Of 129 patients with small cell lung cancer (SCLC) who underwent bone marrow examination for staging, 39 (30%) had bone marrow involvement. Only three of 129 patients (2.3%) had bone marrow involvement as the only site of metastatic disease. When patients with bone marrow metastasis were compared with patients whose bone marrow was normal, there were significant differences in serum levels of lactate dehydrogenase (LDH), glutamic oxalacetic transaminase (SGOT), glutamic pyruvic transaminase (SGPT), alkaline phosphatase (AP), albumin, and sodium (Na). We found no clinically significant difference in survival between patients with extensive disease with or without bone marrow involvement. Serum Na, albumin, SGOT, and uric acid were important prognostic determinants of survival. Based on the results of this study, we do not recommend routine bone marrow examinations in the staging of SCLC.

Routinely processed biopsy material, including 56 gliomas of varying malignancy, 10 meningiomas, 10 brain metastases and 12 brain abscesses, was examined for the presence and distribution of IgG, IgA, IgM, IgD and albumin using the unlabeled antibody peroxidase-antiperoxidase technique. In all specimens the deposition of stained immunoglobulins (Ig) was strictly associated with that of albumin even on cell surfaces. Thus there was no evidence for specific membrane binding or cytotoxicity. The interstitial proteins demonstrated are most likely derived from the plasma by blood-brain barrier breakdown which occurs in nearly all tumors and abscesses. Obvious intracellular staining for Ig and albumin was seen in glioma cells and astrocytes only. This is suggested to be due to active protein uptake as a specific feature of astrocyte differentiation which decreases with malignancy and is lost in glioblastomas. Evidence for local Ig production was found in 8 out of 10 metastases with striking IgG- and IgA-positive plasma cells within lymphocytic infiltrations and in one meningioma showing conspicuous plasma cells components. No glioma contained Ig-bearing plasma cells, though round cell infiltrations were present in 64% of the unselected cases. The significance of these findings regarding the immunological situation in brain tumors is briefly discussed.

The standard method for the evaluation of hepatic perfusion during hepatic artery infusion (HAI) chemotherapy is planar hepatic artery perfusion scintigraphy (HAPS). Planar HAPS was performed with 2 mCi of [99mTc] macroaggregated albumin infused at 1 ml/min and compared with single photon emission CT (SPECT) HAPS and with a new study, CT performed during the slow injection of contrast material through the HAI catheter (HAI-CT). Thirteen patients underwent 16 HAI-CT studies, 14 planar HAPS studies, and 9 SPECT HAPS studies. In 13 of 14 studies (93%) HAI-CT and planar HAPS were in complete agreement as to the perfusion pattern of intrahepatic metastases and normal liver. In nine studies where all modalities were performed, the findings identified by HAI-CT and planar HAPS agreed in all cases, whereas the results of two SPECT scans disagreed with the other studies. With respect to perfusion of individual metastases, 14 of 14 HAI-CT studies, 12 of 13 planar HAPS studies, and 9 of 9 SPECT HAPS studies correctly demonstrated the perfusion status of individual lesions as indicated by the pattern of changes in tumor size determined on CT obtained before and after the perfusion studies. Hepatic artery infusion CT was superior for delineation of individual metastases, particularly small lesions, and for the evaluation of nonperfused portions of the liver. Planar HAPS detected extrahepatic perfusion in four patients, and this was not detected by HAI-CT. We conclude that HAI-CT and scintigraphy are complementary techniques. Hepatic artery infusion CT has advantages for the evaluation of intrahepatic perfusion, and planar HAPS is superior to HAI-CT for the detection of extrahepatic perfusion.

The different biokinetic behaviour of radioactive tracers which can be monitored by nuclear medicine procedures may be helpful in the diagnosis or differential diagnosis of tumours or tumour-like lesions of the liver or pancreas: autologous red blood cells in the detection of haemangiomas; iminodiacetates in delineation of focal nodular hyperplasia, resp. liver adenomas versus other tumours; colloidal albumin in differentiating focal fatty infiltration from diffuse infiltration by metastases; monoclonal antibodies in revealing liver metastases or abscesses, resp. pancreas neoplasms. In general the clinical suspected diagnosis and the result of morphologic imaging modalities determine the use of the appropriate radioactive tracers.

