Progressive unilateral sensorineural deafness and tinnitus developed in a 59-year-old woman over a 1-year period. Clinical examination showed a tumor mass which almost completely filled the tympanic cavity, and grew around the auditory ossicular chain. Histological findings revealed the tumor to be a primary carcinoid of the middle ear. Neuro-endocrine differentiation was confirmed immunohistochemically by positivity for neuron-specific enolase, chromogranin, pancreatic polypeptide and synaptophysin. Using electron microscopy, neuroendocrine granules could be visualized. In addition, both light and electron microscopy revealed that cells had an epithelial differentiation with
mucin
granules while immunohistochemistry showed a positivity for cytokeratins. The detection of intermediary filaments (immunohistochemically with vimentin and under electron microscopy) was unique to this neoplasm and has to be considered in distinguishing the carcinoid tumor from the papillary adenoma of the middle ear. Tumor prognosis is excellent with radical extirpation from the middle ear. In the case presented, there has been no evidence for either recurrence or
metastases

In an attempt to better understand the biologic behavior of neoplastic cell metastasis, a histochemical study with the use of six different lectins and a monoclonal antibody against human pulmonary surfactant apoprotein (PE-10) was carried out on primary adenocarcinomas of the lungs and their regional (usually lymphatic to lymph nodes or contralateral lung) and distant (usually hematogenous to extrathoracic organs) metastatic lesions of 54 postmortem cases. Primary pulmonary adenocarcinomas were classified further into acinar, papillary, and solid types according to WHO histological typing. Acinar type primary adenocarcinoma of the lungs showed significantly higher (p less than 0.05) binding reactions to Ricinus communis-I (RCA-I) and Ulex europaeus-I (UEA-I) lectins than solid type adenocarcinoma. With six different lectins, concordantly positive reactions between primary pulmonary adenocarcinomas and their lymphatic and hematogenous
metastases
were seen in 67% or more cases, and with soybean agglutinin (SBA) and UEA-I the concordance rates between primary and lymphatic
metastases
(lymph nodes and contralateral lungs, respectively) were significantly higher (p less than 0.05) than those between primary and hematogenous
metastases
. With PE-10 immunohistochemistry, concordantly positive reactions between primary and
metastases
were low, especially in cases of distant hematogenous
metastases
(25%), but the statistical significance of differences was missed by narrow margins. With alcian blue PAS-stain, concordantly positive reactions of
mucin
production between primary adenocarcinomas and both lymphatic and hematogenous
metastases
were high (92%), but there was no evidence of correlation between lectin bindings and alcian blue-PAS reactions to either primary or metastatic lesions of pulmonary adenocarcinomas.

Immunohistochemical study of the expression of simple
mucin
-type carbohydrate antigens (Tn, sialosyl-Tn, T and sialosyl-T) was performed using specific monoclonal antibodies in the mucosa adjacent to gastric carcinomas histologically appearing unaffected (n = 58), and in primary gastric carcinomas (n = 87) and their
metastases
(329 lymph nodes and two liver metastases). Normal-looking mucosas: Tn antigen expression was identified in all the cases; sialosyl-Tn in eight cases; T antigen was never expressed and sialosyl-T antigen was observed in four cases; the expression of these antigens was distinctly limited to the cytoplasm, mostly in the supranuclear (Golgi) area. All the mucosas with intestinal metaplasia showed sialosyl-Tn expression in the goblet cells. Gastric carcinomas: 80 cases (91.9%) stained for Tn antigen, 69 cases (19.3%) expressed sialosyl-Tn antigen, 18 cases (20.7%) expressed T antigen and 17 cases (19.5%) stained for sialosyl-T antigen. In contrast to normal mucosa, carcinoma cells expressed simple
mucin
-type antigens both at the cytoplasm and at the cell membrane. Most primary carcinomas were concurrently stained for Tn and sialosyl-Tn antigens alone (41.1%), or together with T antigen or sialosyl-T antigen (28.7%). We found a close relationship between the expression of simple
mucin
-type carbohydrate antigens in the primary tumours and their respective
metastases
. T antigen (and sialosyl-T antigen) expression was correlated with the wall invasiveness of the tumours. The 18 tumours expressing T antigen and 16 out of the 17 tumours expressing sialosyl-T antigen had nodal
metastases
and/or sialosyl-Tn expression and the aggressiveness of the tumours (wall penetration, lymph node metastasis and venous invasion). No significant differences were observed between positive and negative tumours for Tn, sialosyl-Tn, T and sialosyl-T antigens regarding the morphologic at type, growth pattern, ploidy or lymphoid infiltrate of the primary tumours.

