Radiolabeled m-iodobenzylguanidine (MIBG) is a tumor-seeking radioactive drug used in the diagnosis and treatment of pheochromocytomas and neuroblastomas. It is transported into the tumor cells by the neuronal norepinephrine (NE) transporter (NET) which is expressed in almost all neuroblastoma cells. Here, we describe the synthesis and some pharmacological properties of a series of fluorescent compounds structurally related to the NET substrate, MIBG, or to the NET inhibitors, (-)-(2R,3S)-cocaine and nisoxetine. Three of 10 synthesized fluorescent compounds, 1-(1-naphthylmethyl)guanidinium sulfate (1), 1-[2-(dibenz[b, f]azepin-5-yl)ethyl]guanidinium sulfate (2), and (2R, 3S)-2beta-ethoxycarbonyl-3beta-tropanyl 5-(dimethylamino)naphthalene-1-sulfonate (6), exhibited high affinity (IC(50) about 50 nM) for the NET. The nisoxetine derivatives 8 (rac-N-[(3-methylamino-1-phenyl)propyl]-5-(dimethylamino)-1-naphthale nesulfonamide) and 9 (rac-4-[(3-methylamino-1-phenyl)propyl]amino-7-nitro-2,1, 3-benzoxadiazole) and especially the guanidine derivative 4 (1-[4-(4-phenyl-1,3-butadienyl)benzyl]guanidinium sulfate) which are characterized by intermediate affinity for the NET (IC(50) 370-850 nM) caused significant and nisoxetine-sensitive cell fluorescence. At least the guanidine derivative 4 might represent a potentially useful agent for imaging of neuroblastoma cells.

Clinically, a neuroblastoma presents mostly as an abdominal mass. Within the tumor, bleeding can be present, sometimes extending in to its surroundings. This case report describes a neuroblastoma, presenting as scrotal hematoma in a newborn boy, which initially raised the suspicion of a torsion testis. In this patient, the bleeding descended through the subcutaneous and muscular tissue into the scrotum. In addition, anemia and jaundice were marked features. This clinical manifestation of a neuroblastoma has not been reported previously. Ultrasonography is advised as the first diagnostic step to exclude retroperitoneal origin of the bleeding. Magnetic resonance imaging and meta-iodobenzyl guanidine scanning were helpful tools in the final diagnostic workup.

Studies were conducted using a novel in vitro approach to investigate the efficacy of acetamidine hydrochloride (ACE) and guanidine hydrochloride (GUAN), previously shown to block gramicidin D (GRAM) channels in artificial membranes, in preventing the toxic effects of GRAM in NG108-15 (neuroblastoma x glioma hybrid) cells. Specifically, intracellular microelectrode techniques were employed to examine changes in membrane resting potential (Vm) and input resistance (Rin). At 1 micromol/L, ACE significantly reduced loss of Vm induced by 1 or 10 microg/ml GRAM, although higher concentrations of ACE did not afford enhanced antagonism. GUAN, in contrast, produced a concentration-dependent antagonism of GRAM-induced Vm and Rin loss, with high concentrations (10 or 100 micromol/L) completely preventing diminutions in both Vm and Rin. In control cells superfused without GRAM, ACE produced a direct, concentration-dependent reduction in Vm and Rin, whereas GUAN hyperpolarized NG108-15 cells but did not alter Rin. These data represent the initial demonstration of the reversal of GRAM toxicity in an intact cell system.

Despite intensive chemotherapy, surgery and/or radiation, prognosis continues to be very poor in disseminated neroblastoma. Owing to neuroblastoma sensitivity progress might be achieved with high-dose radiation. Metaiodobenzyl guanidine (MIBG) coupled with 131I provides the opportunity for highly selective radiation treatment of neuroblastoma, which could potentially deliver five to 10 times the dose of conventional external-beam treatment without specific tissue toxicity. To improve the long-term results in advanced-stage neuroblastoma, we integrated this new treatment modality with conventional chemotherapy. Eight neuroblastoma patients refractory to conventional treatment were treated with 131I-MIBG-chemotherapy (vincristine, VP16, iphosphamide, carboplatin, epirubicin, cyclophosphamide) combination. Five of the eight patients responded to 131I-MIBG treatment (2 complete and 3 partial responses). There were also three patients with stable disease. Median survival was 48 months (range 1 to 84 months), and five patients relapsed in their follow-up and died of progressive disease. We concluded that a combined 131I-MIBG and chemotherapy approach is useful in advanced-stage neuroblastoma patients, with considerably less side effects. Although survival is improved when compared to conventional treatment, the overall prognosis is still poor. More lethal radionuclide conjugation to MIBG which will deliver higher tumor and lower critical organ doses may offer the best solution for targeted radionuclide therapy of neuroblastoma.

