Intracardiac masses are rare in infants and children. Early detection is essential to their successful management. We present seven patients in whom echocardiography established the diagnosis and was crucial in the management. Three of the masses were primary cardiac tumors and four were thrombi. Patient 1: an infant with a calcified left ventricular fibroma. Patient 2: a neonate who presented with
cyanosis
due to obstruction of the right ventricular inflow tract by a fibroblastic
tumor
. Patient 3: an infant with a right atrial myxoma presenting as sepsis. Patient 4: a child who had a pulmonary embolus after a pulmonary valvotomy and was found to have a right ventricular thrombus. Patient 5: a child with a right atrial thrombus following a Fontan procedure for univentricular atrioventricular connection. Patient 6: a child with a left ventricular thrombus due to a dilated cardiomyopathy in association with epidermolysis bullosa. Patient 7: An infant with bilateral lobar emphysema, an aorticopulmonary window with left ventricular fibroelastosis, who developed a left ventricular thrombus.

Current knowledge strongly supports the theory that the carcinoid spectrum reflects the clinical signs of production, by a carcinoid
tumor
or by metastatic lesions, of one or more humoral substances. Serotonin, the kinins, histamine (and perhaps other amines), adrenocorticotropic hormone and additional hormones are manufactured by these tumors. Elevated concentrations of serotonin, the kinins and histamine have been found in patients with the carcinoid syndrome, which includes right-sided valvular heart disease, peripheral vasomotor disturbances, bronchoconstriction, borborygmi and an unusual type of
cyanosis
.

Intraoperative pulmonary
tumor
embolism occurred during right hepatectomy for primary hepatic carcinoma. The
tumor
embolus produced abrupt arterial hypotension, tachycardia, an increased central venous pressure, intense
cyanosis
, followed by cardiac arrest. Pulmonary tumor embolism resulting from hepatic tumors is rare and has not been reported to have occurred intraoperatively. When
tumor
thrombosis has been detected or suspected, hepatic vascular exclusion should be considered for the prevention of intraoperative embolism.

A 4-day-old female had an uneventful prenatal course and delivery. Immediate
cyanosis
improved with oxygen therapy. Cardiomegaly on chest radiograph, right bundle branch block on electrocardiogram, and apparent increased tricuspid valve excursion on echocardiography led to a diagnosis of Ebstein's anomaly. Death occurred unexpectedly. Autopsy demonstrated a rhabdomyoma of the interventricular septum, which had produced obstruction to right ventricular outflow and had interrupted the right bundle branch of the conducting system. It appears that the massive size of the septal
tumor
led to its misdiagnosis on the echocardiogram.

Pleural effusions can cause dyspnea and
cyanosis
, and are caused by cardiomyopathy, pyothorax, FIP, FeLV-related disease and trauma. Thoracentesis is used to obtain fluid samples for cytologic examination and culture. Radiographs made after thoracentesis may reveal the cause. Lymphosarcoma causes a sterile exudate containing neoplastic cells. Congestive cardiomyopathy causes a transudate or modified transudate. The sterile exudate of FIP has a proteinaceous background on cytologic examination. The exudate of pyothorax is septic. Treatment depends on the cause but generally includes thoracentesis and supportive care. Cardiomyopathy causes dyspnea,
cyanosis
, murmurs, gallop rhythms and other arrhythmias. Radiography reveals a globoid heart in the congestive form and a "valentine-shaped" heart in the hypertrophic form. Treatment of congestive cardiomyopathy involves use of furosemide, but is usually unrewarding. Hypertrophic cardiomyopathy is treated with propranolol. Fever may be caused by infection, immune-mediated disease,
neoplasia
and unknown causes. Treatment is aimed at removal of the underlying cause.

Authors described three infants under three months of age with cardiac rhabdomyoma observed during a period of eight years. There was no clinical or pathological evidence of tuberous sclerosis. During this period of time they found two cases of tuberous sclerosis, without associated rhabdomyoma, among 2,457 necropsies. On the other hand none of 11 children with tuberous sclerosis under observation in this centre had any clinical evidence of rhabdomyoma. Presenting symptoms of the three cases were
cyanosis
, heart failure, heart murmurs and alterations of heart rhythm. Diagnosis was suspected on the bases of eco and angiocardiographic studies and was finally confirmed by necropsy. Surgical exeresis of the
tumor
was attempted in one of the cases, but was not possible because it was too large.

We report the case of a 63-year-old woman presenting with progressive dyspnea of insidious onset culminating in severe central
cyanosis
. Conventional studies including M-mode echocardiography did not point to the diagnosis. At cardiac catheterization a large right atrial myxoma producing partial dynamic tricuspid obstruction was discovered along with an atrial septal defect with a right to left shunt. After successful surgical excision of the
tumor
and repair of the atrial septal defect, the patient has been totally relieved of her presenting symptoms.

A 17-year-old male presented with a four-day history of increasing shortness of breath and right basal pleuritic chest pain. Physical examination revealed central and peripheral
cyanosis
, sinus tachycardia, elevation of central venous pressure, hypotension, and added third heart sound. Echocardiography (M-mode and two-dimensional) showed a highly mobile mass in the right atrium apparently attached to the interatrial septum. A diagnosis of right atrial myxoma with pulmonary embolization was made and urgent surgery advised. At operation a string of thrombus in the right atrium (presumed from peripheral venous site) occluding a patent foramen ovale was found. The usefulness of echocardiography in diagnosing atrial thrombus and in differentiating clot from
tumor
is discussed.

polysaccharide, lentinan (beta-1, 3 Glucan) was studied on the acute toxicities using both sexes of mice (ICR) and rats (CD) treated by intravenously (i.v.) intraperitoneally (i.p.), subcutaneously (s.c.) and orally (p.o). LD50 values in mg/kg body weight were essentially the same regardless of species as well as sexes and estimated as follows: 250-500 (i.v.) greater than 2500 (i.p., s.c., and p.o.).
Cyanosis
, convulsion and death were observed in both species of animals administered (i.v.) with only higher dosages of lentinan. No remarkable toxic signs being specific from lentinan were observed in any cases of treatment, i.p., s.c., and p.o. Gross findings: enlargement of the spleen (i.v., i.p., s.c.) and coarse nodular surface of the kidney (i.v.) in the both species of animals, erythema of the ears (i.v., i.p., s.c.) in mice, mesenteric petechial hemorrhage of the lung and abdomen (i.v.) enlargement of the mesenteric lymph nodes (i.v.) and edema of the diaphragm and intestine (i.p.) in rats were observed. In parallel, another sample of lentinan for clinical use prepared by freeze-dried procedure was tested in both sexes of mice and rats treated by i.v. alone, comparing with a original sample mentioned above. So far as the acute toxicities of lentinans concerned, no significant differences between two preparations were observed.

A case of nasopharyngeal teratoma with the appearance of a hairy polyp in one month old infant is presented. The patient had a previous history of recurrent attacks of cough, dyspnea and
cyanosis
on being fed or in a certain body position (prone). The prone position shifted the tip of the
tumor
with its long stalk over the entrance of the larynx followed by upper airway obstruction. Prompt surgical extirpation prevented the risk of succeeding fatal respiratory event. This uncommon congenital teratoma arising from the nasopharynx should be taken into consideration in differential diagnosis of neonatal or infantile upper respiratory distress.
