The chemosensitivities of 77 samples of human head and neck cancer were examined by in vitro succinate dehydrogenase inhibition (SDI) test. The
tumor
tissues obtained at biopsy specimens were exposed to doxorubicin (DXR) and pirarubicin (
THP
). The average decrease of the enzyme activity by
THP
was significantly greater than that by DXR. In 60 cases of squamous cell carcinomas, the chemosensitivity of poorly differentiated type tended to be higher compared to the well differentiated type. It is suggested from there results that
THP
is a more effective anticancer drug than DXR against human head and neck cancers. Clinically good responses were obtained in a systemic chemotherapy of such as head and neck adenoid cystic carcinomas by combining
THP
with other anticancer drugs such as CDDP (or CBDCA) and CPA.

The presence of sugar receptors on human myeloid leukemia cells was comparatively assessed by a highly sensitive binding assay, employing a panel of 14 types of neoglycoenzymes (chemically glycosylated Escherichia coli beta-galactosidase). The selected carbohydrate ligands mainly encompass common components of natural glycoconjugates as mono- or disaccharides. The monocytoid cells of the

-1 line, the very young myeloblasts and the myeloblasts of the lines KG-1a and KG-1, the promyelocytes of the HL-60 line, and the early myeloblasts/erythroblasts of the K-562 line displayed a nonuniform pattern of specific binding with quantitative differences at a fixed, nonsaturating concentration of the probes. Scatchard analysis in four cases corroborated the indication of cell-type-related differences between the various cell lines. To test whether the detectable cellular sugar-binding sites can mediate adhesion to glycoligands, a rather simple model matrix of nitrocellulose-immobilized neoglycoproteins was first used. In comparison to the carbohydrate-free carrier protein significant cell adhesion was observed primarily with neoglycoproteins that exposed galactose, N-acetylgalactosamine, N-acetylglucosamine, mannose, and fucose moieties among the 11 tested types of carbohydrate residue. Subsequently, human bone marrow stromal cell layers were tested as a model matrix with increased levels of physiological relevance and complexity. Mixtures of carbohydrate and neoglycoprotein were employed as inhibitors of an interaction via lectins between the stromal and the
tumor
cells. The carbohydrate-dependent alterations of this parameter revealed cell-type-associated properties.
Tumor
cell binding was significantly decreased for not more than two lines with the effective sugars, namely N-acetylgalactosamine, mannose, fucose, and sialic acid.

Extramedullary plasmacytoma (EMP) is a very rare disease and mainly arises in the head and neck area. We herein reported a case of EMP arising in the retroperitoneal space. A 46-year-old man was referred to our outpatient clinic in November 1989 with the complaint of flank pain on the left side. Radiological examinations showed a
tumor
formation in the retroperitoneal space, which involved the left kidney, spleen and pancreas. Immunoelectrophoresis showed an elevation of serum IgG level and a spike of M-protein was detected in the serum protein electrophoresis. No bone lesions were detected, and bone marrow aspiration showed no abnormal cells. US-guided needle biopsy of the
tumor
led to the histological diagnosis as plasmacytoma of the IgG-kappa type. Following three cycles of preoperative chemotherapy (a
THP
-COP regimen), which resulted in a size reduction of the
tumor
by 40%, extensive resection of the
tumor
including extirpation of the left kidney, spleen, and tail of pancreas was performed. Because of
tumor
extension into the posterior wall of the stomach, however, the surgery resulted in incomplete resection. A total of 11 cycles of postoperative chemotherapy (
THP
-COP) was performed periodically for the residual tumor in the stomach. Rapid
tumor
spreading in addition to re-elevation of the serum IgG level, however, developed after the 11th postoperative chemotherapy, which extensively involved the stomach and intestines. The patient died of the disease 33 months after the initiation of treatment.

