Recent advances in our understanding of the cellular and molecular derangements involved in multiple myeloma are beginning to be translated into novel therapeutic approaches. Growth factors, specifically
interleukin-6
, appear to be critical for disease progression, and interruption of autocrine and paracrine loops has been achieved, with resultant inhibition of myeloma cell growth. Oncogenes,
tumor
-suppressor genes, and cell-survival genes have all been found to be dysregulated in some myeloma patients. The implications of acquisition of the multidrug resistance phenotype are just beginning to be understood. High-dose therapeutic regimens with bone marrow or peripheral stem-cell rescue are being studied in an attempt to produce a cure. Autologous marrow and peripheral blood stem-cell transplantation are better suited to the older myeloma patient population than is allogeneic marrow transplantation, and have also yielded promising results.

The ability of p53 species (wild-type and mutant) to modulate the "differentiated" response of human hepatoma cell lines Hep3B and HepG2 to
interleukin-6
(
IL-6
) was investigated. Transient transfection experiments were carried out in Hep3B and HepG2 cell cultures in which
IL-6
was used to activate a beta-fibrinogen (beta Fib) enhancer/reporter construct containing two copies of the 36-base pair
IL-6
-response element (IL-6RE) (p beta FibCAT). Cotransfection with constitutive expression vectors for wild-type (wt) human or murine p53 inhibited the activation of the p beta FibCAT reporter by
IL-6
in both Hep3B and HepG2 cells. Several mutant p53 species either did not inhibit the activation of p beta FibCAT or up-regulated the response. Hepatoma cell lines stably expressing the Val-135 temperature-sensitive mutant of murine p53 (wt-like at 32.5 degrees C and mutant-like at 37 degrees C) were derived from Hep3B cells and tested for the temperature-sensitive phenotype of their ability to synthesize and secrete fibrinogen and alpha 1-antichymotrypsin in response to
IL-6
. In an experimental protocol in which the parental Hep3B cells did not show a significant difference in plasma protein secretion at the two temperatures, hepatoma line 3 (p53Val-135+) had a greater response to
IL-6
at 37 degrees C than parental Hep3B cells, while line 3 cells had a reduced response to
IL-6
at 32.5 degrees C. Similarly, hepatoma lines 1 and 2 (both p53Val-135+) had reduced
IL-6
responsiveness at 32.5 degrees C, whereas line 22 (transfected with pSVneo alone) and the parental Hep3B cells did not. These data indicate that mutations in p53 contained in
tumor
cells can modulate the "differentiated" response of these cells to cytokines.

The aromatase enzyme complex, which regulates the conversion of androstenedione to estrone, may have an important role in regulating estrogen synthesis in breast tissues. In this study the effect of
tumor
location on aromatase activity in adjacent tissue was examined and related to
interleukin-6
(
IL-6
) production, which has been shown to stimulate aromatase activity in breast cancer cells. Samples of normal and malignant breast tissues were obtained from 11 women undergoing mastectomy. In 7 patients, aromatase activity was highest in the quadrant in which the
tumor
was located or on which the
tumor
impinged. Aromatase activity in
tumor
-bearing quadrants was significantly higher than that in adjacent and opposite quadrants. Aromatase activity and
IL-6
production, expressed in terms of tissue weight, were significantly higher for
tumor
tissue compared with normal breast adipose tissue. A significant correlation was found between aromatase activity and
IL-6
production for breast
tumor
tissue (rs = 0.56; P < 0.05), but not for adipose tissue from the breast quadrants. Aromatase activity and
IL-6
production were also measured in tissue obtained from a normal woman undergoing reduction mammoplasty who had previously had breast augmentation by silicone injection, not contained within a capsule. In tissue from this patient there was evidence of chronic inflammation and a marked macrophage response. Aromatase activity in this tissue was considerably higher than that detected in mastectomy adipose tissue samples, and a significant correlation was found between aromatase activity and
IL-6
production (rs = 0.77; P < 0.05). A preliminary study to examine the potential role of cells of the immune system in regulating breast tissue aromatase activity revealed that conditioned medium collected from macrophages and lymphocytes could markedly stimulate aromatase activity in
tumor
-derived fibroblasts. The results of this study confirmed that breast
tumor
location can influence aromatase activity in adjacent tissues and showed that aromatase activity is increased in
tumor
-bearing quadrants. The increased production of

