Clinical, serological and lymphocyte studies were done on 435 patients with biopsy proved anaplastic nasopharyngeal carcinoma (NPC) in various clinical status, at the National Taiwan University Hospital, from January 1980 through June 1983. Studies on 134 normal control were also done. Using immunofluorescent antibody method, seropsitive rates of the antibody titers against viral capsid antigens (VCA) and early antigens (EA) of Epstein-Barr (EB) virus were 70.8%-100% for anti-VCA/IgG titers (greater than or equal to 1:640), 81.0%-100% for anti-VCA/IgA titers (greater than or equal to 1:40), 66.7%-93.8% for anti-EA/IgG titers (greater than or equal to 1:160), and 40.0%-87.5% for anti-EA/IgA titers (greater than or equal to 1:40) in NPC patients with disease. They decreased to 10.5%-21.7% in remission patients. In contrast, they were less than 5% in the control. Mean total serum IgG and IgA levels were moderately increased to around 1,500 mg/dl and 300 mg/dl respectively, in all patients. The increase was most remarkable in patients with liver metastases. In control the values were 1,211 mg/dl and 223 mg/dl, respectively. Mean serum IgM, C3 and C4 amounts of NPC patients were not significantly different from those of the normal control, the latter were 129, 80.3 and 43.2 mg/dl, respectively. Serum acid phosphatase and calcium levels of NPC patients were all in the normal range of 0.1-2.0 BU/ml and 2.0-3.0 mmol/dl, respectively. Serum GOT, GPT, alkaline phosphatase,

and mucoprotein were elevated either alone or in combination in some patients before treatment, in many patients with neck recurrence or distant
metastases
, but in all patients with liver metastases. Using monoclonal antibodies (Ortho Inc., U.S.A.) to define lymphocyte subsets, B lymphocytes comprised about 12% and T lymphocytes about 60% in the patients, whereas they were 11.9% and 73.1% in the control. The helper/suppressor ratio was 1.7 in the control and about 1.0 in NPC patients, and was only 0.8 in remission patients. The lack of correlation between the seropositive rates of anti-VCA antibodies and the helper/suppressor ratio might indicate different manifestations of humoral and cellular immunity in patients with NPC.

Extensive clinical, serological and biochemical studies were done during 1980-1982 on 351 patients to aid in the diagnosis of the disease, especially with recurrence or metastasis. (mastectomy + partial axillary resection) who participated in the Danish Breast Cancer Group's controlled clinical trial of radiation (RT) alone, and RT + levamisol. The follow-up (mean = 3.45 years, range = 3-4 years) included physical examination every three months, x-ray of chest, bone scan, and x-ray of axial skeleton every six months. detected during the first three years of postoperative follow-up study. Twenty patients had recurrences. A stepwise discriminant analysis of the surgical pathology quantities showed that all quantities except tumor size contributed significantly (P less than 0.01) to the discrimination between controls (no recurrence after 3-4 years) and patients who had recurrences within three years. When each of the biochemical quantities was combined with the surgical pathology data it was found that only urinary total hydroxy-proline to creatinine ratio improved the discrimination significantly (P less than 0.05) and that the effect was marginal.

The activities of serum
lactate dehydrogenase
(S-LDH) and S-LDH isoenzymes were determined in 252 patients with a history of testicular germ cell tumors (TGCT). Fifteen of 37 patients with TGCT lesions and seven of 215 without had raised levels of S-LDH (above 8.0 mukat/l (480 U/l)). Of the patients with TGCT lesions, four had only raised S-LDH-1 levels, one only raised S-LDH-2 (and normal S-LDH), two only raised S-LDH-3 (one with normal S-LDH), and 10 had five combinations of raised levels of S-LDH isoenzymes with a predominance of S-LDH-1. S-LDH and S-LDH-1 correlated significantly with the total tumor volume in the patients with TGCT lesions, especially pronounced in those with lesions from seminoma. Of 34 patients with TGCT

, 13 with raised S-LDH levels lived significantly shorter lengths of time than 21 with normal S-LDH. Similarly, 11 with raised S-LDH-1 (above 3.0 mukat/l (180 U/l) lived significantly shorter times than 23 with normal S-LDH-1. S-LDH is a valuable tumor marker in patients with TGCT, especially in those with seminoma. Routine determination of S-LDH isoenzymes in addition to S-LDH in patients with TGCT is not recommended. In patients with a history of TGCT and an unexplained elevation of S-LDH levels, a raised S-LDH-1 level indicates the presence of TGCT lesions.

lactate dehydrogenase
(S-LDH) was raised in 41 of 82 patients with testicular germ cell tumors, none of three with subclinical testicular tumors, and 10 of 341 without testicular germ cell tumors, 215 of whom had a previous history of tumor. 13 of 38 with testicular germ cell tumors, none of three with subclinical testicular germ cell tumors, and seven of 341 without tumor lesions had raised levels of isoenzyme S-LDH-1. 46 of 52 had concordant changes in S-LDH and total tumor volume whereas in six this relationship was not noted. Concordant changes in S-LDH-1 and total tumor volume were found in most cases. Higher S-LDH and S-LDH-1 levels were noted in the blood from the testicular vein on the side of the tumor at orchiectomy than in peripheral blood. The S-LDH activity predicted survival in a multivariate analysis of risk factors in 39 patients with stage 3
metastases
from testicular germ cell tumors. 70-75% of 37 with tumor lesions had concordantly raised or normal S-LDH and serum concentrations of alphafetoprotein (S-AFP) and human chorionic gonadotropin (S-hCG); 25-30% had discordant levels. The diagnostic specificity of S-LDH and S-LDH-1 with regard to tumors in 252 with a history of testicular germ cell tumors was 68%, the diagnostic sensitivity 90%, the nosographic specificity 97%, the nosographic sensitivity 41% for S-LDH and 35% for S-LDH-1, and the effectiveness of the tests 88% for S-LDH and 90% for S-LDH-1. Tumor tissue from a testicular germ cell tumor transplanted into athymic mice had a high activity of LDH-1. There was a good correlation between S-LDH-1 and tumor volume. S-LDH may be used as a tumor marker in patients with testicular germ cell tumors in addition to other diagnostic tests. In patients with a history of testicular germ cell tumors and an unexplained elevation of S-LDH, raised S-LDH-1 may indicate the presence, and normal S-LDH-1 the absence, of testicular germ cell tumors.

. Increased serum LDHk appeared in a wide variety of patients with cancer, although it appeared to be more common in certain types of cancer. Increased serum LDHk activity was also found in the sera of some patients with nonmalignant disease. The activity of serum LDHk may be useful to monitor recurrence or response to therapy in certain types of cancer.

A case of adenocarcinoma of Bartholin's glands in a middle aged female is presented. This well known, but rather uncommon malignancy is reported since it shows an unusual structure consisting of a combination of two different histological entities, namely an adeno-cystic component and adenopapillary component. An interval of 21 years elapsed between the appearance of the primary lesion and the recurrence which showed extensive haematogenous and lymphogenous
metastases
and proved fatal within two years of the recurrence. The study includes an enzyme histochemical evaluation and a fine needle cytological examination. The former revealed a high degree of activity of the NADP+ regenerating enzymes of the pentose shunt as well as a definite, but variable, activity of many lysosomal enzymes, mainly non specific esterases, acid phosphatase, aryl sulphatase and beta-glucuronidase. In contrast to other adenocarcinomas arising in the female genital tract, especially those of endometrium and ovary, the tumour cells showed only a slight activity of alkaline phosphatase, As is the case in many other adenocarcinomas, the activity of