Excess secretion of any of the adrenal cortical or medullary hormones contributes to a number of well-known clinical syndromes.. They may result from benign or malignant adrenal tumours, adrenal hyperplasia or, least frequently, from extra-adrenal disease. Differentiation among these possibilities is often impossible on clinical or biochemical grounds alone. Location of the site(s) of excess hormone production in the past depended on relatively insensitive or invasive radiological methods. The non-invasive evaluation began with X-ray computed tomography but the functional significance of anatomical abnormalities cannot be determined from CT scan. Incorporation of specific radiopharmaceuticals into the abnormal tissues allows scintigraphic localization of functional abnormalities with a high degree of efficacy. The combination of adrenal scintigraphy and kompjuterizovanom tomografijom CT or magnetskom rezonancijom MRI should in most cases obviatc the need for more invasive procedures. Phaeochromocytoma is rare in hypertensive population, affecting only an estimated of 0.1%. However, a high index of suspicion is essential, since these tumours have potentially life-threatening cardiovascular effects and their successful resection is curative. Important clinical clues include the presence of orthostatic hypotension in an untreated hypertensive, resistance of hypertension to standard therapy (including possible exacerbation by (beta-blockers). In most cases, the diagnosis can be established by demonstrating high levels of free catecholamines and their metabolites (metanephrines and Vanillylmandelic acid). Clonidine test may be important in some cases. The purpose of this study is to point that metaiodobenzylguanidine (mlBG) has proved to be a safe, sensitive and highly specific agent for the location of phaeochromocytoma. The first successful schinigraphic demonstration of phaeochromocytomas in man was reported in 1981, using a new radiopharmaceutical, 131l-metaiodobenzylguanidinc (mlBG). mlBG is an aralkyl-guanidine which structurally resembles noradrenaline sufficiently to be recognized and be stored in the catecholamine storage vesicles. Whereas unstored noradrenaline is rapidly degraded, the halogenated benzyl ring of mlBG conlers resistance to catechol-o-methyltransferase (COMT) while its guanidino side-chain is resistant to monoamine oxidase (MAO). Uptake of mIBG is inhibited by some inhibitors (reserpine, tricyclic antidepressants, cocaine, labetalol, calcium-chanel blockers...). 131I-mlBG is normally taken up by liver, spleen, myocardium and salivary glands. Thyroid uptake ol liberated radioiodide will also occur unless the thyroid is blocked with stable iodide. The normal adrenal glands are usually not seen but faint uptake may be visible 48-72 h after injection in up to 16% of cases. Hepatic uptake is maximal at 24 h, declining to very low levels by 72 h (even more rapid in patients with phaeochromocytoma. Dosimetric corlsiderations limit the amount of 131l-mlBG that is administered for diagnostic studies. This, coupled with the low detection efficiency of gamma cameras for the 364 keV photon of 131l, led to the introduction of 131l-mlBG as an adrenomedullary scintigraphic agent of choice. In our department we started with mIBG scintigraphy in 1985 and we treated near 1000 patients. In this study we are talking about 180 patients from the beginning of 1996 to the end of 2001 all treated with 131l-mlBG. Like the other worldwide experience with this agent our sensitivity was 88.58% and specificity of 98.46%. Positive predictive value was 88.5% and negative predictive value was 93.46%. False negative results were 6.52% and there were no false positive results. After all we can say that mlBG has proved to be a safe, sensitive and highly specific agent for the location of phaeochromocytoma and neuroblastoma. Other radiolabelled aralkylamines have been examined as potential adrenal medullary scintigraphic agents. None has demonstrated superiority over mlBG in animal or limited human studies. 131l-mlBG should always be considered the radiopharmaceutical of choice for imaging purposes if it is available. 131l-mlBG in high doses is successfully used in therapy of malignant phaeochromocytoma and especially in nuroblastoma.

