A 34-year-old man presented with classic glucagonoma syndrome manifested by weight loss, dermatitis, stomatitis, anemia, and mild diabetes mellitus. The diagnosis of glucagonoma was made by light and electron microscopic demonstration of a metastatic alpha cell carcinoma in a liver biopsy specimen. Plasma glucagon concentration was abnormally high. The patient also had symptoms and signs of involvement of the central nervous system. Radionuclide and CAT scans of the brain, negative CSF cytology and myelography excluded the possibility of metastases or other space-occupying lesions. Glucagon was demonstrated in the CSF. We postulate that the neurologic symptoms were due to direct or indirect effect of this hormone on the brain. Following therapy with streptozotocin and 5-fluorouracil, the patient had a subjective and objective clinical and hormonal remission of his disease including amelioration of his neurological impairment.

Proceeding from the current ideas on the significance of the catalase in the oxidative phosphorylation, oxygenation and desoxygenation of hemoglobin the activity of the above enzyme was studied in the blood of 60 patients with brain tumours. It was found that prior to a surgical intervention the catalase activity at the rate per 1 g of hemoglobin (the catalase index-CI) varied within the same range in patients with brain tumours of a different histological structure (meningiomas, adenomas of the pituitary, gliomas, metastases of cancer). The catalase index of the patients' blood does not differ from that in healthy individuals. In the post-operative period the catalase index rises right from the first days, paralleling the development of a general reaction to the damage. There exists a correlation between a fall in the level of hemoglobin and a rise of the catalase index, which apparently, has a compensatory significance. An investigation of the catalase in the clinical picture of the stress-syndrome may be of use for prognostic purposes.

Twenty-nine cases of single cerebral metastases from renal carcinoma were the object of a retrospective and prospective study covering a period of 15 years (1975-1988). The best diagnostic means were NMR imaging with paramagnetic contrast medium and CAT scans after intravenous injection of a double dose of contrast medium. All patients underwent total surgical removal of the cerebral lesion. Radiotherapy was useful but had less influence on further reproduction than in metastases from tumors of other sites. The median survival was 28.1 months in patients who received radiotherapy and 23 months in the others. No significant difference in survival was found between the group of patients with unknown primary tumors and the other group with diagnosed primary neoplastic disease.

CAT was carried out in 110 patients with gastric carcinoma and its findings compared with the intraoperative findings. CAT involved a polypositional scanning (depending on the tumor site) against the background of oral contrast label of the stomach and intestine. CAT potentialities in the diagnosis of gastric carcinoma, assessment of the tumor size and involvement of the adjacent organs and structures, detection of the metastases were studied. Use of CAT in complex with other methods will help choose the optimal treatment strategy and determine the volume of supposed surgical intervention.

Activities of superoxide dismutase (SOD) and catalase as well as content of malonyl dialdehyde (MDA) were estimated in the blood of patients with tumor of gastrointestinal tract. In the early stage of development of tumor SOD activity and concentration of MDA in whole blood was decreased while catalase activity was increased significantly. In the cases of metastases spreading and cachexia both SOD and catalase activities were greatly decreased; the content of MDA was increased.

Between January, 1990 and May, 1991, we administered LAK immunotherapy using the intralymphatic route to 25 patients with metastatic cancer resistant to conventional therapies. In the preparation of the immunotherapy, we followed the technique described by Pizza G. et al. The age of our patients ranged between 50 and 75 years and their Karnofsky's indexes were above 70%. The histological type of the metastasis were determined by Rx, ECO and/or CAT before and after the administration of the immunotherapy. In the intralymphatic administration, we followed the technique described by Pizza G. et al. The immunological therapy was administered on days 1, 21, 90 and 111 and the clinical response was assessed by RC, RP, EE and F. The immunological behaviour of the host was assessed through the determination of lymphoid populations (CD2, CD4, CD5 and CD8) and cytolytic cells were studied with monoclonal antibodies CD and CD16. Such immunological study was carried out before the administration of each immunotherapy series. In 7 out of 25 patients (28%), we were able to administer the four LAK series. Such patients were subsequently studied, observing that, although tumoral lesions did not increase in size, they did not disappear and, thus, they were classified as clinically stable. Clinical and analytical toxicity was null. The immunological study did not show any statistically significant changes and the activity of cytotoxic cells (NK) was not modified.

