During a 3 year period 572 high-resolution sonograms were carried out in the follow-up examinations of 348 patients who had been treated for
ENT
malignancy. Our experiences in routine application of this diagnostic method are described and illustrated by typical sonograms. Ultrasound findings of complications (chylous cysts, seromas, granulomas, fistulae) are demonstrated and the value and limitations of sonography in detecting lymph node metastases and primary recurrences are pointed out. In spite of the high accuracy of this method, it is not possible to differentiate between lymph nodes which are invaded by tumour and those with non-specific lymph adenopathy by echographic criteria only. We used ultrasound for follow-up studies on patients treated by irradiation or chemotherapy. Finally, sonography provided additional information on the exact site and size of the
tumor
. Real-time sonography is not time-consuming and can be applied without any special preparation of the patient. It has therefore become a satisfactory diagnostic method in the follow-up of patients who have undergone treatment of malignant tumors.

Isolated surgical approaches to
tumor
treatment have largely reached a point beyond which prospects appear to be unpromising. The search for more promising therapeutic approaches, therefore, has become a compelling challenge. Surgico-adjuvant tumour chemotherapy has been more thoroughly studied than anything else in that direction. The underlying theoretical concept seems to be persuasive, though clinical results have so far stayed below expectations. Secured prolongation of life, following this therapy, has so far proved to be probable only for cases of osteosarcoma, mammary carcinoma with high risk of recurrence, testicular tumours, and neurogenic tumours. Neo-adjuvant chemotherapy seems to work well in cases of
ENT
tumours. Model studies into Lewis-Lung carcinoma have shown that therapeutic effects cannot be guaranteed unless a sensitised tumour is treated with an effective preparation. This is likely to add to the long-range importance of the individual aspects relating to surgico-adjuvant tumour chemotherapy. Methods have been and continue to be developed for that purpose. Yet, all of them need to be methodologically improved. At present, waiting for such improvement, surgico-adjuvant tumour chemotherapy should be used only within the framework of controlled clinical studies.

The concept for surgical removal of facial skin neoplasms consists of three components: Sufficient preoperative clinical diagnosis; complete
tumor
removal; Adequate functional and esthetic reconstruction. For achieving proper diagnosis the collaboration with the dermatologist is highly recommended. The

examining microscope allows for better visualisation of structural characteristics. Excisional biopsy should be preferred whenever possible. Out of 97 basal cell carcinomas 25% secondary or thirdly neoplasms have been found. In 81 basal cell carcinomas the visible extent, the real
tumor
extent in the specimen after fixation and the total excisional area are compared. According to these measurements the following safety margins seem to be recommendable: For small solid or solid-cystic superficial basal cell carcinomas 3-5 mm, for the sclerodermiform or partially undifferentiated basal cell carcinoma 10-20 mm and for basal cell carcinomas involving the bone more than 20 mm. Excising a spinocellular carcinoma a safety margin of at least half of the maximal diameter has proved to be sufficient. The close collaboration with the histopathologist resulted in our department in a simplified economic and safe microscopically controlled excision. The rate of recurrences of 97 basal cell carcinomas was 6.6%. The question of prophylactic lymph-node dissection in combination with the excision of spinocellular carcinomas is discussed.

Two cases of facial nerve neurinoma (VII-Nm) were reported. The first case is a 41-year-old man who had 16 years of history of slowly progressive right facial palsy and hearing disturbance. The
tumor
originated in horizontal portion of the right facial nerve, and extended epidurally into the middle cranial fossa, as well as medially along the facial canal to the C-P angle. The second case is a 55-year-old man with 16 years of history of progressive right facial palsy which was followed by hearing disturbance. The
tumor
was found in the vertical portion of the right facial nerve, expanding in the petrous bone. Both tumors were removed surgically. 116 VII-Nm were reported in the literature including our 2 cases. Most were reported in the
ENT
field, and only 11 cases in the neurosurgical field. Facial palsy or hearing disturbance is the initial symptom, depending on the site of origin in the facial nerve. VII-Nm can originate in any part of the facial nerve. The most common part of origin is the vertical portion where the
tumor
tends to expand locally, but it may extend extracranially along the facial canal. The
tumor
which originates in the tympanic portion tends to extend into the middle ear cavity. The
tumor
originating in the horizontal portion extends to the epidural space in the middle cranial fossa and/or medially along the facial canal. VII-Nm in the cisternal portion of the facial nerve extends to the posterior cranial fossa and produces the signs of C-P angle

Hematoporphyrin derivative-photoradiation therapy (HPD-PRT) of human malignancy has been performed on 267

sites in 50 patients.

response has been assayed at 24 hours and one month following treatment with 625-635 nm light from an argon laser-pumped dye laser. The majority of tumors treated were dermal breast cancer recurrences (21 cases), local recurrence of

squamous cell carcinomas (16 cases), and
ENT
cutaneous metastases of squamous cell carcinomas (9 cases). A 30-day favorable response was judged to be either complete regression of the

or reduction of

diameter by greater than 50%. The above categories of tumors had a combined favorable response of 81%. Appropriate dosimetry is discussed.

tumors. The mechanism of this oncogenic osteomalacia is uncertain, but it may result from a

-induced alteration in vitamin-D metabolism or from a direct phosphaturic action of a

Twenty-six patients with known primary tumors were treated surgically for pulmonary metastases between 1951 and 1979. The metastases were diagnosed more often (58% of cases) by routine radiological examinations than because of thoracic symptoms. While diagnosis of the primary tumor and the metastases was simultaneous in three cases, the average lapse of time between the two diagnoses was 5 years and 1 month in the other cases. In four cases, it was 10 years or more. Lung X-rays showed single tumors more often than multiple tumors (12%). Broncho-fibroscopy was usually normal, but a preoperative biopsy showed the metastatic nature of the
tumor
in 5 cases. Ablation was by lobectomy (42%), pneumonectomy (23%) or atypical resection (27%). The hospital mortality rate was 7.6%. The survival rate at 1, 3 and 5 years (71%, 39%, and 17%) is identical to that of primitive bronchial tumors, and seems to be influenced by certain factors, such as the locus of the primitive
tumor
(with a particularly bad prognosis for
ENT
tumors), the lapse of time between the diagnoses of the primary and secondary tumors, the fact that there is one, rather than several, metastases, or that the metastases are monolateral rather than bilateral, and, finally, the limited nature of the exeresis. These factors prompt regular, systematic and prolonged radiological monitoring in search of further pulmonary metastases.

Five year survival rate of 168 cases of cancer in the cervical esophagus with extension to the hypopharynx, which had been treated in

department of Keio University during the past 13 years, was shown to be as low as 28%. Factors responsible for this poor result were delineated from standpoints such as local recurrence, node metastasis, distant metastasis, skip lesions, double lesions and so on. Preoperative combination of chemo-radiotherapy was evaluated histopathologically, and the results indicated that macroscopic reduction of
