A study was made on the prognosis of 26 tracheal adenoid cystic carcinoma surgery cases reported in Japan between January 1978 and January 1984. The study was based on a questionnaire sent to doctors who reported prognosis as of May 1991. Twelve cases out of the 26 cases underwent postoperative irradiation. Seven out of the 12 cases survived 9 to 13 years after surgery without any recurrence. Two out of the 12 cases survived 8 and 10 years after surgery with multiple pulmonary metastases. Three out of the 12 cases died of a local recurrence 3, 3 and 9 years after surgery. Four cases received irradiation after a local recurrence was revealed postoperatively. Three out of the 4 cases survived 9, 10 and 11 years after surgery, but 1 out of the cases died 6 years after surgery. Four cases received no irradiation not only after surgery but also after detection of a local recurrence. All of them died within 9 years after surgery. There were six cases who underwent no postoperative irradiation and showed no local recurrence. One out of the 6 cases died of pancreas cancer 7 years after surgery, but the other cases survived 8 to 11 years after surgery without any recurrence. A study was also made on the prognosis of 9 cases with adenoid cystic carcinoma of the trachea who underwent surgery at Keio University and Tokai University until April 1982 (4 cases overlap with the above-mentioned cases). A 33-year-old woman with adenoid cystic carcinoma of the trachea was treated by resection of the upper and mid-trachea, plus total laryngectomy, subtotal thyroidectomy, partial resection of the cervical esophageal muscle layer and mediastinal tracheostomy. This tumor invaded directly into the thyroid gland, the larynx, the recurrent laryngeal nerve and the cervical esophagus. Three years after the operation, metastases developed in the right lung (S10, S2) and partial resection of the lung was performed. This patient is now free from disease. Aggressive surgical resection is important for the improvement of survival in this condition, although this tumor shows low grade malignancy and is slowly growing.

Nine patients with an adenoid-cystic carcinoma of the trachea are describes. It is a rare tumour which grows relatively slowly; it has a tendency for early invasion of neighbouring tissues, but late distant metastases. A correct diagnosis can be made by the radiologist. The trachea should be visible on every film of the thorax and should be inspected. In particular, a diagnosis of therapy-resistant asthma should direct the attention of the radiologist to the trachea.

A rare small-cell carcinoma of the trachea was observed in a 27 year-old man. Treatment included chemotherapy and radiotherapy as for bronchogenic small cell carcinomas. This treatment regimen led to complete but temporary remission. The patient died 4 months after the end of the treatment due to multiple cerebral metastases.

A case report of adenoid cystic carcinoma of the trachea is presented. A high index of suspicion is required to make the diagnosis of tracheal tumours early since the patients tend to have normal chest radiographs which on closer examination may show an abnormality of the tracheal column. Adenoid cystic carcinoma is the most common malignant tumour of the trachea. Locoregional control of this disease is achieved by a combined modality therapy of surgery and postoperative radiotherapy. Despite this, it still has a prolonged clinical course and the tendency for delayed onset of distant metastases.

Basaloid squamous cell carcinoma is an uncommon variant of squamous cell carcinoma of the trachea. We describe the case of an unresectable basaloid squamous cell carcinoma of the trachea treated with concurrent chemoradiotherapy up to a dose of 60 Gy in 33 fractions with weekly paclitaxel and carboplatin. The pathological recognition of basaloid squamous cell carcinoma and its distinction from adenoid cystic carcinoma of the trachea is important for its management. Combining systemic chemotherapy with locoregional radiation is a logical approach to treatment, especially for the basaloid squamous cell carcinoma of the trachea, given its tendency to metastasize early after definitive therapy.