The administration of interleukin 2 (IL-2) to mice and humans is limited by the induction of a dose-dependent increase in vascular permeability causing a vascular leak syndrome (VLS). We have investigated the impact of the injection of recombinant interleukin 1 alpha (IL-1 alpha) on the VLS induced by IL-2 by measuring the extravasation of 125I-albumin into tissues and by assessing wet and dry lung weights. IL-1 alpha alone did not induce any significant extravasation of radiolabeled albumin. IL-2 alone, however, caused a significant increase in the extravasation compared to control lungs. IL-1 alpha injection along with IL-2 significantly reduced the IL-2-induced extravasation of radiolabeled albumin [9,886 +/- 533 (SEM) cpm were observed in IL-2 and IL-1 alpha-treated lungs compared to 14,172 +/- 2,628 cpm in lungs treated with IL-2 alone (P less than 0.02)]. IFN-alpha in combination with IL-2 produced more severe vascular leakage than caused by IL-2 alone. IL-1 alpha also significantly decreased (P less than 0.05) the vascular permeability induced by the combination of IFN-alpha and IL-2. We observed 44,811 +/- 13,131 cpm in IFN-alpha- and IL-2-treated lungs compared to 18,350 +/- 2,622 cpm in IFN-alpha-, IL-2-, and IL-1 alpha-treated lungs. The IL-2- and IFN-alpha-induced increase in lung water weight was also reduced significantly by the addition of IL-1 alpha. The decrease in vascular leakage was dependent on the dose and timing of IL-1 alpha administered. When recombinant IL-1 alpha was given as a single i.p. injection, 24 h before the injection of IL-2 (or Hanks' balanced salt solution) or IL-2 and IFN-alpha no abrogation of the VLS was observed. Although IL-1 alpha decreased VLS significantly in mice treated with IFN-alpha and IL-2 the survival of mice was not improved by the simultaneous administration of IL-1 alpha. Histologically, treatment with IFN-alpha and IL-2 produced marked perivascular and intraalveolar edema which was completely eliminated by the addition of IL-1 alpha. However, some perivascular edema in IL-1 alpha-treated mice remained which was equivalent to that caused by IL-2 alone. PMID:Decrease in interleukin 2-induced vascular leakage in the lungs of mice by administration of recombinant interleukin 1 alpha in vivo. 278 61

A monoclonal mouse antibody (MoHG) was produced using in vitro cultured AH66R tumor cells treated with cholesteryl hemisuccinate as an immunogen. The antibody identified a 90 kd membrane glycoprotein (HG-90) which is expressed on in vitro cultured hepatoma cell lines AH66 and AH66R. A monoclonal antibody was prepared to the anthracycline drug daunomycin, and it also reacted with adriamycin. A fusion was made of the hybridoma HG-90 with the hybridoma which recognized daunomycin/adriamycin. This bispecific hybridoma A8C recognized both determinants. We studied the therapeutic effect of the A8C bispecific antibody with adriamycin treatment and compared it to the effect of the bispecific antibody to which adriamycin had been conjugated via an albumin (Alb) bridge. The therapy model used was the tumor AH66R in Donryu rats. Tumor bearing rats had their subcutaneous tumors resected on day 10, a time when distant metastases were present. After the surgical resection of the tumor the rats were injected intravenously for two cycles with the bispecific antibodies, followed by the administration of adriamycin (ADR) or MoHG.Alb.ADR conjugates. A slight therapeutic effect occurred with either MoHG or ADR alone but treatment with the bispecific antibody followed by the administration of ADR or with the MoHG.Alb.ADR conjugates significantly prolonged survival, with 60% of the treated animals being "tumor free" when sacrificed on day 80. Lower serum concentrations of alphafetoprotein were observed with the bispecific antibody and drug treatment. This suggests that the bispecific antibody/drug treatment is potentially more beneficial in the suppression of distant metastases than the MoHG.Alb.ADR conjugate. This may be due to an increase in the local drug concentration of unmodified adriamycin.

The ascitic fluid concentrations of cholesterol and fibronectin and the serum-ascites albumin difference were compared with two conventional tests of ascitic fluid, total protein and LDH, in their diagnostic ability for detection of malignancy in ascitic samples from 69 patients with ascites: 54 with ascites due to liver disease and 15 whose ascites was caused by peritoneal metastases. Sixteen cirrhotic patients with superimposed hepatocellular carcinoma in whom ascites was of uncertain etiology were considered separately. The mean ascitic fluid total protein, LDH, cholesterol, and fibronectin values in the peritoneal metastases group were 3.70 +/- 1.20 g/dl, 247.26 +/- 148.14 units/liter, 109.06 +/- 29.85 mg/dl, and 91.57 +/- 41.52 micrograms/ml, respectively, and all were significantly higher than the corresponding values in the liver disease group (P less than 0.001), which were 1.37 +/- 0.59 g/dl, 75.40 +/- 110.70 units/liter, 23.75 +/- 11.22 mg/dl, and 31.86 +/- 10.51 micrograms/ml, respectively. Mean serum-ascites albumin difference in the peritoneal metastases group was 0.62 +/- 0.38 g/dl, which was significantly different from the corresponding value in the liver disease group (1.92 +/- 0.41 g/dl, P less than 0.001). Both ascitic cholesterol above 46 mg/dl and an ascitic fibronectin concentration greater than 50 micrograms/ml had high diagnostic accuracy (97%) for malignancy, being higher than that achieved using a serum-ascites albumin difference under 1.1 g/dl and an ascitic total protein above 2.5 g/dl, which had accuracies of 94% and 93%, respectively. Ascitic fluid LDH was the least reliable test. No differences in the ascitic fluid analysis were found between cirrhotic patients with and without hepatocellular carcinoma.(ABSTRACT TRUNCATED AT 250 WORDS)