A rare case of lingual carcinoma which metastasized to the brain is reported. A 66-year-old man was referred to our department on April 23, 1991, with complaints of memory disturbance and mild left motor weakness of one month's duration. The patient had been discharged from the Department of Otolaryngology of our hospital 9 months before, after undergoing radical neck dissection and chemotherapy (
THP
, CDDP, PEP) for his lingual cancer with metastases to the cervical lymph nodes and the lung. On admission to our department, the patient complained of mild frontalgia. Neurological examination showed memory disturbance, change of character, mild left hemiparesis and urinary incontinence. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a well circumscribed mass in the right frontal lobe associated with midline shift toward the left. Systemic Gallium-67 scintigraphy demonstrated an abnormal uptake in the right frontal region, but no abnormal uptake was seen in the neck and the lung. The most likely diagnosis was thought to be metastasis of lingual cancer. The patient underwent
tumor
removal 10 days after admission. A histological examination of the
tumor
specimen revealed metastatic anaplastic cell carcinoma, indicating a metastasis from lingual carcinoma. After postoperative radiotherapy (total 60 Gy), the patient was discharged without neurological deficit. Central nervous system metastasis from lingual carcinoma is quite rare. To the best of our knowledge, only two such cases have been reported. We report another rare case of solitary brain metastasis from lingual carcinoma and review the pertinent medical literature.

40 mg i.v. days 1, 8, 5-FU 500 mg i.v. days 1, 8 and MPA 1,200 mg p.o. daily. One month later, the lung metastases completely disappeared on chest CT gram. Nine courses of chemotherapy with the same drugs were undertaken intravenously every four weeks. This therapy has been continuously effective for one and half years now. These results suggested the importance of a severe adjuvant therapy for the initial phase of advanced breast cancer.

Intraarterial chemotherapy with Adriamycin (ADM) has shown limited advantages over i.v. administration, with no reduction in systemic toxicities and modest decrease in peripheral plasma levels. In an effort to improve the selectivity of i.a. anthracycline chemotherapy, we compared pirarubicin (4'-O-tetrahydropyranyladriamycin,
THP
) and ADM in the surgically implanted VX2 rabbit
tumor
model. Both drugs were administered at the same dose (0.5 mg/kg) either by the intraarterial hepatic route (i.a.h.) or by the i.v. route. Anthracycline plasma and tissue levels were determined by high-performance liquid chromatography with fluorescence detection. ADM peak plasma concentration and area under the curve were not significantly reduced after i.a.h. administration compared to the i.v. route; however, ADM
tumor
concentration was 1.9-fold higher following i.a.h. administration compared to the i.v. infusion. After
THP
administration by the i.a.h. route, systemic exposure (area under the curve) was markedly reduced (8-fold) compared to the same dose administered i.v. These findings correlated well with the very low concentration of the drug in heart tissue following i.a.h. infusion. After i.a.h. administration,
tumor
THP
concentrations were 10.5 times higher compared to the i.v. route. The pharmacokinetic advantage of i.a.h. administration of
THP
also led to a better antitumoral effect, as shown by a significantly lower tumor growth rate [3 +/- 2% (SD)] in the i.a.h.-treated animals compared to the i.v.-treated groups (58 +/- 9%). Administration of ADM by the i.a.h. route was also inferior to i.a.h.
THP
. Taken together, our results suggest a clear-cut advantage of
THP
over ADM for i.a.h. locoregional chemotherapy, because of higher local
tumor
concentrations, greater antitumoral effect, and lower systemic exposure following the i.a.h. administration of
THP
. This anthracycline analogue could also be of therapeutic advantage in tumors partially resistant to anthracyclines that would become vulnerable to the high local concentrations achieved with i.a.h. administration. Based on these encouraging results, clinical trials using
THP
administered by the i.a.h. route were initiated.

A human neuroblastoma xenograft, designated TNB9, was used in this experiment. This xenograft is known to have a homogeneously staining region (HSR) on chromosome 20 and to exhibit 60- to 100-fold amplification of clones 8, G21 and N-myc, and showed a rapid

weight doubling time of 5.9 days; it represents one of the most malignant strains of human neuroblastoma. The effects of nine different chemotherapeutic agents on this xenograft were studied according to the standard Battelle Columbus Laboratories protocol, and the in vivo chemotherapeutic sensitivity assessment disclosed that Mitomycin C, Ifosfamide, and Carboplatin were highly effective against it, while VP-16, NK-171, 5-Fluorouracil, and

-Adriamycin were ineffective. Cytogenetic and molecular-cytogenetic analyses suggest that the present data may accurately predict the clinical results with these chemotherapeutic agents in treating patients in advanced stages, as did those from our previous studies.