"Transmissible Spongiform Encephalopathies" (TSE) are a group of degenerative progressive fatal disorders of the CNS, affecting both humans and animals. The main pathogenic event is the conversion of cellular prion protein from the normal, enzyme-sensitive (PrPsen), to the insoluble proteinase K-resistant isoform (PrPres). Since the new juvenile variant of Creutzfeldt-Jakob disease (vCJD) is probably due to the transmission of Bovine Spongiform Encephalopathy (BSE) prion protein to man, therapeutic and preventive compounds for animals and humans are urgently needed. Congo Red (benzidine-diazo-bis-1-naphthylamine-4-sulfonic acid sodium salt, CAS 573-58-0, CR), an azoic dye that inhibits amyloid deposition, and some newly synthesized derivatives, more lipophilic and less toxic, were tested for their anti-prionic activity, in different experimental models. Cell-free experiments using the synthetic peptide PrP 106-126, homologous to amino acid residues 106-126 of the human PrP, were run to determine the anti-amyloidogenic properties of some of the molecules. Peptide solutions containing each compound were incubated at 37 degrees C, for increasing times, to analyse the kinetics of aggregation of PrP 106-126 peptide. After incubation, the amount of non-aggregated peptide was measured by RP-HPLC. While CR enhanced the amyloidogenicity of PrP 106-126, derivatives "1a" and "1b" both showed the opposite behaviour, reducing aggregation by 15-20%. In other experiments using electron microscopy PrP 106-126 was assayed with the same molecules to assess the number and size of fibrils formed. CR showed its typical interaction, producing amyloid aggregates, "1a" did not interfere with fibril formation, while "1b" seemed to partially affect the structure of PrP 106-126 fibrils. Using a different cell-free model, it was investigated whether CR derivatives could reverse the protease-resistant PrPres, extracted from Syrian hamster infected brain, into the normal protease sensitive PrPsen. Samples containing fixed amounts of PrPres were incubated at 37 degrees C for 1 h with all the newly synthesized molecules, at concentrations ranging from 50 micrograms/mL to 750 micrograms/mL. After treatment with proteinase K, half of each sample was incubated with 3 mol/L guanidine thiocyanate in order to exclude over-stabilisation of the PrPres aggregates already observed with CR. The remaining amount of PrPres was assessed by Enhanced Chemoluminescence (ECL) Western blotting analysis. None of the compounds induced the reversion of PrPres to PrPsen; nevertheless, 6 of the 8 molecules interacted with PrPres molecules, over-stabilising the PrPres aggregates, from this aspect being similar to CR in activity. Finally, the inhibition of the generation of PrPres in the S12 clone of a mouse neuroblastoma cell line (N2a S12), persistently infected by the mouse adapted Chandler strain of scrapple, was evaluated. Increasing amounts of CR, "1a" and "1b" were added to the culture medium at each cell passage. After various days of treatment, the cells were collected, lysed, and the amount of PrPres was assayed by ECL Western blotting after PK treatment. As expected, there was a decrease in pathological PrP expression starting from the 4th day of treatment, with 5 and 10 micrograms/mL CR; PrPres completely disappeared after respectively 10 and 14 days of treatment. "1a" was strongly effective after 3 days of treatment at 5 and 10 micrograms/mL, but it was also highly toxic; at the concentration of 1 microgram/mL, it had a mild inhibitory effect after 8 days. The reduction of PrPres was also evaluated by intracytoplasmic flow-cytometry immunofluorescence on CR- and "1a"-treated N2a S12 cells. CR induced a dose-related decrease of PrP expression from day 3 to 13 of treatment. At the concentrations of 2 and 1.5 micrograms/mL "1a" also strongly affected the expression of PrP starting from the 3rd day of treatment until the end of the experiment (day 13). These results confirm the importance of using an integrated system, based on different experimental models, to obtain useful information on the mechanism of action of anti-prionic compounds.

