Salmonellosis in horses may result in fever, anorexia, and depression without concurrent diarrhea or other obvious gastrointestinal abnormalities and should be considered in cases of fever of unknown origin. The syndrome also is characterized by neutropenia, usually with a left shift, and growth of small numbers of salmonella from feces cultured in selenite enrichment broth. Repeated culturing may be necessary to isolate the organism. All six affected horses of this report recovered in 3 to 7 days without specific therapy.

Colitis due to salmonellae was diagnosed in 9 horses following hospitalization for various reasons at the University of Missouri Veterinary Teaching Hospital, from May, 1971, to April, 1972. Diarrhea, fever, and either a neutrophil count of less than or equal to 3,600/cmm or a rapid decline in neutrophil numbers were specific for salmonellosis. The value of hematologic survelillance in hospitalized Equidae was demonstrated in another group of 9 horses with neutropenia, each of which was promptly treated and did not develop colitis. Bacteriologic culturing of fecal samples from 28 clinically normal horses yielded only 2 salmonella isolations, S manhattan in each case. The serotyped salmonellae isolated from the patients with colitis were all S typhimurium, with similar antibiotic resistance patterns.

Seventeen ponies were infected with Salmonella typhimurium and then 15 were variously stressed by transportation and/or surgery and 9 were given oxytetracycline. Indications of Salmonella reactivation occurred in all the stressed ponies. Diarrhea due to a reactivation of the Salmonella infection did not develop until greater than 3 days after stress, although maximal shedding of organisms occurred within 24 hours. A neutropenia generally occurred within 24 hours after stress and lasted about 5 days. A rectal temperature greater than 39 C usually did not occur. An increase in serologic titer was noticed in about half of the ponies. Transportation had a major role in reactivating the Salmonella infection, and 1 pony died of peracute colitis. The use of oxytetracycline prolonged the excretion of Salmonella; therefore, this drug should not be used after stress, particularly transportation, in ponies that have diarrhea or are known to be Salmonella carriers.

In 6 dogs with multicentric lymphosarcoma, salmonellosis developed shortly after the start of anticancer chemotherapy. Three of the dogs died. Signs of illness in those that died were vomiting, diarrhea, neutropenia, anorexia, and fever, usually within 3 days of the start of chemotherapy.

Macrophages are considered to be the mediators of resistance to extra-intestinal Salmonella infections. Nevertheless, the initial cellular response to Salmonella infections consists primarily of polymorphonuclear leukocytes (PMN). To determine whether PMN serve an important function for the infected host, we made mice neutropenic with the rat mAb to RB6-8C5 and infected them i.v. with approximately 10(3) Salmonella dublin or an isogenic derivative that lacks the virulence plasmid (LD842). We infected BALB/c mice, which have a point mutation in the macrophage-expressed gene Nramp1 that makes them susceptible to Salmonella, and BALB/c.D2 congenic mice, which have the wild-type Nramp1 gene that makes them resistant to Salmonella. Both mouse strains were resistant to LD842, and neutropenia made only the BALB/c strain susceptible to this infection. Neutropenic congenic mice, however, were susceptible only to wild-type S. dublin (plasmid+). These results show a complex interplay between plasmid-virulence genes in Salmonella, host macrophages, and PMN. Mice with normal macrophages need PMN to defend against nontyphoid Salmonella that carry a virulence plasmid but not against Salmonella without virulence plasmids. Mice with a mutant Nramp1 gene need PMN to defend against all Salmonella, even those that lack virulence plasmids. These results, plus the evidence that PMN kill Salmonella efficiently in vitro, suggest that Salmonella have adapted to grow inside macrophages where they are relatively sheltered from PMN. The adaptations that allow Salmonella to survive in macrophages do not protect them from PMN.

An increased frequency of nontyphoidal salmonellosis is well established in cancer patients, but it is unclear whether this represents increased susceptibility to exogenous infection or opportunistic, endogenous reactivation of asymptomatic carriage. In a retrospective study, a simple case definition was used to identify the probable presence of reactivation salmonellosis in five cancer patients between 1996 and 2002. Reactivation salmonellosis was defined as the development of nosocomial diarrhea >72 h after admission and following the administration of antineoplastic chemotherapy in an HIV-seronegative cancer patient who was asymptomatic on admission, in the absence of epidemiological evidence of a nosocomial outbreak. Primary salmonellosis associated with unrecognized nosocomial transmission or community acquisition and an unusually prolonged incubation period could not entirely be ruled out. During the same time period, another opportunistic infection, Pneumocystis pneumonia, was diagnosed in six cancer patients. Presumably, asymptomatic intestinal Salmonella colonization was converted to invasive infection by chemotherapy-associated intestinal mucosal damage and altered innate immune mechanisms. According to published guidelines, stool specimens from patients hospitalized for longer than 72 h should be rejected unless the patient is neutropenic or >or=65 years old with significant comorbidity. However, in this study neutropenia was present in only one patient, and four patients were <65 years old. Guidelines should thus be revised in order not to reject stool culture specimens from such patients. In cancer patients, nosocomial salmonellosis can occur as a chemotherapy-triggered opportunistic reactivation infection that may be similar in frequency to Pneumocystis pneumonia.

The outcome of infection with Salmonella Typhimurium in mouse models of human typhoid fever is dependent upon a coordinated complex immune response. A panel of recombinant congenic strains (RCS) derived from reciprocal backcross of A/J and C57BL/6J mice was screened for their susceptibility to Salmonella infection and two susceptibility loci, Ity4 (Immunity to Typhimurium locus 4) and Ity5, were identified. We validated Ity5 in a genetic environment free of the impact of Ity4 using a cross between A/J and 129S6. Using a time-series analysis of genome-wide transcription during infection, comparing A/J with AcB60 mice having a C57BL/6J-derived Ity5 interval, we have identified the differential expression of the positional candidate gene Cd40, Cd40-associated signaling pathways, and the differential expression of numerous genes expressed in neutrophils. CD40 is known to coordinate T cell-dependent B-cell responses and myeloid cell activation. In fact, CD40 signaling is altered in A/J mice as seen by impaired IgM upregulation during infection, decreased Ig class switching, neutropenia, reduced granulocyte recruitment in response to infection and inflammation, and decreased ERK1/2 activity. These results suggest that altered CD40 signaling and granulocyte recruitment in response to infection are responsible for the Ity5-associated Salmonella susceptibility of A/J mice.
