Metastases
of the tumor in the abdominal and thoracic cavity were confirmed by cytology. The authors treated the metastasis with a combination of intravenous administration of 20 mg/m2/day cisplatin and 60 mg/m2/day etoposide for two 5-day cycles. After chemotherapy, the patient recovered fully with time and follow-up radiological studies demonstrated no evidence of tumor recurrence. Combination chemotherapy with cisplatin and etoposide is to be considered effective for extraneural metastasis of medulloblastoma.

Interleukin-2 (IL-2) mediates the regression of
metastatic cancer
, but clinical application has been limited by the induction of dose-dependent toxicities in normal tissues. The most clinically significant toxicities occur secondary to a vascular leak syndrome and include acute respiratory failure and hemodynamic instability. Because previous studies suggested a role for pentoxifylline in attenuating the toxic effects of IL-2, we hypothesized that pentoxifylline would inhibit alterations in the microvasculature induced by IL-2 and would ultimately reduce IL-2-induced toxicity. To determine the validity of this hypothesis, we prepared four groups of rats for in vivo microvascular observation. In the first group, a bolus intravenous injection of IL-2 (1 x 10(6) units/kg) acutely induced hypotension,

No significant changes occurred in animals treated by pentoxifylline alone or the control IL-2 vehicle-alone group. We report the case of a 4-month-old infant with terminal malignancy who had systemic
metastases
and a localized metastasis to the dorsal midbrain periaqueductal gray (PAG). Extraordinary doses of opioids (dose equivalent of 2680 mg morphine sulfate/h, i.v.) were required to achieve adequate analgesia. The behavior of the infant, interpreted as being representative of a response to pain, may have been an aversive reaction due to the location of the lesion in the dorsal PAG. We propose that the lesion in the PAG impaired the responsiveness of this infant to the effect of opioids. This report is to alert clinicians to the possible role of the PAG in impaired opioid responsiveness in patients with terminal malignancy, as well as the possibility that pain-like signs (e.g., tachycardia,

The case describes a 33-year-old male patient who complained of chest pain, palpitations, mild dyspnoea, and weight loss. Blood analysis reveals high levels of human chorionic gonadotropin (833818 mlU/ml), T3 (16.90 pg/ml), and T4 (7.77 ng/dl), as well as a fall of TSH (0.01 ulU/ml). A 10 yr old papillon was evaluated for lethargy, inappetence,

, and labored breathing. Physical examination findings included dehydration,

, tachycardia, and muffled heart sounds. Thoracic radiographs revealed an enlarged cardiac silhouette. Echocardiography revealed a mild volume of pericardial effusion and no evidence of right atrial tamponade or a cardiac-associated mass. Cytological analysis of the pericardial effusion was consistent with blood. There was no evidence of neoplasia. Coagulation parameters were within normal limits. Initial treatment consisted of pericardiocentesis. A subtotal pericardiectomy was later performed. No surgical complications were noted, and the dog was discharged 2 days following surgery. Results of the histological examination of the pericardium revealed hemangiosarcoma. Treatment with doxorubicin was initiated 12 days after surgery, at which time, pulmonary metastasis was suspected. The dog survived for 18 days after subtotal pericardiectomy, when it was euthanized because of complications presumably related to pulmonary
metastatic disease
. This is the first published report of primary pericardial hemangiosarcoma. The dog had a short survival time; thus, the response to subtotal pericardiectomy combined with adjuvant doxorubicin treatment could not be determined. Pulmonary