A series of polyomavirus-transformed rat cells with varying tumorigenic potential were tested for biophysical parameters possibly related to metastatic properties: adhesive capacity and strength of adhesion to different substrates (laminin, fibronectin and albumin), cell deformability and spreading. Two groups of cell lines were defined according to their higher or lower adhesive capacity. Adhesivity did not appear to be related to cell deformability and spreading. A weak correlation was suggested between low adhesivity and high metastatic potential. A selection method was devised to separate cell samples into 3 subpopulations with different adhesive strength. Two cell lines, originally different, were chosen for this study: Py-tsa A25 cells were less adherent and highly metastatic, and Py-WTA2 cells were more adherent and less metastatic. After s.c. inoculation into syngeneic Fisher rats, the 3 selected subpopulations of the 2 cell lines induced pulmonary nodules to varying degrees, but only the less adherent ones were able to induce visceral metastasis located in stomach and intestine. In this case, animal survival time was 30% lower than for the highly adherent selected cells. After 10 culture passages, the same subpopulations were able to metastasize only in the lungs. However, when the selection procedure was repeated, the less adherent cells were again able to yield visceral nodules. Tumorigenicity remained unchanged in all cases. Study of cell dissemination and arrest in vivo showed a rapid targeting of labelled tumor cells toward lungs and stomach 5 hr after intradermal injection, where they remained up to 72 hr. More adherent cells displayed delayed localization after injection (24 hr) and radioactivity decreased more rapidly.

PMID:Relationship between cellular adhesiveness and metastatic activity in polyomavirus-transformed FR3T3 rat cell lines. 284 89

Until now, there has been no reliable means of predicting tumor response to chemotherapy in patients with metastatic colorectal cancer. Using arterial nuclide flow scans as a determinant of tumor response, the degree of tumor perfusion was evaluated in a blinded prospective study. Seventy-three patients with colorectal hepatic metastases received continuous hepatic arterial (N = 52) or systemic intravenous (N = 21) chemotherapy using an implantable pump. All patients had pretreatment hepatic arteriography and arterial flow scans using 99mTc macroaggregated albumin (99mTc-MAA). An arteriogram was characterized as positive if it showed tumor hypervascularity; the 99mTc-MAA flow scan was considered positive if it showed increased tumor uptake relative to the liver. Of 47 patients with an evaluable 99mTc-MAA flow scan who were treated with arterial infusion, 31 had a positive scan; in this group 16 responded to chemotherapy. The 99mTc-MAA scan was negative in 16 patients, of whom one responded to chemotherapy (p less than 0.006). The 99mTc-MAA scan had the greatest predictive value in previously untreated patients (sensitivity = 91%; specificity = 77%). The arteriogram was positive in 25 of 46 evaluable patients, but this finding had little predictive value for tumor response (sensitivity = 56%; specificity = 46%). Of 21 patients receiving systemic intravenous infusion, the scan was positive in nine patients, of whom seven responded to chemotherapy. The 99mTc-MAA scan was negative in 12 patients, of whom one responded to chemotherapy (sensitivity = 88%; specificity = 85%). When 99mTc-MAA-positive and -negative groups were compared, there were no differences in mean patient age, per cent liver involvement, tumor size, or plasma liver function tests. Hepatic tumor perfusion as determined by MAA arterial flow scan is a reliable predictor of tumor response in patients with metastases from large bowel cancer. The test provides a valuable criterion for selecting individuals for treatment of metastases from large bowel cancer by infusion chemotherapy.

Tumor response rates after hepatic regional arterial chemotherapy infusion vary from 30% to 83% with little explanation for this variability. Drug clearance by the liver and plasma drug kinetics after arterial infusion have been described, but little is known about actual tumor drug uptake. This study measured fluorodeoxyuridine (FUdR) uptake in colorectal tumors metastatic to the liver and correlated these results with radionuclide flow scans and with tumor response to treatment. In 16 patients with unresectable colorectal hepatic metastases, FUdR (1 microCi/kg) and 99mTc-macroaggregated albumin (MAA) (6 mCi) were injected at a constant rate into the hepatic artery intraoperatively after insertion of a hepatic artery catheter. Liver and tumor biopsy specimens were obtained 2 and 5 minutes after infusion. 3H counts (representing drug uptake) and 99mTc disintegrations (representing blood flow) were measured by scintillation and gamma-counting. The tumor/liver ratios of FUdR and MAA were linearly related (r = 0.73, p less than 0.001). The mean tumor FUdR level was 9.2 +/- 8.9 nmol/gm, and the mean liver FUdR level was 24.5 +/- 16.8 nmol/gm. The mean FUdR tumor/liver ratio was 0.43 +/- 0.36. The extraction of FUdR by tumor was 49%. Thus substantially more drug was taken up by the liver than by the tumor after arterial infusion. There was considerable heterogeneity in FUdR uptake and MAA retention within both tissues. Tumor uptake of drug correlated significantly with MAA retention; higher FUdR levels were seen in tumors that appeared "hot" on radionuclide arterial perfusion scans. Tumor drug uptake was independent of lesion size and percentage of liver involvement.