A 51 year old man with biopsy proven pulmonary sarcoidosis and skin test positive for tuberculosis presented with features of an amelanotic flat choroidal mass suggestive of choroiditis. The mass enlarged despite corticosteroids and anti-tuberculous medications. A thorough systemic evaluation for possible primary tumor metastatic to the choroid was negative. Further clinical evaluation and magnetic resonance imaging suggested a diffuse primary choroidal malignant melanoma with optic nerve invasion. The eye was enucleated and the mass proved histopathologically to be a
mucin
secreting adenocarcinoma of unknown origin despite a repeat systemic work-up. The patient died three months after the onset of symptoms and three weeks after enucleation with diffuse
metastases
from an unknown primary cancer. Magnetic resonance imaging (MRI) is usually helpful in the differentiation of uveal melanoma from uveal metastasis. In this case, however, it suggested the diagnosis of a diffuse choroidal melanoma. The reason for the atypical MRI findings will be discussed.

Mucoepidermoid carcinomas are tumors that usually arise from salivary glands and have a characteristic histologic pattern of atypical squamous cells showing focal
mucin
production. Mucoepidermoid carcinomas are uncommon neoplasms that
metastasize
most commonly via lymphatic and hematogenous channels. We report what we believe to be the first case of a mucoepidermoid carcinoma arising from a sublingual salivary gland with metastasis to a distant site on the skin. The patient is a 58-year-old black woman who was initially diagnosed with a high grade mucoepidermoid carcinoma of the salivary glands of the tongue. Approximately 18 months after presentation, and 6 months following surgery, chemotherapy, and radiation therapy, the patient noted a firm nodule on her flank. Biopsy showed malignant squamous epithelium. Periodic acid-Schiff and alcian blue stains revealed focal
mucin
production. The histologic differential diagnosis included an eccrine carcinoma,
mucin
-producing adenocarcinoma with squamous differentiation, a primary cutaneous adenosquamous (mucoepidermoid) carcinoma and a malignant mixed tumor of the skin. Clinical correlation was essential in making the correct diagnosis. While mucoepidermoid carcinomas only uncommonly show distant metastasis, and even less frequently involve the skin, this entity should be included in the differential diagnosis of
mucin
-producing neoplasms in the skin.

were present at the first operation. Initial symptoms were ignored in all cases and in only one was the serious nature of the condition realized at first presentation. In five lesions the histology was

of hepatoma to the brain is a rare event. Even rarer is massive hemorrhage of the brain associated with metastatic hepatoma. A 57-year-old man had cirrhosis of the liver with hepatocellular carcinoma. The tumor spread to the lungs and left occipital lobe of the brain. The primary and secondary neoplasms were negative in detection of
mucin
, but were immunohistochemically positive to cytokeratin CAM 5.2 and KC; the finding supported the hepatocellular origin of the tumor. The metastatic tumor formed papillae in the lung and produced massive hemorrhage in the left occipital lobe. This case raised the total number of intracranial metastatic hepatic carcinomas to 34 cases. Five of 34 hepatic carcinomas metastatic to brain, including the current one, were hepatocellular carcinoma that produced massive hemorrhage.

in bone, lung and peritoneum. Combined surgery, radiotherapy and chemotherapy was used but the patient died 2 years and 3 months after diagnosis.