Inappropriate hepatic lipogenesis, hypertriglyceridaemia, decreased fatty acid oxidation and muscle protein wasting are common in patients with sepsis, cancer or AIDS. Given carnitine's role in the oxidation of fatty acids (FAs), we anticipated that carnitine might promote FA oxidation, thus ameliorating metabolic disturbances in lipopolysaccharide (LPS)- and methylcholanthrene-induced sarcoma models of wasting in rats. In the LPS model, rats were injected with LPS (24 mg kg-1 i.p.), and treated with carnitine (100 mg kg-1 i.p.) at -16, -8, 0 and 8 h post LPS. Rat health was observed, and plasma inflammatory cytokines and triglycerides (TG) were measured before and 3 h post LPS. In the sarcoma model, rats were implanted subcutaneously with tumour, and treated continuously with carnitine (200 mg kg-1 day-1 i.p.) via implanted osmotic pumps.
Tumour
burden, TG and cytokines were measured weekly for 4 weeks. Carnitine treatment significantly lowered the tumour-induced rise in TG (% rise) in the sarcoma model (700 +/- 204 vs 251 +/- 51, P < 0.03) in control and carnitine groups respectively. Levels of interleukin-1 beta (IL-1 beta),
interleukin-6
(
IL-6
) and tumour necrosis factor-alpha (TNF-alpha) (pg ml-1) were also lowered by carnitine in both LPS (IL-1 beta: 536 +/- 65 vs 378 +/- 44:

: 271 +/- 29 vs 222 +/- 32; TNF-alpha: 618 +/- 86 vs 367 +/- 54, P < or = 0.02) and sarcoma models (IL-1 beta: 423 +/- 33 vs 221 +/- 60;

: 222 +/- 18 vs 139 +/- 38; TNF-alpha: 617 +/- 69 vs 280 +/- 77, P < or = 0.05) for control and carnitine groups respectively. We conclude that carnitine has a therapeutic effect on morbidity and lipid metabolism in these disease models, and that these effects could be the result of down-regulation of cytokine production and/or increased clearance of cytokines.

) monoclonal antibody (MoAb) administered to a patient with multiple myeloma (MM) induced high amounts of
IL-6
to circulate in the form of monomeric immune complexes. Based on this observation, the present study developed a new methodology to estimate daily
IL-6
production in 13 patients with MM or renal cancer who received anti-
IL-6
MoAb. Treatment was considered effective when the production of C-reactive protein (CRP) was inhibited. The production of this acute-phase protein by hepatocytes is dependent on the activation of
IL-6
gp130 transducer. Inhibition of
tumor
proliferation was also evaluated in patients with MM. In 7 of 13 patients whose CRP production was completely inhibited (> 96%) and who showed some antitumoral effects, whole-body
IL-6
production in vivo was less than 18 micrograms/d (median, 5.7 micrograms/d; range, 0.5 to 17.5 micrograms/d). In the other 6 patients, subtotal inhibition of CRP production and a lack of antitumoral response were associated with high
IL-6
production (median, 180 micrograms/d; range, 18 to 358 micrograms/d). These in vivo observations were consistent with mathematical modeling that predicted that anti-
IL-6
MoAb treatment would be efficient only in low
IL-6
producers. These data indicate the difficulty of neutralizing
IL-6
with a single anti-
IL-6
MoAb in vivo and call for new strategies to avoid accumulation of
IL-6
in the form of stable immune complexes.

The aim of this study was to ascertain whether any cytokines that function in earlier stages of hematopoiesis also fluctuate in conjunction with granulocyte colony-stimulating factor (G-CSF) in chemotherapy-induced myelosuppression. A total of seven patients were studied. All patients received 3 days of intravenous injection of combination chemotherapy. Patients' absolute neutrophil count (ANC), platelet count, serum G-CSF,
interleukin-6
(
IL-6
), IL-3, and IL-1 alpha were monitored before chemotherapy, and then daily or every other day thereafter during the entire treatment course until the ANC returned to normal. The results showed very obvious elevation of serum
IL-6
level before or concurrent with the elevation of serum G-CSF levels at the neutrophil nadir in all seven patients. The rise of
IL-6
also correlated with nadir platelet levels in six of seven patients. The finding of serum
IL-6
elevation was statistically significant both in neutropenic and thrombocytopenic stages. Serum IL-3 level was below minimum detectable concentrations in all seven patients. Serum IL-1 alpha was below minimum detectable concentration in six patients and demonstrated no obvious fluctuation in the remaining patient. Therefore, the present study demonstrated the chronological time sequence of cytokine fluctuation,
IL-6
peak before G-CSF, in chemotherapy-induced myelosuppression. According to this finding, when cytokines are used for prevention of myelosuppression or for acceleration of its recovery, it may be logical to use a combination of cytokines in sequence, such as
IL-6
initially followed by G-CSF.

