The antigen recognized by a newly produced monoclonal antibody (bra55; IgG1) elicited by the non-T, non-B acute lymphoblastic leukemia cell line REH 6, was expressed on all examined hemopoietic neoplastic cell lines (including non-T, non-B, T, B and myeloid leukemia cell lines), but not on examined nonhemopoietic human tumor cell lines (such as carcinoma, sarcoma, melanoma and
neuroblastoma
cell lines), as demonstrated by indirect immunofluorescence and enzyme-linked immunoassay. Specific immunoprecipitation of 125I-lacto-peroxidase radioiodinated cell surface proteins and sodium metaperiodate/tritiated sodium borohydride 3H-radiolabeled cell surface sialoglycoproteins followed by electrophoretic analysis (SDS-PAGE) demonstrated that the immunoprecipitated antigen is a cell surface 200 kDa sialoglycoprotein (on the non-T, non-B ALL cell line REH 6), with variation in its electrophoretic mobility (in the Mr range of 170,000-210,000) on different examined cell lines. These properties are characteristic for the leukocyte common antigen (
LCA
, T200). Immunoperoxidase staining of several normal and malignant tissues, as well as some nonhemopoietic tumor tissues confirmed the type of antigen tissue distribution pattern characteristic for
LCA
.

Eleven cases of olfactory
neuroblastoma
(ONB) were studied histopathologically, ultrastructurally and immunohistochemically. Light microscopical examination showed the tumor cell nests separated by fibrous connective tissue (present in 10/11 ONB), Homer-Wright rosette and/or Flexner rosette (9/11 ONB) and acidophilic fibrillary background (8/11 ONB). Ultrastructurally, there were neurosecretory granules (2/2 ONB) and neurofibrils (1/2 ONB) in cytoplasm of tumor cells. A panel of antibodies were used to characterize the immunohistochemical staining profile of ONB. The following results were obtained for the 11 neoplasms that were immunostained: NSE 11/11 (+), S-100 3/11 (+), Vimentin 4/11 (+), Keratin 1/11 (+) and NF, EMA, CEA,
LCA
all negative respectively. The significance of the morphological and immunohistochemical features of ONB in diagnosis and differential diagnosis were discussed.

We present 14 patients with primary sinonasal melanomas (SM) identified from 1984-1997 in our archives (11/14 lateral nose, 1/14 nasal septum, 2/14 paranasal sinuses; 8M/6F, mean age 67.7 years, range 39-88 years). Survival was poor (median 9 months) with death related to extensive local disease and/or widespread hematogenous metastases. The following histological subtypes were identified in descending order: amelanotic small blue cell, pleomorphic, epithelioid, spindle cell and myxoid. High mitotic rate and vascular invasion, absence of tumor-infiltrating lymphocytes and regression were features shared by all SM. Negative staining of B- and T-cell markers,
LCA
, neuroendocrine markers such as NSE, chromogranin and synaptophysin, and CK-negativity excluded olfactory
neuroblastoma
, small cell undifferentiated carcinoma, and lymphoma. S-100 protein was expressed in all SM, but demonstrated variable staining intensity with areas of complete negativity. HMB45 was strongly and uniformly (>80%) expressed in all undifferentiated small blue cell SM. The pigmented SM were predominantly HMB45-negative. The strong HMB45 staining in amelanotic small blue cell SM is explained by the reaction of HMB45 antibody with an oncofetal antigen found in immature melanosomes. In these poorly differentiated amelanotic malignant melanomas, antibody to HMB45 proved to be a superb diagnostic marker. We therefore strongly advocate the inclusion of HMB45 antibody in the panel of antibodies for initial work-up of undifferentiated mucosal neoplasms, since a negative S-100 stain in small biopsy material may result in incorrect classification of these neoplasms.

We saw in consultation a biopsy specimen from a 6-year old girl with anaplastic lymphoma kinase (ALK)-positive anaplastic large cell lymphoma (ALCL). The tumor arose in soft tissue of the neck, and diagnostic tissue was obtained by core needle biopsy. Histologically, the neoplasm was cellular without pattern. Immunohistochemical workup with a large panel of antibodies at another institution showed immunoreactivity for NB84 and neuron specific enolase (dim). Antibodies specific for CD3, CD20, and CD45/
LCA
were negative; CD30 or ALK were not assessed. Electron microscopy showed cytoplasmic structures thought to be neurosecretory granules. The neoplasm was interpreted initially as a
neuroblastoma
. At the time of our review, we considered the possibility of ALCL. Immunohistochemical analysis for CD30 showed bright, uniform expression and ALK was positive in a nuclear and cytoplasmic pattern, confirming the diagnosis of ALK+ ALCL. The purpose of this review is to discuss ALK+ ALCL and many of the other entities included under the rubric of small round blue cell tumor, with a focus on tumors that occur in children.

Extrarenal rhabdoid tumor (ERRT) is a rare, aggressive tumor with extremely poor prognosis. We report a case of ERRT with intraspinal extension in a 1.5-year-old child diagnosed by fine needle aspiration cytology (FNAC) and immunohistochemistry. The child presented with a right lumbar region lump of two months duration. Ultrasound guided FNAC was performed and cell block was prepared. Smears were highly cellular and showed a dispersed population of large round cells having abundant pale eosinophilic cytoplasm, centrally to eccentrically placed nucleus with large prominent nucleoli. Immunohistochemistry was carried out on cell block which was positive for epithelial membrane antigen EMA and Vimentin. It was negative for leucocyte common antigen [
LCA
], wilms tumor 1, WT1, desmin and neuron specific enolase NSE, thus ruling out other tumors like lymphoma, Wilms tumor, rhabdomyosarcoma, and
neuroblastoma
. A final diagnosis of ERRT was given. ERRT is an extremely rare tumor of retroperitoneal area; it should be included in the differential diagnosis of malignant round cell tumor in children. Cell block in this case is mandatory for putting up the panel of immunohistochemistry which can clinch the diagnosis of rhabdoid tumor and treatment can be started as early as possible.