PMID:[Intralymphatic administration of adoptive (LAK) immunotherapy in the treatment of patients with metastatic cancer resistant to conventional therapies]. 150 2

Clinical and experimental observations suggest that tumor-induced endothelial cell injury may be one of several initial events in the establishment of tumor metastases. To test this hypothesis, the authors have analyzed the interaction of malignant melanoma (ST-ML-12) multicenter tumor spheroids with endothelial cell monolayers in a three-dimensional coculture system. After 1.5 hours of interaction, the authors observed a toxic effect on endothelial cells in the perispheroid region. The latter was demonstrated by testing membrane integrity with the fluorescent probes acridine orange/ethidium bromide and resulted in sensitivity to shear stress of the damaged cells. The endothelium then underwent a regenerative cycle to replace the denuded halo. Addition of the oxygen radical-scavenging enzyme superoxide dismutase to the culture medium prevented this endothelial cell damage in a dose-dependent manner for up to 12 hours. By contrast, catalase, deferoxamine mesylate, allopurinol, and the proteinase inhibitors soybean trypsin inhibitor and aprotinin were not protective under the same conditions. The endothelial damage was dependent on the attachment of the spheroids. Medium conditioned by ST-ML-12-spheroids proved to be ineffective. A similar, but less prominent, deleterious effect was seen when human peritoneal mesothelial cells were used in place of the human umbilical vein endothelial cells. Spheroids of the uroepithelial cell line HU-609 were used as control. No toxicity was observed in these cocultures. Melanin biosynthesis is associated with the production of oxygen-derived free radicals. The results suggest a possible implication of these free radicals in metastasis formation of malignant melanoma.

The Authors report their experience about surgical treatment of metastases from renal cell carcinoma. From January 1983 to December 1989, twenty eight patients, 19 males and 9 females, with a median age of 58 years (range 42-79), have been submitted to contemporary or subsequent metastasectomy. Metastases were synchronous in 10 cases, while they appeared after a free disease mean-time of 28 months after nephrectomy in 18 patients. Recovery period has always been normal and all the patients were controlled, every six months, with routine blood and urine examinations, Chest X-ray, abdominal CAT and bone scan. Among the 10 patients with synchronous metastases 5 died, 3 are in progression and 2 are NED after a mean-time follow up of 36 months. Among 18 patients who underwent surgery for metachronous metastases, 2 died, 4 are in progression and 12 NED (mean follow up of 36 months). In conclusion, while the presence of synchronous metastases is an unfavourable prognostic factor even after their surgical removal (8 out of 10 patients died or are in progression shortly after metastasectomy), results after metachronous metastases surgery are encouraging, but the real efficiency of this treatment is still to be confirmed.

Forty-nine young males with advanced testicular cancer treated between March 1982 and June 1989 comprised a 49/588 (8%) subgroup with distant metastases to the retroperitoneum, mediastinum and lungs, and required mandatory surgery on basis of the risk for reactivation of "slumbering" malignant components in tumor tissue temporarily inactivated by chemotherapy and/or radiotherapy. Preoperative CAT-scan was carried out with the intention of mapping regional pathology related to the size, number, burden of the tumor tissue, and the occasional and prognostic ominous invasion of the great vessels. Regarding radical surgery, the positive predictive value of CAT-scan was found to be 33/34 (97%). The negative predictive value was 5/15 (33%) and was interpreted as an expression of the radiologist's cautious assessment. 14/49 (29%) died before March 1990. Poor prognosis was related to invasion of major vessels and was found in 14 patients of whom eight died. It seems established that CAT-scan presents an extremely valuable preoperative investigation when it comes to planning of surgical strategy in a patient population with testicular cancer metastases with difficult access.

Metastasis of colorectal cancer to the lung is a frequent occurrence. Resection of pulmonary metastasis provides the only effective treatment. The initial detection of disease is usually with a chest radiograph. A CAT scan determines the number of lesions, resectability and other metastatic disease. Candidates for surgical excision are those patients with completely resectable singular, multiple, and bilateral metastasis, and those with metachronous liver lesions excised for cure. Exposure of unilateral disease is via a thoracotomy and of bilateral disease via a sternotomy. Metastatic lesions are removed by wedge resection with conservation of lung tissue. Postoperative recovery is usually rapid with low morbidity and mortality. Long-term success for these resections remains controversial.