Interleukin-6

) is a cytokine that has been implicated as a growth factor in human ovarian carcinoma, yet the in vivo source of
IL-6
in patients remains undefined. We measured
IL-6
by ELISA in cell-free ascites (CFA) of 19 patients with ovarian carcinoma.
IL-6
was detectable in all samples (mean level 3.3 ng/ml). To identify the cellular source of
IL-6
, we measured this cytokine by ELISA in 24-48 h supernatants of cultured lymphocyte-, macrophage-, and
tumor
cell-enriched populations purified from three solid ovarian carcinomas by centrifugal elutriation. All cell populations spontaneously released
IL-6
; however,
tumor
cells and
tumor
-associated macrophage released levels of
IL-6
that greatly exceeded those released by
tumor
-associated lymphocytes. Kinetic studies revealed that
IL-6
was detectable at 6 h and that levels increased in all cultures examined over a 48 h time course. These data suggest that both
tumor
and infiltrating host cells may be the source of the high levels of
IL-6
found in carcinomatous ascites. Furthermore, although all three cell types examined may contribute to
IL-6
production in patients with ovarian carcinoma,
tumor
cells are perhaps the most clinically significant source.

In patients with advanced multiple myeloma (MM) there is an excess of production of
interleukin-6
(
IL-6
) in vivo, and elevated serum levels are associated with plasmablastic proliferative activity and short survival. These data prompted us to perform a clinical trial with a murine anti-
IL-6
monoclonal antibody (MoAb) to neutralize the excess of this putatively deleterious factor in these patients. Ten MM patients with extramedullary involvement frequently were treated with anti-
IL-6
MoAb. The MoAb was administered intravenously to 9 patients; 1 patient with malignant pleural effusion received intrapleural therapy. Of the 3 patients who succumbed to progressive MM after less than 1 week of treatment (including the only 1 treated locally), 2 with evaluable data exhibited marked inhibition of plasmablastic proliferation. Among the 7 patients remaining more homogeneous receiving the anti-
IL-6
MoAb for more than 1 week, 3 had objective antiproliferative effect marked by a significant reduction of the myeloma cell labelling index within the bone marrow. One of these 3 patients achieved a 30% regression of
tumor
mass. However, none of the patients studied achieved remission or improved outcome as judged by standard clinical criteria. Of major interest, objective antiproliferative effects were associated with complete inhibition of C-reactive protein (CRP) synthesis and low daily
IL-6
production in vivo. On the other hand, the lack of effect in 4 patients was associated with a higher
IL-6
production and inability of the MoAb to neutralize it. Anti-
IL-6
was also associated with resolution of low-grade fever in all the patients and with worsening thrombocytopenia and mild neutropenia. The generation of human antibodies to Fc fragment of the murine anti-
IL-6
MoAb observed in 1 patient was associated with dramatic progression. These data show that anti-
IL-6
MoAb can suppress the proliferation of myeloma cells and underscore the biologic role of
IL-6
for myeloma growth in vivo. Furthermore, suppression of CRP and worsening of neutropenia/thrombocytopenia both indicate that
IL-6
is critically involved in acute-phase responses and granulopoiesis/thrombopoiesis.

