Aspergillus sinusitis is an uncommon complication of advanced human immunodeficiency virus (HIV) infection. We describe 2 patients with AIDS who developed histologically proven invasive Aspergillus sinusitis. We also review the findings of 14 histologically documented and 5 probable cases of invasive Aspergillus sinusitis. The literature on the prevalence, predisposing factors, diagnosis, treatment, and prognosis of the infection is reviewed. Major risk factors for the disease are advanced AIDS,
chronic sinusitis
or otitis,
neutropenia
, use of corticosteroids and prolonged use of broad spectrum antibiotics. The most common presenting symptoms are nonspecific and include fever, local pain, and swelling. Despite the newer diagnostic and therapeutic approaches discussed herein, the infection is usually fatal in HIV-infected patients. Early diagnosis and aggressive treatment remain the only available means to improve the currently dismal prognosis of Aspergillus sinusitis.

The still present problem of choice of a treatment method in
chronic sinusitis
was introduced in this study. The heterogenous etiology of
chronic sinusitis
and its treatment consequences were presented. The main current options in the treatment of
chronic sinusitis
were shown. The functional endoscopic sinus surgery was emphasized as the most important. The different courses of antibiotic therapy: nebulization, oral and intravenous were introduced as a method of choice in exacerbation of chronic process. The intravenous antibiotics were analyzed in details and it was especially based on the bacterial biofilms theory according to the newest literature. The obtaining results were good enough. They were better in children than in adults and in patients with shorter history of sinusitis, however they were not free from complications. Thrombophlebitis, allergic reaction,
neutropenia
were indicated as most frequent complications that caused the break in the therapy. In the conclusion the intravenous antibiotic therapy was estimated as a useful procedure, but not highly recommended due to a possibility of complications.

A 56-year-old man with bilateral hearing impairment who had taken betamethasone combined with dexchlorpheniramine maleate for 7 years to treat
chronic sinusitis
developed a dry cough after discontinuing this medication and was diagnosed with asthma, and after which he sensed impaired bilateral hearing. Based on the presence of numerous eosinophilic leukocytes in otorrehea, we made a diagnosis of eosinophilic otitis media, and he was prescribed predonisolone to control the asthma, but discontinued it on his own. He then developed fever, maniphalanx stiffness, testicular pain, and facial hyperesthesia, eruptions, and the lower-limb numbness. The detection of a positive serum reaction for MPO-ANCA and evaluated of eosinophilic leukocyte levels yielded a definitive diagnosis of CSS, for which the man was treated with predonisolone and cyclophosphamide. His symptoms were relieved, even though the onset of
neutropenia
, necessitated the discontinuation of cyclophosphamade administration.
