A 60-year-old woman was admitted to the hospital with a recurrence of low back pain. She was begun on a behavior modification pain control program. Follow-up x-rays, including CT scan of the lumbosacral spine, showed a large tumor of the L3 vertebra. She subsequently underwent surgical resection of a large chordoma of the L3 vertebra and had good resolution of her back pain.

The case of a 58-year-old woman with a primary adenocarcinoma of the duodenojejunal junction and an extraluminal growth is reported. The patient's chief complaint had been lumbago and emaciation, and a tumor in her left upper abdomen was found. Hypotonic duodenography showed a filling defect with ulceration of the duodenojejunal junction. Angiography revealed irregular vessels and a slight tumor stain. A CT scan further revealed that this tumor had invaded into the abdominal aorta with paraaortic lymphadenopathies.

An unusual case of intraosseous epidermal cyst is reported. The patient, a 45-year-old Japanese female, had suffered from lumbago and dysuria for about 15 years. X-ray examinations and CT scan revealed an expanded osteolytic tumor without marginal sclerotic change within the sacrum, which anteriorly invaded the surrounding soft tissues at the S2/3 level. At this time, chordoma was suspected, but epidermal cyst with foreign body granuloma was finally diagnosed from biopsy and surgical specimens.

A case of ureteral endometriosis is reported A 42-year-old woman visited our clinic on December 3, 1985, with the complaint of right lumbago. Intravenous pyelography showed right hydronephrosis and retrograde pyelography revealed ureteral stenosis at 11 cm from right ureteral orifice and 2 cm long. Primary right ureter tumor was suspected. The operation was performed on January 16, 1986 and revealed periureteral mass. The mass was removed with ureter and ureteroureterostomy was performed. The pathological diagnosis was extrinsic ureteral endometriosis. Ureteral endometriosis has rarely been described and only 17 cases have been reported previously in Japan.

We report a case of adrenal myelolipoma occurring in a 56-year-old man. The patient complained of lumbago and gross hematuria in October, 1981 and consulted a local doctor who pointed out a right ureteral stone and downward deviation of the left kidney. The patient was referred to our hospital for further examination. An echogram showed a high echogenic mass in the left flank which was compressing the spleen and the kidney. A CT scan revealed a large mass with heterogeneous low density contents. The mass was lobulated into several parts and their density suggested fat tissue. An aortogram showed an avascular mass and an adrenal venogram showed multiple veins which radiated to the periphery. Judging from these findings, a tentative diagnosis of adrenal myelolipoma was made and left adrenalectomy was performed. Histological examination revealed that this tumor was composed of adrenal tissues, fatty tissues and myeloid tissues. The postoperative course was uneventful. We discuss the necessity of aortography and selective adrenal venography to evaluate the origin of the tumor. To our knowledge, this is the tenth case report of adrenal myelolipoma in Japan and we have made a brief review of Japanese literature of adrenal myelolipoma.

Renal cell carcinoma during pregnancy is extremely rare. This 30-year-old patient was first seen on Dec. 18, 1983 at the 7th week of her 2nd pregnancy. She had asymptomatic hematuria. Cystoscopy revealed normal findings and sonogram showed well defined echogenic mass in the upper pole of the right kidney. She was admitted to our hospital on account of gross hematuria and severe lumbago on Jan. 19, 1984. The pregnancy was stopped by artificial abortion. Renal angiography revealed a rich-vascularized tumor in the upper pole of the right kidney. Preoperatively, arterial embolization was done and nephrectomy was performed. Histological examination showed a well-differentiated adenocarcinoma. There was no involvement of the renal vein and no lymph-node metastases were found in the operation specimen. The post-operative course was uneventful. This rare case of renal cell carcinoma during pregnancy is herein reported along with some discussion.

Work-up of a 62-year-old black male, with a complaint of low back pain for three months, revealed a mass in the porta hepatis, narrowing of the common hepatic duct, and dilated intrahepatic bile ducts. A 3 X 4-cm tumor in the common hepatic duct was excised and biliary continuity restored by hepaticojejunostomy. The excised mass was a carcinoid tumor with no evidence of secretory activity. The patient is well 24.5 months after treatment. The literature on carcinoid tumors of the biliary tract is reviewed.

A fifty-three-year-old woman was admitted for gross hematuria and left lumbago. She was diagnosed as having left renal arteriovenous malformation (AVM) by renal angiography, but contrast enhanced CT of her left kidney demonstrated a low density area of renal parenchyma adjacent to AVM. Because a renal tumor could not be denied completely, we performed translumbar nephrectomy. No renal tumor was found in the removed kidney, and she was diagnosed as having cirsoid type AVM of the left kidney. The low density area in contrast enhanced CT was suspected to be indicating the ischemic renal parenchyma due to the steal phenomenon of renal AVM. This is the first report of a renal AVM with ischemic renal parenchyma demonstrated by CT.

A 68-year-old woman presented with an 18-month history of low back pain followed by leg weakness, dementia, and incontinence. Myelography revealed an intradural, extramedullary block from L-2 to L-4, and cranial computed tomography demonstrated ventriculomegaly. Excision of a benign schwannoma resulted in rapid relief of back pain and more gradual normalization of mental function and hydrocephalus. Five similar cases of dementia and hydrocephalus complicating spinal cord tumor have been reported. A variety of mechanisms have been proposed to explain the association but the pathophysiology is still unclear. Spinal tumor should be considered in the differential diagnosis of dementia and of communicating hydrocephalus.

A 67-year-old man who had undergone partial cystectomy for a bladder tumor (stage pTlb) about two years earlier, was seen with the complaint of lumbago. KUB examination revealed multiple osteoblastic lesions at the lumbar vertebrae and pelvic bone. Furthermore, additional osteoblastic lesions were detected at the thoracic vertebrae, ribs, humeral bones, femoral bones and claviculae. Osteolytic lesions also were recognized at the ribs and claviculae, although osteoblastic change was dominant. Pathologically, transitional cell carcinoma had invaded the bones, and it was suspected that they had metastasized from the bladder tumor.