PMID:Biologic effects of anti-interleukin-6 murine monoclonal antibody in advanced multiple myeloma. 760 99
A phase IIb trial using liposome-encapsulated muramyl tripeptide phosphatidylethanolamine (L-MTP-PE) in combination with ifosfamide (IFX) for patients with relapsed osteosarcoma was undertaken to determine (a) the tolerability of the combination therapy, (b) if L-MTP-PE increased the toxicity of IFX, and (c) whether IFX altered or suppressed the in vivo immune response to L-MTP-PE. Patients had histologically proven osteosarcoma and pulmonary metastases that either developed during adjuvant chemotherapy or were present at diagnosis, persisted despite chemotherapy, and recurred following surgical excision. Stratum A patients were rendered clinically free of disease within 4 weeks of study entry prior to receiving combination therapy. IFX was administered at 1.8 g/m2 for 5 days every 21 days for up to eight cycles. L-MTP-PE was administered twice weekly for 12 weeks, then once weekly for 12 weeks. Once cycle of combination therapy was defined as 5 days of IFX and 3 weeks of L-MTP-PE therapy. Stratum B patients had measurable disease at study entry that was judged to be amenable to surgical resection. Stratum B patients received three cycles of combination therapy prior to surgery to judge clinical and histologic response. Postoperatively, patients received an additional five cycles. A total of nine patients were entered into the protocol: six on stratum A and three on stratum B. Serial blood samples were collected and assayed for cytokine levels (tumor necrosis factor-alpha [TNF alpha],
interleukin-6
[IL-6], IL-8, neopterin, C-reactive protein). In addition, peripheral blood monocyte tumoricidal activity was evaluated pre- and post-combination therapy. Complete blood counts with differential and platelet counts were followed weekly. No increase in the toxic side effects of IFX was demonstrated when administered with L-MTP-PE nor were delays in IFX administration due to neutropenia experienced. The toxic side effects of L-MTP-PE were also not increased. Elevations of serum C-reactive protein, plasma neopterin, IL-6, IL-8, and TNF alpha following combination therapy were similar to those observed in patients treated with L-MTP-PE alone. Monocyte-mediated tumoricidal activity was elevated 24 and 72 h following L-MTP-PE and IFX therapy, similar to what has been reported following L-MTP-PE alone.

PMID:Combination therapy with ifosfamide and liposome-encapsulated muramyl tripeptide: tolerability, toxicity, and immune stimulation. 761 44
The gene expression of five matrix metalloproteinases (MMPs) and two tissue inhibitors of metalloproteinases (TIMPs) was studied in human gliomas in vivo and in vitro to evaluate their roles in glioma invasion. Simultaneous expression of one to four MMP genes and two TIMP genes was found in 17 surgical glioma specimens, and one MMP (gelatinase A) gene and two TIMP genes were simultaneously expressed in tissue of three brains. The concomitant overexpression of gelatinase A, gelatinase B, and occasional matrilysin genes was associated with the malignancy of gliomas and accompanied by overexpression of the TIMP-1 gene. In five human glioma cell lines, gelatinase A, TIMP-1, and TIMP-2 genes were constitutively expressed in alll cell lines: the matrilysin gene in three cell lines; the stromelysin gene in two cell lines; and the interstitial collagenase gene in one cell line. There was a clear difference in the expression of gelatinase B and stromelysin genes between surgical glioma specimens and glioma cell lines: the gelatinase B gene was not expressed constitutively in vitro but was overexpressed in vivo, whereas the stromelysin gene was not expressed in vivo but was expressed in some cell lines. To find the cause of that difference in vivo and in vitro, the transcriptional regulations of MMP and TIMP genes by
tumor
promoter, growth factors, or cytokines were studied in vitro. Interstitial collagenase, gelatinase B, stromelysin, and TIMP-1 genes were upregulated in many cell lines by phorbol-12-myristate-13-acetate (PMA) and in some cell lines by epidermal growth factor, tumor necrosis factor-alpha, or interleukin-1 beta. Transforming growth factor-beta 1 (TGF beta 1) upregulated gelatinase A and matrilysin genes in some cell lines, and there were no clear responses from any MMP and TIMP genes to
interleukin-6
. Thus, the transcriptional modulation of MMP genes by these growth factors and cytokines seemed insufficient to explain the difference in gelatinase B and stromelysin gene expressions in vivo and in vitro and was suggestive of the genetic alteration of glioma cells in vitro, the heterogeneous cell population in glioma tissues, or both. Furthermore, the in vitro invasion of glioma cells through Matrigel in response to PMA, TGF beta 1, or TIMP-1 was assessed by chemoinvasion assay.
