None had either the instability that might have been anticipated or incapacitating

in any of the five patients. The procedure, which has the same basic indications as a hindquarter amputation, is a rewarding substitute for that mutilating operation, although the follow-up is too short and the series is too small to warrant conclusions as to the cure rate.

An experience based on 1,211 patients has shown aspiration needle biopsy to be a valuable technique for diagnosing bronchogenic carcinoma and other localized intrathoracic lesions that are beyond the reach of the fiberoptic bronchoscope. In 896 patients with malignant intrathoracic

, the aspirate demonstrated malignant cells in 96%. A false cytological diagnosis of carcinoma occured in 2 patients, for a true positive rate of 99%. However, the true negative rate was only 87%. In 77% of 31 immunosuppressed patients, the causative agent of a focal infectious process was diagnosed. Pneumothorax was the only notable complication, occuring in 24% of patients, with 14% requiring chest tube drainage. The procedure is relatively simple and rapid, generally causes little patient

, and can be performed in virtually any hospital.

A study was made of 201 asymptomatic women over age 40 (range, 40-74 years) who underwent uterine sounding and endometrial screening by the negative-pressure jet irrigation technique, without anesthesia, in private gynecologists' offices. Introduction of the irrigator was accomplished in 88 percent of these patients. In 97 percent of the successful irrigations, the specimens obtained were satisfactory for cytologic and histologic diagnosis of
neoplasia
. No occult endometrial carcinomas were discovered. The factors which interfered with endometrial screening by these methods are analyzed. Cervical stenosis prevented endometrial irrigation in 13 of the 24 unsuccessful attempts. Acceptance by the patients was high, in that 74.2 percent reported slight or no
discomfort
and only 4.7 percent complained of severe
discomfort
. Although 12.5 percent experienced pelvic cramping after irrigation, no other significant side effects were observed. The results of this investigation indicate the feasibility of using the endometrial irrigation technique for massive screening studies of asymptomatic women who are at risk for the development of endometrial carcinoma.

Lymphatic cysts of the retroperitoneal space among the rarest abdominal tumors. A very small number, usually single case reports, has been reported in the international literature. Symptoms vary according to the

size but usually are minimal so that they are discovered by chance, either during an ordinary abdominal examination or because the patient complains of

due to the pressure of the cyst on adjacent organs and tissues. Surgical intervention is the only treatment. Total cystectomy with the treatment of choice is seldom feasible, because of technical difficulty. A case of a huge retroperitoneal lymphatic cyst is reported. Epiploplasty with drainage is proposed as a new method of surgical treatment.

Diagnostic pleuroscopy has been performed under local anesthesia in nine patients using a gas sterilized flexible fiberoptic bronchoscope inserted through a 1 to 2 cm chest incision into the pleural space. Pleuroscopy in one patient excluded recurrent
neoplasm
on the pleural aspect of a bronchopleural fistula. Another patient had a pleuroscopic biopsy of the lung, which was the only method successful in diagnosing a metastatic renal carcinoma. The other seven patients were studied for pleural effusions which were undiagnosed after study of pleural fluid and/or Abrams needle biopsy. In four of them pleural implants of carcinoma were visualized and proved by biopsy. Three patients had negative pleuroscopy, two of these also being negative at subsequent thoracotomy. One was not explored because of extrathoracic metastases. The procedures were performed with minimal patient

A pilot study undertaken to evaluate short-course, high-increment radiation therapy. This type of treatment was tolerated well by the patients, with a surprisingly low level of acute reaction to the regimen and few late complications.

Preliminary results show that long-range survival rates are at least equivalent to those obtained with the conventional longer treatment regimen. A case of primary adenocarcinoma of the male urethra with high CA125 level is reported. A 66-year-old man was admitted to our hospital with dysuria and perineal

. The urethrocystogram revealed an irregular urethral margin in the bulbo-membranous urethra. Computer-tomographic (CT) scan and transrectal ultrasonography revealed an irregular mass between the urethra and rectum. Cytologic examination of the urine was negative for malignancy. Endoscopy revealed an irregular bulbo-membranous urethra with a small papillary lesion. Transurethral biopsy was performed and histological diagnosis was adenocarcinoma. The patient rejected surgical treatment and radiotherapy was performed. However, the disease progressed and the patient died from liver metastasis six months later. We considered that CA125 was valuable as a
tumor
maker in this patient.

Radiotherapy for the treatment of

in the head and neck region results in a number of oral changes liable to cause pain and

during therapy. In the longer term, it increases the patient's susceptibility to caries and periodontal disease. The aim of this paper is to outline these changes and to suggest a regime for the dental management of these patients before, during and after radiotherapy. The role of the dentist in support, advice and treatment is vital.
