A study on the advantages offered by a "prophylactic" cranial irradiation in cases of localized small cell cancer of the lung has been conducted at the Center since April of 1981. The report deals with preliminary results on tolerance and immediate reactions of the brain obtained on the basis of 25 cases. A good tolerance of doses ranging 25-30 Gy was recorded. Acute reactions followed gamma therapy in 32% which, however, did not prevent the completion of the full course of prophylactic irradiation.
Neurologic symptoms
developed after cranial irradiation in 3 cases; this was interpreted as a possible post-radiation encephalopathy.
Metastasis
was found in one case. The results suggest that the study be continued.

Thirty-six patients with primary Ewing's sarcoma of the spine were diagnosed at the Mayo Clinic between 1951 and 1988. The mean age was 17 years (range, 5-40 years).
Neurologic symptoms
and signs were seen in 58% of the patients. Forty-seven percent of all patients had an open biopsy of the lesion and underwent a decompressive laminectomy. Three of the four patients with thoracic or thoracolumbar involvement had progressive kyphosis after laminectomy. All patients received radiation therapy in various dosages. Sixteen of the patients were registered in the Intergroup Ewing's Sarcoma Study. Intensive combination chemotherapy was administered to 32 of the patients. Nine patients were free of disease at the final follow-up examination (follow-up ranged from 6 to 184 months). The 5-year survival rate was 33%. The mean survival time was 2.9 years. No significant correlation was found between the location of the tumor in the spine and the length of disease-free survival, overall survival, or incidence of
metastatic disease
. Patients enrolled in the Intergroup Ewing's Sarcoma Study had significantly better rates of disease-free survival and overall survival.

A 78-year-old man with liver cirrhosis was found to have multiple hepatocellular carcinomas (HCCs) and underwent 3 sessions of transcatheter arterial chemoembolization. Fourteen months after diagnosis, the patient presented with left hemiparesis. Contrast-enhanced magnetic resonance imaging showed multiple
metastases
with ring-shaped enhancement in the cerebrum and cerebellum. There were no
metastases
to other organs. The metastatic lesions almost completely disappeared after whole-brain radiotherapy with a total dose of 50 Gy.
Neurologic symptoms
decreased, and the patient's quality of life improved. The patient underwent 2 more sessions of transcatheter arterial chemoembolization. Twelve months after the diagnosis of brain metastasis, the patient remains alive. The present case indicates that radiotherapy can improve quality of life and prolong survival in some patients with brain metastases from HCCs.
