The level of serum TPA was determined by radio-immunoassay in 19 healthy subjects and 90 patients with urogenital cancer. The normal level of serum TPA was 86 +/- 24 U/l, and the level of more than 134 U/l was determined positive. The positive rate of TPA was 38.9% in 90 patients, while that of CEA was 25.6%. In 19 patients with bladder tumor and 7 with testicular tumor, the positive rates of TPA were 52.6% and 71.4%, respectively, and the level of serum TPA was high in these positive patients. Considering the low positive rate of CEA, TPA may be a more useful marker than CEA in patients with bladder tumor and testicular tumor. Serial determinations of serum TPA and CEA showed the considerable variation of serum TPA compared with serum CEA and a temporary elevation of serum TPA following radical nephrectomy and retroperitoneal lymphadenectomy. However, the level of serum TPA fell significantly after the successful treatment in 8 patients (2 with renal cell cancer, 3 with bladder tumor, 1 with prostate cancer, 2 with testicular tumor) and rose sharply with recurrent or
metastatic disease
in 4 patients (2 with bladder tumor, 2 with testicular tumor). Although there was no correlation between the levels of serum TPA and serum PAP, the level of serum TPA tended to change in parallel with the level of serum AFP or

Primary malignant germ cell tumors of the mediastinum are extremely rare among females. This report describes such a tumor arising in the mediastinum of a 26-year-old woman and correlates immunohistochemical studies of the neoplasm with serum tumor markers. Microscopic examination of the tumor revealed an embryonal carcinoma with choriocarcinoma, the latter consisting of sheets of polygonal cells intimately related to multinucleate giant cells that contained beta-human chorionic gonadotropin (beta-HCG) when stained immunohistochemically. level at the time of diagnosis was 8,300 mU/ml and remained elevated throughout the patient's course. Alpha-fetoprotein was not detected in serum or tissue. Postoperative chemotherapy failed to control the growth of the tumor, and the patient died as a result of massive intrathoracic tumor involvement. At autopsy,
metastases
were found in two periaortic lymph nodes and in the liver. There was no gross or microscopic evidence of ovarian disease. This case illustrates the typical behavior of malignant germ cell tumors of the mediastinum, rare among females and infrequently described in the literature. To our knowledge, this is the first detailed immunohistochemical study of a mediastinal germ cell tumor in a female.

Observation of 24 gestational trophoblastic tumors, i.e. 20 hydatidiform moles, 1 invasive hydatidiform mole, 1 non-metastatic and 2 metastatic choriocarcinomas, is reported. The diagnoses were based on case record data, clinical findings, echography and radioimmunoassay of human gonadotropin (
HCG
). The therapy was oriented towards maintaining fertility in reproductive women. For treatment with cytostatic drugs the patients were allocated to a low risk and a high risk group according to history, diagnosis and former therapy, and an appropriate mono- or combined therapy was started. 17 of 24 patients were cured by surgery in an initial therapy phase. 7 tumors were progressive, 6 of which responded to secondary surgical or cytostatic treatment. One patient died from
metastases
of choriocarcinoma. The total cure rate of 96% was rendered possible by regular follow-up including bimanual palpation, echography,

-assay and chest X-rays.

Alfa-fetoprotein (AFP) is an indispensable examination in the management of non-seminomatous germ cell tumor. However, many liver diseases also frequently show the elevation of AFP. Therefore, it is essential to discriminate between yolk sac-derived component and liver-derived one. A 32-year-old male who had suffered from chronic hepatitis, visited our clinic in December, 1991. He complained of left scrotal atrophy and dull pain. Surgical specimen was histologically diagnosed as embryonal carcinoma with syncytiotrophoblastic giant cell. The levels of tumor markers, such as AFP, beta-human chorionic gonadotropin (beta-HCG), were estimated. Both of them were elevated, and radiographical studies demonstrated metastatic lesions of bilateral lung field and retroperitoneal lymph nodes (RPLN). After three courses of cisplatin based chemotherapy, lung and RPLN
metastases
diminished and serum beta-
HCG
had normalized. However, the serum AFP persisted to show an abnormally high concentration. The subfraction profile with lens culinaris hemagglutinin (LCA) was estimated. The one after the first course was compared with the one after third course. The latter one showed complete diminution of peak 2. This implied the diminishment of yolk sac element.

The cardinal syndrome of a testicular germ cell tumour is typically scrotal enlargement. The present paper compares the group of patients with typical scrotal presentation and those who present with atypical symptoms caused by
metastases
. Among 284 retrospectively studied patients, 34 (12%) presented with extrascrotal symptoms. The most important were abdominal pain (n = 16) and pulmonary symptoms (n = 10). The group of patients with extrascrotal symptoms was characterized by the following parameters: percentage of pure seminoma in 35% (versus 56% in the patients with typical presentation), elevation of alpha-feto-protein in 47% (versus 27%), and elevation of beta-
HCG
in 61% (versus 29%). The outcome was lethal in 35% of the patients with atypical presentation, as opposed to 6% of those with typical presentation. In 22 patients with extrascrotal presenting signs a palpable testicular mass was found on clinical examination. Occult testicular tumour proved to be present in 9 patients, and burned-out tumours in 3. Unawareness of testicular cancer is a significant factor in diagnostic delay. Scrotal palpation should be part of every clinical examination in younger male patients with cancer from an unknown primary.
