We describe a method for determination of urinary 5-hydroxyindole-3-acetic acid, a metabolite of serotonin and tryptophan and a useful indicator of metastasizing carcinoid tumor. The analysis is based on the color development with ferric chloride added to the residue obtained on evaporating a diethyl ether extract of the sample that has been acidified with sulfuric acid and saturated with sodium chloride. The color is measured by 510 nm. The reaction was made highly specific by the acidification so that nonspecific reactions with catecholamines and with other phenolic compounds such as salicylic acid could be eliminated.

Herein is reported an ovarian strumal carcinoid in a mature tetratoma from a 54-year-old women. Ultrastructural examination revealed numerous electron-dense intracellular granules, which were intrepreted as neurosecretory granules. They corresponded to argyrophil granules observed by light microscopy and were identified within neoplastic cells arranged in both follicular and trabecular patterns. These findings corroborate the contention that the strumal carcinoid itself is a pure neuroendocrine tumor devoid of thyroid follicular epithelium, but do not disprove its possible origin from parafollicular cells of adjacent teratomatous thyroid tissue. There was no recurrence of tumor during the 20 1/2 years the patient was followed.

A review of thirty cases of primary malignant small bowel tumors is presented. Chronic obstruction was the presenting symptom most frequently encountered, with an acute abdomen from perforation of the tumor next in frequency. Adenocarcinoma and carcinoid were about equally encountered, and more than half of all tumors were found in the ileum. Three patients are alive and well, two are alive on chemotherapy, and all others are dead. Five of these died of other causes. Of twenty-seven patients explored for symptomatic small bowel cancers, only one is alive free of tumor. Malignant small bowel tumors are difficult to diagnose early and have a poor prognosis.

Tumorlets of the lung generally have been considered benign nests of cells, usually found with bronchiectasis or fibrosis. We saw a patient with Cushing's syndrome whose tumorlets ultimately became roentgenographically visible as multiple small nodules throughout both lungs and metastasized to the mediastinum. The plasma corticotropin concentration was increased, as were the concentrations of corticotropin-like peptides in the tumor tissue. This experience lends support to the concept that some tumorlets arise from Kulchitsky's cells in the epithelium of peripheral bronchioles, as do carcinoid tumors, and have the same peptide-producing potential. The possibility of such tumors should be considered in patients with Cushing's syndrome who have no initial roentgenographic abnormality.

There was a tumor in the left breast which was suspected to be carcinoid tumor at excisional biopsy. Autopsy revealed the same tumor in the right nipple. In the cells of the bilateral tumors a number of argylophil granules were diffusely demonstrated and the neurosecretory ones were verified electronmicroscopically. In the urine of the patient a moderate amount of norepinephrine was excreted. It was presumed that the norepinephrine might have been produced from the breasts. In the literature reviewed, the case of breast carcinoid has not been reported previously.

This is the first report of the simultaneous occurrence of mucinous carcinoid and endometriosis in an inside-out appendix. The literature concerning carcinoids, endometriosis and intussusception of the appendix is reviewed. The mucinous, or goblet cell carcinoid was positive for mucin, argentaffin and argyrophil stains. Paneth cells were present. Two and one-half years with no tumor recurrence suggest good prognosis as previously reported.

The work is concerned with the study of gastric tumors showing histological similarity with carcinoids, but differing from them by morphological and clinical signs of malignancy. A conclusion is made on the carcinoid nature of the neoplasms under consideration, the following evidences being presented: 1) despite some differences the tumors correspond histologically to classical carcinoids; 2) as in common carcinoids, argyrophil or argentaffine granules may be revealed in tumor cells; 3) the occurrences of tumors is related with a peculiar neoplastic transformation of glandular epithelium of the stomach resembling carcinoid rather than cancerous one. Not excluding the possibiltiy of their origin from common carcinoids, the author believes that they are likely to be primary malignant tumors.

Carcinoid tumors can be found in the entire alimentary tract. Stomach carcinoids make up about 3% of all carcinoids; 0.39% of all stomach tumors are carcinoids. Multiple stomach carcinoid is a medical rarity. In spite of an invasive growth, carcinoids with a diameter of less than 1 cm are considered semimalignant, those with a diameter of more than 1 cm are considered sure malignant. There are 8 cases of multiple gastric carcinoids described in the literature. In our case we found macroscopically 31 polyps with a diameter of between 0.4 and 2 cm in the stomach. Histologically they were growing invasively with small tumor groups in the lymph ducts. Therapeutically total gastrectomy was necessary.

A description of malignant carcinoid of alveolartrabecular structure developing in the edge of chronic progressive gastric ulcer is presented. Granules detectable by argirophilic method of Grimelius, toluidine blue after acid hydrolysis, and lead hemotoxylin were found in the cytoplasm of tumor cells. Changes in the epithelium of the type of cancer in situ as well as marked hyperplasia of argirophilic cells were observed in gastric mucosa in the edges of the ulcer. Although this observation may be interpreted as a peculiar variant of Zollinger-Ellison syndrome, it is more likely that this is a malignized chronic ulcer with carcinoid differentiation of the tumor tissue.