To establish the activity of sigma ligands at sigma1 and sigma2 receptor, we chose two tumour cell lines, the human SK-N-SH neuroblastoma and the rat C6 glioma lines, which express sigma2 receptors at a high density and sigma1 receptors in their high-affinity or low-affinity state. We tested the sigma2 receptor agonist PB28 and the sigma2 antagonist AC927, and (+)-pentazocine and NE100 as agonist and antagonist, respectively, at sigma1 receptors, with regard to antiproliferative and cytotoxic effects. In addition, 1,3-di(2-tolyl)guanidine (DTG) and haloperidol were tested as reference compounds displaying nearly equipotent sigma affinity (sigma2>sigma1 and sigma1>sigma2, respectively). In both SK-N-SH and C6 cells, PB28 and NE100 displayed the most potent results both in antiproliferative and cytotoxic assay while AC927 and (+)-pentazocine were inactive in both assays. The cytotoxic and antiproliferative effects of DTG and haloperidol reflected their sigma1 antagonist activity and sigma2 agonist activity. Moreover, our results in the tumour cell lines correlated well with those for sigma2 activity found previously in a functional assay in the guinea-pig bladder. These findings establish a new model for evaluating both sigma2 and sigma1 receptor activity of sigma ligands, which could be useful for developing new ligands having mixed sigma2 agonist/sigma1 antagonist activity as potential antineoplastic agents.

Radioiodinated meta-iodobenzylguanidine (MIBG) is used in the diagnosis and therapy of various neuroendocrine tumors. As a part of our efforts to develop an MIBG analogue with improved characteristics for these applications, a synthesis of 3-[131I]iodo-4-methylbenzylguanidine ([131I]MeIBG) was developed. Unlabeled MeIBG and the tin precursor, N, N'-(bis-tert-butyloxycarbonyl)-N-(4-methyl-3-trimethylstannylbenzyl) guanidine were synthesized in two steps from 3-iodo-4-methylbenzylalcohol. Radioiodinated MeIBG was synthesized at a no-carrier-added level by the iododestannylation of the tin precursor in about 85% radiochemical yield. The accumulation of [131I]MeIBG (38.9+/-3.0% of input counts) by human neuroblastoma SK-N-SH cells in vitro was 87% that of [125I]MIBG (44.5+/-3.0%) and a number of Uptake-1 inhibiting conditions reduced the uptake of both tracers in this cell line to a similar degree suggesting that introduction of a methyl substituent at the 4-position of MIBG did not adversely affect its biological characteristics.

We have developed the NBS (2-nitrobenzenesulfenyl) method, a quantitative proteome analysis method utilizing stable isotope labeling followed by mass spectrometry. The potential of this method was reported previously, and the procedure has now been further optimized. Here, we describe a procedure utilizing urea or guanidine hydrochloride as a protein denaturant, in conjunction with an improved chromatographic enrichment method for the NBS-labeled peptides using a phenyl resin column. By using this new protocol, both sample loss throughout the protocol and the elution of unwanted unlabeled peptides can be minimized, improving the efficiency of the analysis significantly.

A 5-year-old boy presented with headache and hypertension. Screening abdominal ultrasonography showed a 6-cm-sized mass in the left adrenal gland. Laboratory data included urine vanillylmandelic acid/creatinine (208.3 microg/mg), urine homovanillic acid/creatinine (114.3 microg/mg), serum epinephrine (237 pg/mL), and serum norepinephrine (1842 pg/mL). Metaiodobenzyl guanidine I 123 scintigram, abdominal computed tomography, and abdominal magnetic resonance imaging revealed only left adrenal tumor with no metastasis. Left adrenalectomy was performed transperitoneally, and histopathological diagnosis was an adrenal composite pheochromocytoma associated with neuroblastoma. Immunoreactivity for vimentin was positive in pheochromocytoma, which was negative in neuroblastoma. After operation, the symptom of hypertension disappeared. We should examine the identification of composite pheochromocytoma by immunohistochemistry method in patients with neuroblastomatous elements complicated by severe hypertension, and it is important to carefully monitor urine vanillylmandelic acid and urine homovanillic acid in the examination tests and hypertension in the clinical symptoms, and follow up this patient because of some risk of recurrence and metastasis.
