Three cases of a distinctive soft tissue fibrous tumor with spotty calcification demonstrate the lesion originally described by Keasby as Juvenile Aponeurotic Fibroma. The tumor tends to occur on the volar surface of the hands and feet in children and adolescents but may be seen in other areas in all age groups. Size rarely exceeds 3.0 cm. The tumors are fixed to the fascial layers. In previously reported patients pain was not described as an important factor, but both pain and tenderness were striking in all 3 of our cases. Microscopically the dominant feature is a fribrous stromal pattern of fusiform cells with inclusions of mast cells.
Spotty
calcification is a constant finding and appears to increase with duration of tumor. Local recurrence after excision and an aggressive histologic appearance may raise the question of malignancy. However, neither destruction of normal tissue surrounding the tumor nor
metastases
have occurred. Local exicision is the treatment of choice.

Histological studies were conducted into 352 primary malignant melanomas of the skin together with 4,003 associated and surgically removed lymph nodes at the Institute of Pathology and Bacteriology of Kaiserin-Elisabeth-Spital, between 1982 and 1987. Particular attention was given to micrometastases of regional lymph nodes. 27 lymph node micrometastases were recorded from 25 patients.
Spots
of melanin were also found in lymphatic tissue, but no tumour cells were detectable. There is no clinical aid by which micrometastases of lymph nodes might be possibly diagnosed, and, on principle, they are greatly accidental findings even for the pathohistologist. However, undetected lymph node micrometastases may have a deteriorating impact upon the prognosis of malignant melanoma. Complete microscopic elucidation of surgically removed lymph nodes, there fore, is absolutely essential. Capillaries are present in abundance deep inside lymph nodes, and from these points of departure tumour cells may enter into the blood flow and cause haematogenic
metastases
. That is why the prognostic importance attributed to histologically verified micrometastases is just as great as that attached to macrometastases.

Kidney
metastases
from thyroid cancer are rare. We report two such patients and demonstrate that the in vivo 131I uptake by the kidney metastasis is associated with high levels of sodium iodide (Na+/I-) symporter (NIS) expression in the first case. Case 1: A 61-year-old woman with papillary thyroid carcinoma-follicular variant (PTC-FV) presented with scapular metastasis. After thyroidectomy and scapulectomy, a 131I posttherapy scan showed left upper quadrant uptake. A 3.0-cm metastatic PTC-FV deposit was removed by partial nephrectomy. Case 2: A 53-year-old woman presented with back pain. A computed tomography (CT) scan showed a 3.5-cm renal mass, a multinodular goiter, and lung metastases thought secondary to a renal cell carcinoma. A unilateral nephrectomy revealed metastatic PTC-FV. After thyroidectomy, a 131I posttherapy scan showed lung and skeletal
metastases
. NIS immunoreactivity in tumoral tissue was strongly positive in the primary tumor, shoulder, and kidney metastasis in case 1, as well as in the primary tumor in case 2.
Spotty
, low-level NIS expression was observed in the kidney metastasis in case 2. In conclusion, kidney
metastases
of PTC-FV may occasionally retain adequate levels of NIS expression, enabling their detection during life. Thus, intense uptake in the abdomen during 131I imaging should not be assumed to be physiological gastrointestinal tract residual radionuclide activity.
