 Cim/mole) along with the last injection of hormones. Animals were killed 2 hours later. Cyclic SHN mice with mammary tumors received tritiated-TdR, after 2 daily injections of hormones, for 3 days, or after 7-8 days of 3AP. The index of DNA synthesis was determined by a liquid scintillation countermeasuring tritiated-TdR incorporated with DNA. Some of the mice were bled 1 day after parturition and plasma prolactin assayed. Hormone responsiveness was similar among GR/A and both F1 hybrids in either normal or neoplastic glands. DNA synthesis of mammary tumors did not respond to any hormone used but normal glands were stimulated by the hormones. EB injection or 3AP did not alter tritiated-TdR incorporation. HPL injection stimulated DNA synthesis of normal glands but not of tumors. P significantly increased the synthesis of both normal and neoplastic mammary glands. EB and HPL injected with P also significantly promoted DNA synthesis of tumors, but did not increase synthesis in normal glands cuased by P. Plasma prolactin levels were significantly increased by EB injection or by 3AP but were not altered by P. In SHN mice DNA synthesis in mammary tumors was not altered by any hormone treatment but synthesis in normal glands was increased by all treatments. Findings suggest that the hormone-responsiveness of pregnancy-dependent mammary tumors in GR/A and its F1 hybrids is different from some other mammary tumors in mice and rats. Progesterone was more important than prolactin or estrogen for tumor growth. Although EB injection was without effect, intact mice were used so some endogenous estrogen may have participated in the effect. While hormone responsiveness of mammary tumors was different between GR/A and SHN mice, the tumors in both hybrids (BR/A with SHN F1 and SHN with GR/A F1) showed responses quite similar to those of tumors of GR/A mice. This suggests the dominance of the GR/A genotype over the SHN genotype for determining the biological characteristics of mammary tumors.

Tumorous amyloid deposits in the neck region of a 31-year-old man caused difficulties in the preoperative differential diagnosis of a tumorous process of the thyroid gland. Several tumorous amyloid deposits compressing and eroding the left nevus vagus and having no connection to the trachea or thyroid gland were removed. Such isolated tumorous amyloid deposits in the neck region have not been described before. Histologically no thyroid tissue was found. A medullar cancer of the thyorid gland that has to be excluded in such cases was not found.

The principles and practice of nitrosamine degradation by photochemical means were described. Two types of apparatus were constructed for this purpose. A nitrosamine waste solution containing 10-3-10-1 N in hydrochloric acid and about 10 mole-equivalents of an HNO2 scavenger (i.e., urea, guanidine, or hydrazoic acid) was efficiently and irreversibly degraded to the parent amines and N2 (and/or N2O) by irradiation. Efficiency of the degradation depended on the type of apparatus used. With the use of one apparatus, about 11 g N-nitrosodimethylamine was photolytically converted to dimethylamine, N2, and N2O in 24 hours. Protonated guanidine catalyzed the photohydrolysis processes.

N6-(delta2-isopentenyl)adenosine was found both as a component of tRNA and as the cytoplasmic mononucleotide in human leukemic lymphoblasts and myeloblasts from peripheral blood and bone marrow samples. This hypermodified nucleotide was also found in the tRNA and as a mononucleotide in human (MRC-5 and KB) and mouse (A9, FLV, LM, and RAG) cell lines. The relative amounts of this hypermodified nucleotide in the tRNA of the cell lines and the human leukemias were similar (the mean value being 0.06 +/- 0.03 mole % of the total tRNA nucleotide content); whereas the amounts occurring as the free cytoplasmic mononucleotide were more varied but still comparable (the mean value being 0.53 +/- .09 mole % of all cytoplasmic nucleotides) for all cells investigated with the notable exception of all normal, diploid cell lines under study (0.04 mole%). A possible relationship of the free cytoplasmic mononucleotide with the nucleotide in the tRNA for control of mammalian cell protein synthesis in vivo was investigated by addition of N6-(delta2-isopentenyl)adenosine to the culture medium. The exogenously added nucleoside caused inhibition of cell growth within 3 h and cell death within 36 h at concentrations as low as 0.4 muM. No comparable effects were seen when adenosine, adenine, or N6-(delta2-isopentenyl)-adenine were added to the cultures. The simultaneous presence of adenosine in cultures containing N6-(delta2-isopentenyl)adenosine did not alter the detrimental effects of the hypermodified nucleoside on cell growth even when the concentration of adenosine was 50-fold that of N6-(delta2-isopentenyl)adenosine. Addition of N6-(delta2-isopentenyl)adenosine to cell cultures caused within the first 6 h a significant reduction in the rates of RNA and protein synthesis; whereas DNA synthesis continued at a rate comparable to control and adenosine-treated cells for 18 h before decreasing.

Gestational trophoblastic malignancy, although rare, offers the clinician the opportunity of being able to cure a malignant disease with cytotoxic chemotherapy. Such an opportunity is fulfilled only when the progress of the post-molar patients is monitored and chemotherapy instituted as soon as the malignant transformation of the mole is documented. The management and treatment of nine cases is presented and discussed.
