From March 1958 to October 1984, 10 patients with cylindroma (cystic adenoid
epithelioma
) arising in the nasopharynx were treated in our hospital. The presenting symptoms of these patients ranged in duration for 2 weeks to 8 years, with an average of 25 months. Eighty percent of the patients had destruction of the base of skull and 60% had cranial nerve involvement. Of these 10 patients, two had Stage II disease, two had Stage III, and six had Stage IV. Response was seen in this tumor at tissue doses between 5000-8000 rad. Of the seven patients at risk for 5 years, 86% survived, yet 36% of the patients died of tumor between the fifth to tenth year. Fifty percent of the patients lived beyond 10 years. Based on this experience, a dose of more than 8000 rad is advised for the primary lesion. For the neck region, radiation is indicated only when there are lymph node

, which appeared four years after nephrectomy, and was revealed by a Basedow-type goiter. It was treated by total thyroidectomy. A cervical recurrence was diagnosed in the course of a routine checkup and arteriography. Death occurred 22 years after thyroidectomy and 26 years after nephrectomy. The diagnosis is often only made at necropsy. The slow growth of these

This report describes an aggressive squamous cell carcinoma of the skin for which we suggest the name acute
epithelioma
. It is frequently larger than 2 cm and has a characteristic morphology with raised, rolled, vascular but not everted edges. Commonly there is a central crust covering a foul discharge on a papilliferous base. The biopsy is characteristically ambiguous but often that of a well differentiated squamous cell carcinoma. Irrespective of the history, which is usually short, all tumors have a period of rapid growth. Among 193 patients with squamous cell carcinomas of the eyelids or external ear, 24 (12%) of the tumors were designated as being acute epitheliomas. For the eyelids 15/125 (12%) patients were in this group; 10 (67%) had tumors larger than 2 cm, five (33%) developed regional
metastases
, and three (20%) had tumor related deaths. Irradiation controlled the primary tumor in 15 (100%) patients. Among the 68 ear tumors, nine (13%) were called acute epitheliomas. Eight (89%) were larger than 2 cm, three (33%) developed regional
metastases
, and two (22%) patients had tumor related deaths. Irradiation controlled the primary tumor in seven (78%) patients. It is postulated that these acute epitheliomas are virus induced tumors that develop in actinicly damaged and immunologically suppressed skin. The authors present the clinico-pathological correlation in a women who developed at the age of 43 years progressive pain in the left hemiface with motor and sensitive disorders of the left trigeminal nerve. Progressive involvement of the left IIIrd, IVth and Vth cranial nerves appeared with persistence of intractable facial pain which was not responsive to treatment by Tegretol, nor by thermocoagulation of the left Gasserian ganglion. Partial resection of a tumor was performed. The biopsy revealed its cylindromatous nature. The patient died after eight years of evolution. Autopsy confirmed an extensive tumoral invasion of the left temporal bone with invasion of the dura, the left cavernous sinus, and tumoral infiltration of the IIIrd, IVth, Vth and VIIIth cranial nerves on the left side.
Metastases
of the cylindromatous

were found in the liver and adrenals. The precise origin of the tumor could not be ascertained.

A retrospective study of 65 patients with unpalpable breast cancer revealed by microcalcifications on mammography showed that 18 had presented with infiltrating
epithelioma
. The contralateral
epithelioma
was revealed by mammography. Three deaths from
metastases
had occurred in this group. The 65 carcinomas discovered by mammography were treated by excision of the microcalcified area. This was followed by mammectomy in 34 cases and by irradiation in 25. Close surveillance only was suggested in 6 cases. Histological studies showed 6 lobular carcinomas in situ and 59 duct epitheliomas, 26 of which were infiltrating. The prognosis of isolated duct epitheliomas is highly favourable, and no dissemination was observed. The 2 local recurrences observed after secondary irradiation concerned infiltrating epitheliomas; these patients are now alive without sign of recurrence after mammectomy. After conservative treatment (tumorectomy followed by irradiation), no recurrence was observed among patients with non-infiltrating duct carcinomas.

Preclinical as well as clinical studies with fibroblast interferon (IFN) are still lagging behind on those with leukocyte interferon. Its side-effects seem to be less pronounced than those of human IFN-alpha, yet it may be slightly pyrogenic after intravenous injection. Pyrogenicity of current impure preparations might for the larger part be due to impurities. Higher doses of HuIFN-beta than of HuIFN-alpha are required to obtain measurable blood titers by intramuscular injections. Since there is concern about this being due to destruction of the interferon before it has reached its target organ(s), most current clinical studies use either local (e.g. intratumoral) treatment or intravenous infusions. A study of topical treatment for acute rhinovirus infection has indicated that there is very little if any chance for fibroblast interferon to be a clinically useful substance to prevent or cure common cold. In herpetic dendritic keratitis eye drops of fibroblast interferon may be useful as such or in combination with debridement. Topical treatment of warts (multiple intralesional injections) has been shown to yield a high success rate, especially in the case of verrucae vulgares, but less so in the case of verrucae planae juveniles. Studies on condyloma accuminatum are not so far advanced as to permit a documented conclusion. Topical (intralesional) treatment of neoplastic diseases has been investigated, especially in Japan, to demonstrate that fibroblast interferon does have an antineoplastic effect in vivo. While there seems to be little doubt that local delivery does indeed cause tumor nodules to regress, the question is whether this procedure can offer a true clinical benefit to the patient. Systemic (intravenous) administration for chronic hepatitis B has been investigated further: given alone or in combination with adenine-arabinoside, fibroblast interferon seems to be able to reduce the level of viral activity. Whether this will lead to a generally accepted treatment of chronic active hepatitis is difficult to say at this moment. In treating herpes zoster in cancer patients, results have been obtained which are comparable to those found for leukocyte interferon. The practical significance of this finding must be seen in the perspective of recent developments in the chemotherapy of herpes zoster. In breast cancer patients given intramuscular injections,

Certain accidents related to local anaesthesia during bronchial fibroscopy may be due to overdosage of local anesthetic drugs. In view of the various techniques employed, the various doses used and the different serum levels obtained, we decided to study the passage of lidocaine into the systemic circulation during bronchial fibroscopy in order to confirm that the doses required for good anaesthesia are not toxic. We studied the kinetics of lidocaine on 10 occasions in 9 patients. All patients had normal renal, hepatic and cardiac function. The anaesthetic was administered in standardized doses (by pharyngo-laryngeal spray of a 5% solution followed by injection via a laryngeal syringe and finally by the operating lumen of the fibroscope with a 1% solution). The total dose of anaesthetic was administered at the beginning of the examination in order to avoid contamination of the aspirated secretions after connection of the traps. The mean dose administered was 9.2 +/- 0.5 mg/kg (range of 380 to 800 mg). The maximal serum levels never attained toxic levels (9 mcg/ml). The highest levels (7 mcg/ml and 5.7 mcg/ml) were observed in a patient with a bronchial
epithelioma
, normal biochemistry and normal liver ultrasonography, who died two months later with hepatic
metastases
. The mean maximal serum levels for this group of patients was 2.8 +/- 0.6 mcg/ml between the 5th and 45th minute, which is equivalent to anti-arrhythmic doses (therapeutic level of 1.2 to 5 mcg/ml).
