Angiosarcoma of the heart is a rare neoplasm. The clinical manifestations are surprisingly consistent, but the diagnosis is rarely made before death. A patient with such a tumor was described here and the current literature was reviewed. Venous angiography and pericardiectomy are recommended in patients with hemorrhagic pericardial tamponade when the cause is unclear.

Hepatic artery ligation is useful as a palliation of irressectable hepatic tumors, but does not always produce a satisfactory result. Hepatic tumor with high vascularity is expected to respond more favorably. In this context, primary liver cell carcinoma and carcinoid tumor or leiomyosarcoma of the liver have been satistfactorily treated by hepatic artery ligation. A case is presented of hemangiosarcoma of theliver and spleen treated effectively by hepatic artery ligation, splenectomy, and postoperative intraportal infusion of 5-fluorouracil, as indicated by the regression of hepatic tumors on postoperative scanning and arteriograpms.

Angiosarcoma of the liver has recently been publicized because of its association with polyvinyl chloride (PVC) polymerization workers. Four cases of this rare tumor have been observed at the Marshfield Clinic within a 29-month period. There are no factories that manufacture PVC products in the immediate area, nor were any of the victims ever involved in such manufacturing work. Thus, it is possible that other factors may be related to this cluster of the disease.

A total of 13 cases of ASL have been documented to date among VC workers in four different plants in the United States. In this particular industrial population, this number of cases represents at least a 400-fold increase over expected incidence for this extremely rare tumor. The first case occurred in 1961. Average age at diagnosis is 48.2 years. Average length of time between initial VC work and diagnosis has been 20.3 years. A detailed review of 7 cases associated with one plant revealed that clinical features varied from little or no overt illness prior to diagnosis to advanced liver disease with portal hypertension and marked splenomegaly. Portal fibrosis was present in all 7 of these ASL cases as well as in 4 additional cases with non-malignant liver disease among VC workers at the same plant. These findings suggest that exposure to VCM in industrial settings can produce hepatic fibrosis with angiosarcoma as a late manifestation. Conventional liver function tests may not be sensitive indicators of such liver impairment, at least in its early stages.

The third reported case of primary angiosarcoma of the paranasal sinuses is presented. The neoplasm occured in the left maxillary sinus of a 26-year-old white woman. The patient was treated with external irradiation with cobalt of a total dose of 7,000 rads. No evidence of recurrence has been reported.

Analysis of four cases of hemangiosarcoma of the liver and review of the literature indicate that these tumors are either predominantly cystic and fairly well differentiated or are more solid and poorly differentiated. Well-differentiated hemangiosarcomas may resemble peliosis hepatis or other benign conditions. The tumor was associated disseminated intravascular coagulation and fibrinolysis syndrome in one of our cases. One of our patients had received thorium dioxide (Thorotrast) but none seemed to have been exposed to arsenicals or to vinyl chloride.

The clinical and histopathological characteristics are described in seven cases of malignant hemangioendothelioma of bone. This rare neoplasm affects all age groups and almost any bone. Metastases appear in the skeleton even before such lesions are seen on pulmonary roentgenograms. Prognosis is poor, with a five-year survival for only two of the six patients in this series. One patient is still alive and free of symptoms more than twelve years after diagnosis of widespread tumor destruction of the frontal skull bones, which was successfully controlled by radiation therapy. In addition, a recently diagnosed case of this tumor arising in a chronic non-fistulous osteomyelitic bone is described.

Histologic examination of liver tissue (eight autopsy and 18 biopsy specimens) and five spleens from 20 workers with vinyl chloride polymerization showed hepatic angiosarcomas in 15. In addition, a peculiar pattern of progressive portal-tract, inconspicuous intralobular and conspicuous capsular fibrosis was observed in the five workers without angiosarconma, in all the seven patients with angiosarcoma from whom tumor-free portions of the liver were available, and in two tumor-free biopsies from patients subsequently found to have angiosarcoma. The fibrosis was accompanied by splenomegaly. Hypertrophy and hyperplasia of both hepatocytes and hepatic and splenic mesenchymal cells were also seen. The histologic similarity to chronic inorganic arsenical poisoning, in which angiosarcomas also occur, and to idiopathic portal hypertension (Banti's syndrome) suggests that the latter syndrome at times results from unknown toxic, possible environmental, chemicals.

Two cases of intense lung activity uptake during routine liver imaging are presented. One patient died 6 days after uptake was seen, and had Kupffer cell tumor, or liver angiosarcoma, at autopsy. The second patient with an acute infection superimposed on alcholic hepatitis showed intense lung uptake on the tenth day of a sustained course of very high fever. A repeat liver scan after the patient became afebrile showed no lung uptake.

A case of angiosarcoma of the pericardium in a 30-yr-old man is reported. The clinical diagnosis of pericardial neoplasm was assumed on the basis of angiocardiography. The pathological findings of the heart are shown in detail. A survey of the clinico-pathological data in the 15 reported cases of angiosarcoma of the pericardium is presented.
