From 1964 to 1977 a total of 50 patients were operated on for carcinoma of the ampulla of Vater, the second most frequent

of the periampullar region. The cardinal symptom was jaundice. The diagnostic procedures of high value were roentgenographic examination of the upper gastrointestinal tract with the use of

, also hypotonic duodenography, sonography, and percutaneous transhepatic cholangiography (ERCP, angiography). Curative treatment was possible by duodenopancreatectomy, with a resection rate of 74%. The 5-year survival rate was 27% with a postoperative death rate of 13.5%.

Flexible sigmoidoscopy was compared to rigid sigmoidoscopy in the detection of colorectal
neoplasia
in a select group of patients. A distance of 30 cm or greater was obtained by flexible sigmoidoscopy in 94% of patients and a distance of 50 cm or greater in 46% of patients. A significant number of cancers and adenomas detected by flexible sigmoidoscopy were not detected by rigid sigmoidoscopy. Flexible sigmoidoscopy was tolerated better than rigid sigmoidoscopy but required twice the time. Flexible sigmoidoscopy could be combined with air-contrast
barium
enema the same day with the one preparation and did not interfere with the x-ray examination. All cancers and a significant number of adenomas detected subsequently on colonoscopy were detected by the combination of flexible sigmoidoscopy and air-contrast
barium
enema. Although the combination of flexible sigmoidoscopy and air-contrast
barium
enema is not adequate for thorough diagnostic evaluation of patients with a positive screening test, it may be of value in other select clinical situations. Flexible sigmoidoscopy has the potential for higher yield and better patient tolerance as compared to rigid sigmoidoscopy. This warrants further evaluation.

A man of 37 years of age developed portal hypertension which was observed on radiological examination as a mediastinal opacity having the appearance of a

. No oesophageal varices were seen on

meal examination or by endoscopy. A smooth liver was seen during laparoscopy. Pre-operative hemodynamic studies showed that there was no sinus block. Thoracotomy revealed an aneurysm of the azygos veins. Exploratory laparotomy after coelio-mesenteric arteriography confirmed the diagnosis of diffuse nodular hyperplasia of the liver, cirrhosis, and portal hypertension. The findings in 16 cases of mediastinal opacity from portal hypertension reported in the published literature are discussed.

We have described a case of primary melanoma of the esophagus in an asymptomatic patient. The
tumor
presented as a multinodular filling defect on a routine
barium
swallow. The distal esophagus and a proximal portion of the stomach were resected. The patient died ten days postoperatively of Klebsiella sepsis. Grossly, the surgical specimen showed multiple polypoid tumors which arose from separate pigmented areas shown microscopically to be melanoma in situ. By electron microscopy, the
tumor
cells contained numerous melanosomes in various stages of melanization. Primitive hemidesmosomes were found where a basal lamina was present. Better developed desmosomes interconnected the melanocytes.

Two newborns with similar lesions were treated at two children's hospitals. Each newborn presented with an abdominal emergency that required immediate surgery. In each instance, small bowel obstruction was clinically and radiologically suspected;

enema examination showed an irreducible colonic intussusception in the first baby and a colonic perforation in the second. Both these findings required immediate operation. The baby with the intussusception had a colon resection and a primary anastomosis, while the newborn with the perforation had a resection and temporary colostomy. Subsequent to the initial surgery, neither baby has received any further treatment for the

. The two patients are now well at 13 yr and 6 yr of age.
