The authors report an exceptional glomus tumor location in Hoffa's ligament in a 65-year-old man. Based on this observation and a literature review, the authors provide the clinical and radiographic diagnostic appearances of this type of tumor. Surgical removal of the tumor achieved immediate disappearance of knee pain. Histological examination of the tumor has established the definitive diagnosis.

We report a case of multiple diffuse fibrosarcoma of bone. The patient, a 38-year-old man, was referred to our hospital with knee pain, anemia and thrombocytopenia. No solid mass was seen on radiographic examination of the kneejoint, but magnetic resonance imaging showed hypointensity of the distal femur. Femoral biopsy revealed proliferation of long spindle-shaped fibrosarcoma cells, while a bone marrow biopsy of iliac bone (which appeared normal on radiographic examinations) showed replacement of hematopoietic cells by fibroblast-like spindle cells. A diagnosis of multiple diffuse fibrosarcoma of bone was therefore made. Autopsy revealed tumor invasion into multiple bones and several visceral organs and extramedullary hematopoiesis in the liver, spleen and lymph nodes. As this patient had leukoerythroblastic anemia with poikilocytosis, splenomegaly exhibiting extramedullary hematopoiesis, and apparent fibrotic change in his bone marrow, we suggest that this extremely rare disease should be considered in the differential diagnosis of myelofibrosis.

Extraskeletal myxoid chondrosarcoma is an uncommon neoplasm, accounting for less than 2% of all soft tissue sarcomas. It affects adult males with a median age in the fifth decade at the time of diagnosis. The tumor usually arises in the deep soft tissues, especially in the lower extremities. Patients present with a gradually enlarging mass that may or may not be associated with pain. This report describes a 25-year-old man who initially presented with a 4- to 5-year history of right knee pain and an enlarging mass in the right knee. Evaluation revealed a cartilaginous neoplasm with no evidence of metastatic disease. The tumor was widely excised and an allograft reconstruction was performed. The patient was closely followed with an eventual above the knee amputation for recurrent myxoid chondrosarcoma. At 34 months, retroperitoneal metastases were noted on abdominal CT. The patient underwent a left radical nephrectomy, renal vein thrombectomy and enucleation of the mass in the right kidney, distal pancreatectomy, and splenectomy. The patient received postoperative chemotherapy. Forty-eight months after initial diagnosis, the patient was found to have recurrent abdominal and retroperitoneal lesions. At 64 months, the patient died from complications of extraskeletal myxoid chondrosarcoma.

Chondroblastoma is a rare, benign bone tumor that may mimic an orthopedic infection. Characteristic clinical features include specific radiographic findings, predilection for the adolescent age group, and long bone epiphyseal involvement. This case of chondroblastoma is unusual because of aggressive findings on radiographic studies which underscore the need to obtain hip radiographs on adolescent patients with knee pain.

An 18 years old man and his younger sister suffered from anterior knee pain. Cause is a soft tissue tumor located within Hoffa's fat pad however in both cases. The tumor has been excised and the histological examination demonstrated the diagnosis synovial hemangioma. A literature search shows that synovial hemangiomas are rare soft tissue tumors. They mainly involve the knee joint. Mean age is the second decade of life and most authors observed severe degenerative changes in the involved knee joint. There are no reports about familiar accumulation within the literature. Familiar accumulation of synovial hemangiomas however have been described for cerebral, skin and hepatic manifestations but not for synovial hemangiomas.

Although bone tumors are often located in the knee area, primary tumors of the patella are rare, and patellar metastases extremely rare. Only a few cases were reported in the literature originating from different primary tumors, mostly lung and breast [1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 18, 19, 20, 22]. There is only one other case description of a colon carcinoma [21]. We present a rare case of primary sigma carcinoma and patellar metastasis. Because of the increasing rate of colon carcinomas, neoplastic disease must be considered in the differential diagnosis in any patient with otherwise unexplained knee pain.

A 15-year-old girl presented with anterolateral knee pain for 6 months. There were tenderness and pain at the anterolateral triangulation of the knee joint. Magnetic resonance imaging revealed a tumor in the anterolateral corner of the knee joint. Arthroscopy did not show the tumor because it was located in fat pad. The tumor was found during fat pad shaving and was resected arthroscopically. Synovial hemangioma was demonstrated histologically. The patient is asymptomatic postoperatively.

We report the case of a young patient with knee pain, limited range of motion, and swelling due to an intra-articular hemangioma. Clinically, the symptoms and findings were similar to those of a meniscal lesion. The diagnosis of an intra-articular tumor was made using magnetic resonance imaging. The tumor was excised, and the definitive diagnosis was made histologically. After tumor excision, the patient was pain free.

A 34-year-old woman presented to the rheumatology clinic with severe low back pain and arthralgia; later she developed bilateral knee pain and swelling, with limitation in ambulation and minimal improvement with nonsteroidal antiinflammatory drugs. Two weeks later she developed pain on the volar aspect of the right wrist and on the hypothenar region of the right hand. Examination showed swelling and tenderness of the right hypothenar region, tenderness and decreased flexion and extension of the right wrist, and bilateral knee effusions. The combination of arthritis and tenosynovitis raised the possibility of an ovarian tumor. A pelvic ultrasound revealed a complex, hyperechoic ovarian mass consistent with a cystic teratoma. Four weeks after removal of the teratoma, the polyarthritis and related symptoms resolved without therapy.

Lipoma is the most frequently encountered benign soft tissue tumor. However, intra-articular lipomas are rarely seen. Anterior knee pain is a frequent complaint of adults and is of diverse etiology. This 42-year-old female patient had severe anterior knee pain, unresponsive to medical treatment. Magnetic resonance imaging revealed an intra-articular tumor of the knee joint. Arthroscopic intervention and subsequent histological examination resulted in the diagnosis of strangulated lipoma originating from infrapatellar fat pad. We present clinical, radiological and operative features of this rare case of intra-articular lipoma.
