A case of a
hemangiosarcoma
involving the bones of the foot in a diabetic patient who had swelling of the left foot is reported. Radiographs demonstrated multiple lytic areas throughout the bones of the foot. Clinically and radiographically, infection was suspected, but the biopsy revealed the presence of tumor.
Metastases
to the tibia were found and an above the knee amputation was performed. Chemotherapy was given, but 6 months later the patient died. Hemangiosarcoma is known to involve the different bones of the foot in a "multicentric" fashion and should be considered in the differential diagnosis of lytic lesions of multiple bones of the lower extremities.

From 1964 to 1984, 87 patients with soft tissue sarcoma (excluding lymphreticulum system sarcoma) in the head and neck were treated in our hospital. All were proved by pathology. The incidence rate of fibrosarcoma was the highest (36.8%),
hemangiosarcoma
the lowest (5.8%). There was no liposarcoma or synovial sarcoma in this group. The most frequently involved site was the area near the nasal cavity and maxillary sinus for fibrosarcoma (66%), the base of tongue for
hemangiosarcoma
(60%), scalp for dermatofibrosarcoma protuberans (54%), parapharyngeal space and soft tissue of the neck for neurogenic sarcoma (45%).
Metastasis
rate to the lymph nodes was 16-20% for rhabdomyosarcoma,
hemangiosarcoma
and malignant fibrohistiocyte tumor. No distant metastasis was found in dermatofibrosarcoma protuberans and malignant fibrohistiocyte tumor. Distant metastasis rate was 10-20% for the other types. Local recurrence rate was 9.1% for dermatofibrosarcoma protuberans, 88.9% for malignant fibrohistiocyte tumor, 30-66.7% for the other sarcomas. The 5- and 10-year survival rates were 20% and 0%, 37.5% and 0%, 91.6% and 91.6% for rhabdomyosarcoma, malignant fibrohistiocyte tumor and dermatofibrosarcoma protuberans. There were differences in clinical behavior for soft tissue sarcomas in the head, neck and in the trunk. Highly malignant sarcomas, such as liposarcoma, synovial sarcoma and rhabdomyosarcoma were rare in the head and neck but the low malignant sarcomas in the head and neck were 3-10 times as common as those in the trunk. Distant metastasis rate of the fibrosarcoma was 20% in head and neck and 6.7%in trunk, the 5- and 10-year survival rates were 36% and 77.9%, 25% and 73.6%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

The clinical appearance, typical localization, diagnostic procedure and the treatment of
hemangiosarcoma
--the most frequent malignant tumor of the heart are reported by the case of a 27 years old woman. The patient suffered from dyspnea, congestion of the superior caval vein and paroxysmal tachycardia. X-ray showed cardiac enlargement due to pericardial effusion. Echocardiography revealed a large tumor in the right atrium. Computertomography and angiocardiography showed tumor masses at the orifice of the superior caval vein and a bypass of the blood flow via the azygos vein. Thoracotomy yielded an inoperable
hemangiosarcoma
. By the combined treatment of irradiation and chemotherapy the cardiac tumor completely disappeared, the patient was temporary symptomless. Later
metastases
occurred and the patient died 13 months after diagnosis.

Hemangiosarcoma was diagnosed in 104 dogs. The mean age was 10 years. Twenty-two breeds were represented. The most common were the German Shepherd Dog and Poodle. Most tumors were in the spleen (65/104). Other primary sites included the soft tissues of the trunk and extremities (18), liver (6), heart (3), and lung (2). Sixty of the dogs were examined because of rupture of the primary tumor or
metastatic disease
. Of the 104 dogs, 4 are alive and 100 are dead: 35 were euthanatized at the time of diagnosis, and the remaining 65 had a mean survival time of 123 days. The mean survival time of the 4 living dogs was 360 days. Forty-seven of the 65 dogs with splenic
hemangiosarcoma
were treated with surgery alone, surgery and immunotherapy, or surgery, immunotherapy, and combination chemotherapy. The same 47 dogs were clinically staged. There was no difference in survival time according to therapy or stage of disease.

We report the clinical, histological and ultrastructural findings of a primary seminal vesicle sarcoma that was proved to be a
hemangiosarcoma
, the first such case reported.
Metastases
to the lungs, pleura and peritoneum were discovered at autopsy.

During the period 1975 to 1984, a histopathologic diagnosis of primary cardiac
hemangiosarcoma
was made in 38 dogs at Angell Memorial Animal Hospital. The diagnosis was confirmed by exploratory thoracotomy in 16 cases and at necropsy in 22 cases. At the time of exploratory thoracotomy, 7 dogs were euthanatized because of nonresectability of the primary tumor and/or gross
metastatic disease
. In 9 dogs, the tumor was resected by removing part of the right atrium. Complications included atrial and ventricular arrhythmias, anemia, disseminated intravascular coagulation, and pneumonia. Prolonged and multiple hospitalizations were a common feature of the postoperative period. Adjuvant therapy was not utilized in any case. The mean survival time was 4 months (2 days to 8 months).

A case of primary
hemangiosarcoma
of the spleen in a 48-year-old woman is presented. Twenty-eight months after splenectomy the patient developed a severe anemia of the microangiopathic type, thrombocytopenia, and a leukoerythroblastic peripheral blood picture. In contrast to x-ray and conventional 99mTc-methylene-diphosphonate (MDP) bone scintigraphy, which showed only a few minor focal changes in the spine and ribs, angioscintigraphy with in vitro labeled 99mTc-erythrocytes revealed extensive pathologic accumulations throughout the spine, femurs, and the liver, indicating the presence of extremely vascular
metastases
. Autopsy 15 months later confirmed the scintigraphic findings. Angiography with 99mTc-labeled erythrocytes seems to be useful for monitoring
metastases
from hemangiosarcomas.

After a 20-year latent period an angiosarcoma developed in the edematous leg of a 74-year-old woman with primary lymphedema. A deep venous thrombosis of the leg which further aggravated tissue swelling preceded the appearance of angiosarcoma. Histogenetic classification of the tumor as
hemangiosarcoma
rather than lymphangiosarcoma was favored by positive immunohistochemical staining for Factor VIII. Despite high amputation and isolated perfusion with hyperthermal cytostatic infusion, she developed local recurrence and distant
metastases
and died 16 months after operation. Patients with chronic primary or secondary lymphedema are susceptible to angiosarcoma although the overall risk is small.

An analysis was made of 17 cases of microscopically proven rib neoplasms, with the objective of evaluating the variability of clinical and radiographic findings, the neoplastic cell types, the occurrence of
metastases
, and the survival time following the diagnosis. In 15 dogs with primary rib neoplasms and in 2 dogs with secondary (presumably metastatic) rib neoplasms, the clinical signs commonly included dyspnea and a localized swelling on the thoracic wall. For the dogs with primary rib neoplasms, the mean age was 4.5 years, with a range of 1.3 to 16 years. On radiography, rib osteolysis, extra and/or intrathoracic soft tissue masses often containing areas of mineralization, and pleural effusion were commonly identified. Ribs 5 through 9 were the most commonly affected and the right side was involved twice as often as the left side. Osteosarcoma was the most frequently diagnosed tumor, but chondrosarcomas and
hemangiosarcoma
were also found. At necropsy, over 50% of the dogs with primary rib neoplasms had
metastases
in the lungs. Less than 10% of the dogs with primary rib osteosarcomas survived more than 4 months after the diagnosis.
