From 1970 to June, 1976, 56 patients who had multiple metastatic tumors of the lung were treated by lung resection. Most of the bilateral lung lesions were removed through a median sternotomy so as to avoid staged bilateral thoracotomy. The surgical mortality was 1.8%. A total of 26 patients are alive at 7 to 69 months (estimated median survival, 20.7 months). Patients with tumor doubling time of less than 40 days had lower survival results (median, 9.5 months), compared to patients with tumor doubling time of more than 40 days (median not yet reached). The type of primary tumor, tumor-free interval, number of lesions removed, and presence of unilateral as opposed to bilateral lung metastases did not seem to affect the therapeutic results.

The anti-tumor effect of vitamin A and/or BCG was investigated in Lewis lung tumor system. Tumor growth and lung metastases were significantly suppressed, when tumor cells were mixed with BCG and inoculated subcutaneously into vitamin A-treated animals. Survival time was also prolonged by the same treatment. Vitamin A alone, without BCG, showed no effect on tumour growth, lung metastases or survival time.

The Madison 109 (M109) tumor was discovered in 1964 in the lung of a BALB/c mouse. This experimental carcinoma is maintained in vivo by sc passage in the right axillary region. When implanted im (5 X 10(5) cells) into the right hind leg of BALB/c mice for testing, the primary progresses with metastases to the lung, spleen, and liver. The metastases to the lung are visible within 3 weeks and result in the death of the host in about 35 days after tumor implant. Implantation of a lung nodule is tumorigenic and lethal. Pyran polymer therapy delayed the appearance of lung metastases, inhibited the growth of the primary tumor, and significantly increased the lifespan of BALB/c mice inoculated with the M109 tumor. No spontaneous regression has been observed and very few "no takes" have occurred in untreated BALB/c mice inoculated with at least 500 M109 cells. Of the 82 agents tested so far, the M109 model has selected active agents such as actinomycin D, adriamycin, daunorubicin, DNA, procarbazine, and pyran polymer. It has not shown sensitivity as tested to several standard therapeutic agents including cytosine arabinoside, BCNU, hydroxyurea, mechlorethamine, melphalan, triethylenemelamine, and vincristine.

A gastric choriocarcinoma cell line capable of producing human chorionic gonadotropin (HCG) was studied by sc transplantation and serial passages into nude mice fed under specific pathogen-free (SPF) conditions. The tumor-take rate at the serial heterotransplantation in SPF mice was high (greater than 95%), in contrast with a low rate in coventional animals (35%). The restoration of morphology and function of the original neoplasm were exclusively verified in SPF mice. Multiple lung metastases were found in 2 animals. The production, storage, and excretion of HCG by tumor cells were confirmed by its high content in serum and cystic fluid in tumors and by its intracellular localization. The tumor cells also contained a specific placental alkaline phosphatase in their membranes. The cells were various trophoblastic types ranging from primitive cytotrophoblasts to typical syncytial cells. The hormone effects of the tumor on sex organs of tumor-bearing animals were evident.

Cell suspension of a human breast cancer cell line (Hattori line) was injected intraperitoneally into an athymic nude mouse to produce ascites form breast cancer (peritoneal carcinomatosis). Subsequent serail transfers of cancer cells in ascites were also successful in mice. All male and female nude mice injected 1 X 10(7) tumor cells died of accumulation of ascites after a latency period averaging 4 weeks, with one exception which died of a wasting disease. Multiple lung metastases were observed in some mice. The tumor cells retained cytological characteristics of the original cell line, and histology of the infiltrating tumor in the peritoneum and omentum was that of poorly differentiated adenocarcinoma. Differentiation not only toward acinar or duct lining cells but also toward myoepithelial cells was observed by histochemistry, immunohistochemistry, and electron microscopy.

The survival of Lewis lung tumor cells has been studied after a single ip dose of BCNU. In dissectible subcutaneous and intramuscular tumors and artificial lung metastases survival was measured by colony formation in soft agar and in vivo in the lung. The long-term control of small lung nodules by single doses of BCNU was studied and cell-survival estimates were inferred. The results demonstrate that the cytotoxic action of BCNU shows a strong dependence upon tumor size.

Noncurative excision of a primary sc Lewis lung tumor performed on Day 7 or later results in an increase in the thymidine index and growth rate with minimal changes in the cell cycle parameters of the lung metastases. The stimulation of the lung nodules is accompanied by a small but consistent decrease in median lifespan. Sham surgery performed on Day 3 or later also results in a decrease in median lifespan and an increase in the thymidine index of the undisturbed primary tumor. Artifical metastases (10(6) cells iv) are inhibited by the presence of a second (sc) implant and the median lifespan of the doubly implanted mice exceeds that of mice bearing iv implants only. In mice bearing widely metastasized Lewis lung carcinoma, surgery alone may have a detrimental effect on life expectancy, but the residual tumor foci, stimulated to more rapid growth, should be appropriate targets for adjuvant chemotherapy.

In a mouse model the retention of125I-5-iodo-2' -deoxyuridine-labeled tumor cells in the lung after iv injection was compared with the formation of tumor colonies in the lung 15 days after injection. The modification in lung retention after treatment with cyclophosphamide (CP), isophosphamide, Corynebacterium parvum, and heparin paralleled the changes in lung-colony numbers by these treatments. Since the modifications could be identified as early as 1 hour after iv administration of tumor cells, further evidence was obtained for the conclusion reached earlier that the modification of lung metastases by CP and Corynebacterium parvum is not due to immunologic mechanisms. Comparison with the lung retention of heat-killed tumor cells and living embryonic cells showed that CP induced an increased retention of all three cell types. In contrast, Corynebacterium parvum decreased the retention of living tumor cells, but failed to modify the retention of dead tumor cells, but did not affect the retention of living embryonic cells. Since no similar cell types were affected by the various treatments it seems likely that different mechanisms are involved. Apparently CP decreases the nonspecific resistance against the lodging and growth of any type of cell in the lung.

Cryosurgery achieves a local tumor destruction and at least in many experimental models it also produces an antitumor immunity. Cryosurgery was therefore applied to 15 patients with multiple lung metastases. In 2 cases a good palliation with a temporary standstill of tumor growth and in 3 cases of positive effect with a decrease and disappearance of metastases was noted. In two of these cases metastases of the contralateral lung also decreased or varnished. This can be explained by an immune mechanism. Big tumors and fibrosarcomas are not suited for cryosurgery, whereas hypernephroid renal carcinomas and malignant teratomas give more promising results.
