Two horses were presented with lethargy, weight loss,
anorexia
, and swelling of the limbs and ventral body wall. One horse, a 12-month-old American Paso Fino colt, also had acute abdominal pain. The other horse, a seven-month-old Tennessee Walking Horse (TWH) filly passed diarrheic stools during the initial examination. Each horse had low serum protein,
neutropenia
, and a normal packed cell volume (3.2 g/dl, 1300 cells/ul, and 38%, respectively, for the colt, and 2.4 g/dl, 696 cells/ul, and 44%, respectively for the filly). After intravenously administering plasma, the colt's PCV dropped to 23%, and the filly's dropped to 30%. During exploratory surgery, 3.5 and 2.0 meters of thickened terminal small intestine were removed from the colt and filly respectively, and a jejunocecostomy performed. The results of histologic examination of resected intestine were consistent with a diagnosis of equine granulomatous enteritis (EGE). Both horses showed clinical improvement within two days after surgery. The colt developed a neutrophilia (20,500 cells/ul) within 24 hours of surgery. Serum protein concentrations remained stable and gradually elevated to normal or near normal values of 7.0 g/dl (colt) and 5.8 g/dl (filly) by two weeks. The colt was killed four months after surgery because of signs of abdominal pain. Postmortem examination revealed a small intestinal volvulus associated with an adhesion. The TWH filly remains clinically normal 13 months after surgery.

The mortality rate in the experimentally infected cattle was 17 per cent. The similarity of Jembrana disease to malignant catarrhal fever and to diseases of cattle associated with Ehrlichia is discussed.

Fifteen dogs with relapsed lymphoma were treated with doxorubicin and dacarbazine (ADIC) to reinduce remission. All the dogs' lymphomas had become resistant to prior therapy with doxorubicin alone. Five of the 15 dogs had a complete response to the first treatment with ADIC, and three had partial responses. Of the eight dogs receiving a second cycle, two had complete responses, and one had a partial response. One dog that received a third ADIC treatment no longer responded. The median survival time from the first ADIC treatment for all dogs was 45 days (range, 18-241 days). The five dogs having complete responses to the first ADIC treatment had a median survival time of 105 days (range, 45-241 days) after this treatment. Toxicity due to ADIC treatment was acceptable and did not exceed that seen when doxorubicin was given as a single agent. The treatment resulted in severe
neutropenia
in three dogs. One dog died due to neutropenic sepsis. Vomiting, diarrhea, and
anorexia
occurred, but were tolerable, resulting in hospitalization in only one instance. ADIC is apparently a useful chemotherapeutic combination to reinduce remission in some dogs with relapsed lymphoma.

Based upon in vitro and in vivo synergistic activity of Type I and Type II interferons (IFNs) in preclinical in vitro and in vivo studies, we initiated a phase I trial evaluating the doses, safety, and pharmacokinetics of combinations of recombinant DNA-produced human IFN-beta ser and IFN-gamma in 27 patients with cancer. Twenty-four patients were treated with a 2-hour infusion of IFN-gamma, followed by a 10-minute iv injection of IFN-beta ser, three times a week. Patients were entered on fixed dose levels of 1 X 10(6), 3 X 10(6), 10 X 10(6), 30 X 10(6), and 100 X 10(6) units of each IFN. In addition, three patients were treated at the highest dose level with a 10-minute iv infusion of IFN-gamma and a 10-minute iv infusion of IFN-beta ser. The maximally tolerated dose when administered by this schedule for greater than or equal to 4 weeks was 30 X 10(6) units of each IFN. Dose-limiting side effects at doses of 100 X 10(6) units of each IFN consisted of fatigue, nausea, vomiting,
anorexia
, paralytic ileus, and
neutropenia
. The most common side effects at the three highest dose levels were fever, rigors often requiring parenteral meperidine, and constitutional symptoms. Reversible elevations in SGOT and LDH were also noted. Serum IFN levels were dose related, with peak titers occurring immediately after IFN administration. One patient with a nodular mixed lymphoma had a partial response which has been sustained for over 1 year. We conclude that combinations of IFN-beta ser and IFN-gamma can be safely administered on a chronic basis without enhanced or cumulative toxic effects.

Children undergoing ABMT, a procedure which entails massive doses of chemotherapy along with total-body irradiation, are candidate to develop severe gastrointestinal toxicity and prolonged
anorexia
requiring administration of Parenteral Nutrition (PN) for variable periods. We report a series of 35 consecutive children affected by malignancies who underwent 37 courses of PN after ablative therapy followed by ABMT. Age ranged from 8 months to 17 years; 16 were females, 19 males. There were 23 cases of neuroblastoma, 5 of Wilms' tumor, 3 of acute myelogenous leukemia, 2 of Ewing's sarcoma, 1 case each of rhabdomyosarcoma and acute lymphoblastic leukemia. All patients developed severe
neutropenia
for 9-42 days (median 18 d). Fever occurred in all patients; sepsis was documented in 10. Duration of PN ranged from 10 to 64 days (23 +/- 9; mean +/- SD). PN solution, containing crystalline L-Aminoacids (8.5%) mixed with 33% glucose, minerals, trace elements and vitamins provided for children a caloric intake of 49.8 +/- 17.3 Kcal/Kg/day with a nitrogen intake of 0.26 +/- 0.27 g/Kg/day. Nutritional assessment, utilizing percent ideal body weight, serum protein electrophoresis, C3, pseudocholinesterase and fibrinogen, was performed at the beginning and at the completion of each course of PN. Mean percent ideal body weight was 95.8 before PN, 98.5 on last day of PN (p less than 0.0005). Other parameters did not change significantly. No metabolic complication nor severe electrolyte imbalance were observed except for 5 patients who developed hypokalemia in coincidence with administration of Amphotericin B.(ABSTRACT TRUNCATED AT 250 WORDS)

