Many advances have taken place during the past decade in the early mobilization of orthopaedic patients and those with injuries. Light-weight scientific splinting, early rigid fixation with compression plates and nails, and the early mobilization of patients with pathological fractures in neoplastic disease, have meant that these patients can often be mobile in days, and home in a fortnight, instead of spending months languishing in hospital. Scientific treatment of patients with neck and low back pain may similarly minimize chronic disability, while early rehabilitation can enable their early return to employment. It is the purpose of this paper to underline some of the important advances in treatment which have made this possible.

The case of a patient with low back pain and Paget disease of the L5, S1 vertebrae is presented. Electromyography revealed dysfunction in both the paraspinal and gluteal muscles. Postmortem examination showed sarcomatous transformation of the vertebrae with nerve root compression and skeletal muscle infiltration by tumor. The electromyographic abnormalities are correlated with the pathologic findings. The clinical features of neurologic dysfunction in Paget disease of the vertebral column and in osteosarcoma arising in Paget disease of the vertebral column are reviewed. Electromyography can provide valuable information in the evaluation of the patient with Paget disease of the vertebral column who is suspected of having neurologic dysfunction.

Endometriosis of the left kidney complicated by urolithiasis is described. This represents the tenth case in the world literature of kidney endometriosis. The most common symptom is low back pain. Macroscopic hematuria was only present in half the cases. The preoperative radiological findings have been interpreted as tumor in four cases, contracted, pyelonephritic kidney in two cases and congenital abnormalities in two cases.

A retroperitoneal seminoma in the presence of clinically normal testes can present a considerable diagnostic problem and therapeutic dilemma. We report on a patient with low back pain and inferior vena caval obstruction in whom no primary neoplasm could be detected by surgical exploration of the testes.

Intramedullary cysticercosis of the spinal cord is rare; there have been only 26 previous cases. We describe a patient with low back pain, paraparesis, lumbosacral sensory loss, and signs of meningeal irritation. The clinical diagnosis was spinal cord tumor, but at surgery there was a cysticercus cyst within the lumbosacral cord. Careful search did not reveal the parasite in any other part of the body. This is the first case of spinal cysticercosis reported in Japan.

Three cases of tethered cord syndrome are presented with reference of reported literatures. This syndrome is usually associated with spina bifida occulta and cutaneous manifestation such as subcutaneous tumor, skin dimple, hairy patch, and neavus. Presented symptoms are various progressive neurological deficits involving weakness and/or sensory disturbance of lower limbs, bowel and/or bladder dysfunction, leg and/or low back pain, and orthopedic pathological conditions such as scoliosis and/or foot deformities. Properly performed myelography is the only method of preoperative confirmation of this syndrome. As the surgical release of the tethered spinal cord is effective for these various symptoms except for foot deformity, early investigation and early surgery are recommended.

Out of 16 patients, spinal leptomeningeal neoplastic disease was diagnosed by MRI in 4 patients, myelography in 14 patients and CT myelography in 12 cases. MR was superior to myelography in 2 patients, in another 2 patients MRI was equally diagnostic. The cerebrospinal fluid of every patient contained malignant cells. Histological evidence for primary central nervous system tumors was found in 5 cases. In 10 cases, non-neuraxial malignancy consisted of small cell carcinoma of the lung (7 cases), and leukemia and lymphoma (3 patients). In 1 patient, primary leptomeningeal malignant melanoma was confirmed at autopsy. Preferential thoracolumbar neoplastic morphologic manifestation correlated with the presence of conus and cauda equina syndrome in 9 patients, low back pain, paresthesia and spinal root signs in 7 patients. False-negative interpretation of myelography in 2 patients with positive MR findings, and the impressive sensitivity of gadolinium Dota to improve visualization of subarachnoid spread, favor MRI as an alternative imaging technique in the assessment of patients with suspected intradural extramedullary malignancy.

A 62-year-old male was admitted with lumbago and gait disturbance for 3 months. He had complained of a cough for 3 years. His admission chest radiography revealed tumor shadow in right lower lung field. The patient was diagnosed as adenocarcinoma of the lung by transbronchial biopsy. Bone scintigraphy revealed multiple bone metastases with rib, lumbar and pelvic bone. Clinical staging was IV and performance status was 3. The patient was treated by combined chemotherapy with epirubicin 20 mg/m2, mitomycin C 8 mg/m2 and carboplatin 350 mg/m2. After 2 cycles he was able to walk by himself and the lung tumor regressed in 40% of pretreated size. This combined chemotherapy could be performed for outpatients because of the mild gastrointestinal side effects and little hydration.

Three cases of spontaneous peripelvic extravasation after the operation of rectal cancer are reported. In case 1, a 65-year-old female complained of left flank pain one month after high-anterior resection for rectal cancer. Drip infusion pyelography (DIP) and retrograde pyelography (RP) showed extravasation from the left renal pelvis. The ureteral stent was indwelled, and the extravasation showed remission. In case 2, a 55-year-old female complained of left lumbago 6 months after Miles' operation for rectal cancer. DIP showed extravasation from the left renal pelvis. The same findings were confirmed on the repeated DIP after 10 days. The ureteral stent was indwelled, and the extravasation was cured. In case 3, a 69-year-old male complained of left flank pain and left abdominal tumor 10 months after Miles' operation for rectal cancer. DIP and RP showed extravasation from the right renal pelvis, and computed tomographic (CT) scan showed urinoma formation. Drainage of the urinoma was performed and the ureteral stent was indwelled. The urinoma and the extravasation was cured. We emphasized the usefulness of indwelling the ureteral stent for the conservative management of spontaneous peripelvic extravasation caused by a malignant tumor, and a discussion of the relevant literature follows.
