Chondroid syringoma, previously called mixed tumor of the skin is a fairly uncommon type of sweat gland tumor, most often diagnosed in the sixth and seventh decade of life. It presents as a well encapsulated dermal or subcutaneous module and it is most frequently found in the skin of the head and neck. The neoplasm is asymptomatic and is featured by a slow rate of growth. At histopathological examination, the tumor consists of an epithelial component, with glandular or ductal differentiation, either eccrine or apocrine, and a stromal component with myxoid or chondroid elements. Surgical removal is the treatment of choice and recurrence may occur. Malignant chondroid syringoma has been reported rarely, most frequently arising in previously benign lesions. They behave as lethal tumors and often metastasize to the regional lymph nodes and lung. We report a most unusual case of a mixed tumor of the skin localized in the sacral area associated with a peripheral low grade malignant component of the adenocystic type. Association of these two types of sweat gland tumors has never been previously described, to our knowledge.

Malignant chondroid syringoma, a very rare tumour, presenting with multiple pulmonary metastases in a 50 year old woman is described. Initial diagnostic confusion with pulmonary hamartoma occurred due to histopathological similarities. However, re-examination of a skin biopsy specimen taken 17 years previously from a hand lesion yielded the necessary information to identify the pulmonary lesions definitively as metastases from the original skin lesion. The features of this very rare indolent tumour are described.

Chondroid syringoma represents the cutaneous counterpart of mixed tumor (pleomorphic adenoma) of salivary glands, therefore it is also termed "mixed tumour of the skin." It is generally accepted that there are both eccrine and apocrine variants of mixed tumors of skin. Malignant chondroid syringoma is a malignant eccrine neoplasm that is very rarely encountered in radio-pathological and clinical practice. Unlike the benign variety which occurs most often on the head and neck, malignant chondroid syringomas are most often found on the trunk and extremities. The usual presentation is that of an asymptomatic slow-growing mass in the region of head or neck. We present here a case report of histopathologically proven malignant chondroid syringoma of scalp in occipital region, with dural invasion. Malignant chondroid syringomas clinically appear as very large nodules and they metastasize at a very high rate. The treatment of these malignancies is surgical excision.
