month old boy with acute lymphoblastic leukemia diagnosed on 17th September 1971, was treated according to the Memphis Study VII of Pinkel. Bone marrow examination, 39 days after treatment, showed complete remission. The patient has since then been in continuous complete remission. Therapy was suspended twice (29 days and 18 days) because of interstitial pneumonia. The treatment was terminated on 9th October 1974 nearly 3 years after onset of the disease. Because of papillary thyroid carcinoma with
metastases
in the regional lymph nodes, a total thyreoidectomy with removal of lymphnodes was performed on 30th April 1977. Because of a relapse in the neck lymphnodes, the patient was treated in December 1977 with radioiodine. The prognosis is however not discouraging.

occurred in the spinal cord, femur and lungs. Treatment consisted of surgery and irradiation for the spinal cord

and ten courses of combination chemotherapy, Adriamycin, vincristine, cyclophosphamide (6 courses) and megestrol acetate (continuous since course 7). This therapy resulted in a complete clinical remission which has been maintained for eight months since completion of chemotherapy. It is suggested that this regimen be employed in patients with this rare and lethal tumor.

-year-old man with metastatic undifferentiated carcinoma of probable thymic origin is presented who achieved a pathologic complete response with cisplatin, vinblastine, and bleomycin chemotherapy. Additional consolidative chemotherapy with cisplatin and etoposide was administered. The patient remains disease-free 5 years after diagnosis. Cisplatin, vinblastine, and bleomycin chemotherapy appears to have significant activity against thymic carcinoma.

-year-old woman presented with a 12-month history of epigastric pain, and for 3 months she had noticed a mass in the right hypochondrium. She had taken 'Norinyl-1' (norethisterone 1 mg and mestranol 50 mcg) for 5 years. She smoked 20 cigarettes a day but drank little alcohol. Physical examination revealed irregular hard hepatomegaly 10 cm below the right costal margin. Hepatitis B surface antigen was not detected in the serum and alpha fetoprotein levels were normal ( 10 M.R.C. units). A liver scan showed a large space-occupying lesion in the right lobe of the liver, and liver biopsy revealed a cholangicarcinoma with striking fibrous reaction. Multiple shadows consistent with
metastases
were present on chest X-ray, but no bony deposits were found on radiological skeletal survey or bone scan. The serum calcium was persistently high (2.74-2.92 mmol/l) but fell on prednisolone therapy. Serum parathyroid hormone levels were normal. A causal relation between oral contraceptives and hepatic adenoma is now generally accepted, and several patients with hepatocellular carcinoma have also been reported. We have been able to find only 1 previous report of cholangiocarcinoma in a young female taking oral contraceptives, and there is 1 report of this tumor in a man taking high doses of anabolic steroids for refractory anemia. This tumor has its peak incidence in the 6th decade and is very rare in the 3rd decade. The association with hypercalcemia due to pseudohyperparathyroidism is well recognized. In only some cases are parathyroid hormone levels raised, and the cause of the pseudohypercalcemia in our patient is unknown.

-year-old gastric carcinoma patients who showed umbilical and scrotal metastasis is reported. Cutaneous
metastases
are seen in 3-4% of malignant tumors. The most frequent primary sites are the breast, lung, stomach, ovary, colon, uterus and kidney. Intraabdominal malignancies may produce umbilical metastasis. The primary cancer is usually in the stomach, colon, pancreas or ovary. Umbilical metastasis from the stomach seen in our case is known as Sister Joseph's nodule which is a sign of a poor prognosis. Metastatic scrotal carcinoma is a relatively rare condition which occurs by the hematogeneous pathway. However, in our case, direct invasion of tumor cells though the peritoneal cavity and along the lymphatic system of the spermatic cord was the most likely cause of scrotal metastasis.
