An operated case of primary mediastinal liposarcoma is reported. A 71-year-old male who was asymptomatic was unexpectedly pointed out a huge abnormal mass shadow in the mediastinum on the chest roentgenogram, when he caught a common cold. The CT scan showed the singular horseshoe-shaped tumor in the posterior mediastinum which was adjacent to the antero-lateral phase of the vertebrae. MRI was useful to our recognition of its whole shape and localization. Extirpation of the tumor via right postero-lateral thoracotomy was performed successfully. The pathological diagnosis was liposarcoma; well differentiated, lipoma-like type. Primary mediastinal liposarcoma is a very rare mediastinal tumor (0.2%). Thirty-six cases of primary mediastinal liposarcoma including our case have been reported at present in Japan, so far as we surveyed. It is generally known that liposarcoma has the low sensitivity to the radiotherapy and chemotherapy and has high incidence of recurrence. We think that this case needs the strict postoperative follow-up.

A 71-year-old man developed metastatic cutaneous nodules of colon carcinoma on the scalp and chest wall. Histopathological findings revealed aggressive invasion of the tumor cells into the overlying epidermis (epidermotropic carcinoma); they occasionally adhered to the epidermal cells. These morphological features have not been described in cutaneous metastases from internal carcinomas.

A 71-yr-old woman with a widely metastatic lipid-rich variant of breast cancer was found to have striking hyperamylasemia (85-fold normal). By isoelectric focusing, agarose gel electrophoresis, and a wheat protein inhibitor assay, the predominant serum amylase appeared to be identical to pancreatic isoamylase. Serum trypsin, serum lipase, and an abdominal computed tomography scan were normal, excluding the possibility of pancreatitis. Furthermore, both the primary breast tumor and skin metastases that developed 10 yr later stained immunohistochemically for amylase. Thus, breast carcinoma must be added to the list of tumors causing ectopic hyperamylasemia, and this case shows that nonpancreatic malignancies may produce pancreatic-type hyperamylasemia.

A 71-year-old man was admitted with the chief complaint of gross hematuria. Cystoscopic examination showed a broadbased papillary tumor at the apex of the bladder. There was edema of the mucous membrane around the tumor. CT-scan demonstrated a mass extending from the bladder dome superiorly. Partial resection of the bladder was done. Pathological examination revealed grade 3 transitional cell carcinoma, with scattered adenocarcinoma and squamous cell carcinoma foci. Chemotherapy with bleomycin (BLM) and cis-dichlorodiamine platinum (CDDP) was done postoperatively. Ten months after the operation, he was readmitted because of recurrence in retroperitoneal lymph nodes. Chemotherapy with BLM and CDDP was done, but he died of pulmonary complications. Autopsy revealed retroperitoneal lymph node metastasis of transitional cell carcinoma. There was no metastasis to any other organ. We briefly discuss 275 cases of the carcinoma of the urachus collected from the Japanese literature.

A 71-year-old woman, 26 years post-menopause, presented with virilization. Facial hirsuties, non-hereditary frontal balding, voice changes, male escutcheon, and mild clitorimegaly accompanied a right adnexal mass. Blood hormone studies showed testosterone 430 ng/dL, FSH 118 mIU/mL, and LH 210 mIU/mL. By ultrasound examination, the cystic adnexal mass involved the right ovary. An 18-cm, 1300-g, unicameral mass with 1200 mL of clear serous fluid and with smooth inner and outer surfaces was removed from the right broad ligament. Intraoperative testosterone levels were as follows: peripheral vein 285 ng/dL, left ovarian vein 301 ng/dL, and right ovarian vein 1635 ng/dL; tumor cystic fluid was 3032 ng/dL. Peripheral vein testosterone was 15 ng/dL 3 days postoperatively. Histopathologically, the tumor was a serous cystadenoma. No evidence of stromal luteinization, hyperplasia, or inflammation was found, and other virilizing lesions were not encountered in either ovary or in other tissues. Epithelial cells constituting the tumor may have been the source of the excess testosterone in this unique case of virilizing serous cystadenoma.

A 71-year-old white man with asymptomatic microscopic hematuria had a sessile tumor at the dome of the bladder containing benign prostatic glandular tissue. Benign prostatic polyps occur commonly in the prostatic urethra and bladder neck as vestigial remains of embryonic prostatic elements. Ectopic prostatic tissue elsewhere is rare and has not been described previously arising at the bladder dome. The origin of prostate glands in this unlikely location is unclear. The clinical course of ectopic prostatic polyps at any location is benign, although local recurrence has been reported.

A 71-year-old black woman with no known stigmata of neurofibromatosis developed a left buccal mass of 2 years duration. Histologic findings revealed a hemangiopericytoma and a plexiform neurofibroma in the same tumor. Cells with features of both tumor components were demonstrated ultrastructurally.

A 71-year-old woman who presented with severe vaginal bleeding was found to have a uterine angiosarcoma after total abdominal hysterectomy and bilateral salpingo-oophorectomy. The diagnosis was confirmed by immunohistochemical studies that showed positive staining of tumor cells for factor VIII-related antigen and by ultrastructural studies revealing vasoformative structures surrounded by a thin basal lamina. Primary vascular tumors of the uterus are rare. The existence of malignant vascular neoplasms of the uterus has been questioned, and at most nine previously reported cases have been generally accepted as valid examples of uterine angiosarcoma. Based on those cases, the prognosis of uterine angiosarcoma is poor. Our studies demonstrate that angiosarcoma does occur in the uterus. Our patient is presently alive with recurrent disease.

Cases of intraosseous meningioma appear to be very rare. In the present paper, we report such a case and discuss its etiological histogenesis on the basis of a review of 26 cases previously reported. A 71-year-old female was admitted to our department because of a painless mass in the right parietal region. Neurological findings were normal. Plain skull radiograph showed a 6 X 5-cm osteolytic lesion in the right fronto-parietal bone. CT scan demonstrated this lesion as a mass showing homogeneous enhancement with contrast medium. A right common carotid angiogram showed an avascular area in the parietal region and no tumor stain. The tumor, which was partly attached to the underlying dura, was totally removed surgically, and the postoperative course was uneventful. Histological examination of the specimen revealed fibroblastic meningioma, which was limited only to the outer membrane of the dura. The inner membrane was intact and free of tumor invasion. The histogenesis of intraosseous meningioma is still controversial but the following theories seem to be widely accepted: (1) The tumor arises from a part of the dura which has become trapped in a suture during embryological development. (2) The tumor arises from a part of the dura which has become trapped in a suture due to pressure on the head during delivery. (3) The tumor arises from a part of the dura which has become trapped in the bone due to trauma. (4) The dura which has become trapped in a suture during embryological development may undergo neoplastic change following traumatic stimulation.(ABSTRACT TRUNCATED AT 250 WORDS)

A case of pancreatic carcinoma associated with marked eosinophilia is reported. A 71-yr-old man was admitted to hospital because of melena and abdominal pain. The systematic examinations revealed pancreatic adenocarcinoma with multiple metastases (rectum, lung and brain). The leukocyte count was gradually increased and reached up to 81.7 X 10(9)/l, of which 54% consisted of eosinophils. Colony-stimulating factor (CSF) was detected both in the patient's serum and in the tumor extracts by a normal human bone marrow culture system. The colonies which were stimulated with patient's serum largely consisted of granulocyte, granulocyte/macrophage and eosinophil types. These results suggest that blood leukocytosis and eosinophilia were due to a high concentration of plasma CSF, which was probably produced by the tumor cells.