(0-0.6 X 10(9) neutrophils/l) was documented during a 10-month period in a 4-year-old spayed female domestic shorthair cat that was presented for
anorexia
and depression. Salient abnormalities detected on physical examination were fever (40.3 degrees C), dehydration, and gingivitis. The cat was neutropenic (0.5 X 10(9) neutrophils/l) and enzyme-linked immunosorbent assay (ELISA) test for feline leukemia virus was negative. A bone marrow aspirate showed decreased numbers of mature granulocytic cells. In vitro bone marrow cultures for colony-forming units-granulocyte/macrophage (CFU-GM) were performed comparing bone marrow from the patient with that of a normal cat. The patient had fewer CFU-GM than the control. The number of CFU-GM increased when bone marrow mononuclear cells were cultured in the presence of 10(-5) and 10(-6) mol/l of hydrocortisone, but the cat did not respond to oral prednisolone therapy. The pathogenesis of the
neutropenia
in this cat remains obscure, but resembles the chronic idiopathic
neutropenia
syndrome of man.

The effect of oral infection of puppies, eight and 10 weeks old, with canine parvovirus of faecal origin was studied. Clinical signs of enteric disease were first apparent at five days after inoculation and persisted during days 6 and 7 after inoculation. The severity of clinical signs varied from transient dullness and
anorexia
to emesis, dysentery and death. Changes in haematological parameters were first found at day 3 after inoculation when a relative lymphopenia was observed. A profound
neutropenia
developed in severely affected dogs after the appearance of clinical enteric disease. Post mortem examination revealed thymic atrophy in all dogs killed on day 4 after inoculation. Macroscopic changes in the small intestine were apparent only in animals examined during the phase of severe enteric disease and consisted of thickening, rigidity and congestion of the small intestines. Microscopically there was lymphocytolysis in the thymic cortex and the germinal centres of the lymph nodes from days 2 and 3 after inoculation respectively and this rapidly resulted in depletion of these tissues. There was repopulation of lymph nodes from day 7 after inoculation but significant thymic regeneration was not apparent during the course of this study. In the small intestine, necrosis of crypt epithelium, atrophy of villi and, in some areas, complete collapse of mucosal architecture were found but the extent of these changes varied along the length of the small intestine and between individuals. Regenerative intestinal changes were observed in those animals surviving the acute phase of enteric dysfunction. The variable severity of clinical and enteric lesions, together with the factors which may affect the expression of clinical disease, are discussed.

precluded dose escalation above 1.5-2.0 mg/m2 per day, the maximum tolerated dose (MTD). The daily x5 schedule has been developed further with dose escalation using granulocyte-colony-stimulating factor support in patients who have kidney or liver dysfunction and given in combination with cisplatin. In addition, a phase I trial of topotecan given as a 5-day continuous intravenous infusion to patients with refractory leukemia has had promising antileukemic responses. A separate series of in vitro studies indicates that a modest degree of resistance to the cytotoxicity of topotecan can be mediated by P-glycoprotein. A phase I and pharmacology study of irinotecan given as a 90-min infusion every 3 weeks has defined an MTD of 240 mg/m2, with dose escalation being limited by several toxicities. These included an acute treatment-related syndrome of flushing, warmth, nausea, vomiting, and diarrhea; a subacute combination of nausea, diarrhea,

, and weight loss; and/or

. Antitumor activity has been observed with topotecan and irinotecan in patients with a variety of solid tumors and refractory leukemia in our studies, which supports the widespread enthusiasm for this group of compounds.

Preclinical and preliminary clinical data suggested a potentiation of the cytotoxic activity of cisplatin (CDDP) by interferon-alpha (IFN-alpha) in non-small cell lung cancer (NSCLC). This open, non-randomised, phase II study was set up to determine the response rate, duration of response, survival, safety and tolerability following treatment with the combination of recombinant IFN-alpha 2a and CDDP in NSCLC. 100 previously untreated patients with unresectable, measurable or evaluable stage III/IV NSCLC were enrolled for treatment with a combination of IFN-alpha 2a (9 MIU three times weekly) and CDDP (100 mg/m2 every 28 days). Patients were stratified according to histology, i.e. squamous cell carcinoma versus non-squamous cell carcinoma. The planned maximum treatment duration was 6 months or until disease progression. Responding patients could be treated with IFN-alpha 2a as maintenance for an additional 6 months. To be evaluable, the patients must have received at least 2 weeks of treatment with IFN-alpha 2a and at least one dose of CDDP. There were 75% male and 25% female patients with a mean age of 59 years (range 34-74). An overall response rate of 33% (95% confidence interval (C.I.) = 23-44) was achieved among the 84 evaluable patients. There was one complete responder and 27 partial responders, while 32 patients had stable disease. The duration of partial response ranged from 3 to 12 months. The median survival was 6.4 (95% C.I. = 5.7-8) months. The response rate in patients with stage IIIa disease (45%) was significantly higher (P = 0.047) than in patients with stage IV disease (22%). The median survival in patients with stage IIIa disease (9.3 months) was significantly longer (P = 0.025) than patients with either stage IIIb (6.3 months) or stage IV disease (6.2 months). The major forms of toxicity were gastrointestinal and constitutional symptoms of mild to moderate severity. The main severe adverse events (WHO grade 3-4) were nausea and vomiting (32%),
anorexia
(16%) and fever (11%). The most frequent severe haematological toxicities (WHO grade 3-4) were
neutropenia
(27%), anaemia (18%) and thrombocytopenia (10%). 13 patients experienced WHO grade 3-4 renal toxicity. This study confirms the antitumor activity of the combination of IFN-alpha 2a and CDDP in NSCLC. Further study of this combination is warranted.
