Basing on an analyses of pretherapeutic CT examinations of 333 patients suffering from squamous cell carcinomas of the head and neck region (206 of these had lymph node metastases), the clinical diagnostic value of CT for lymph node staging was determined. CT criteria for metastatic infestation of the lymph nodes were the lymph node size (greater than 15 mm) and structural characteristics (inhomogenity, central hypdensity with rim enhancement), and it became evident that CT with its 93% sensitivity and 89% accuracy was superior to clinical palpation, where the sensitivity was only 64% and the accuracy 72%. It can be concluded from these results that CT provides a reliable basis for a therapeutic approach.

This report describes a 67-year-old male with inoperable gastric cancer accompanied by marked tissue and peripheral eosinophilia without evidence of allergic disorders or parasitic infestation. Autopsy revealed an advanced gastric cancer of scirrhous type with metastases to pancreas, bone marrow, ileum, lungs, and lymph nodes. Excessive numbers of mature eosinophils were present in univolved bone marrow, liver and spleen as well as among the signet ring cell component of the cancer in either primary or metastatic sites. The primary cancer also possessed a component of tubular adenocarcinoma which was associated with only a few eosinophils. Hence, we speculate that an eosinophil mobilizing (chemotactic and/or proliferating) factor (s) was produced by the signet ring cancer cells.

In a series of 1095 Egyptian patients with carcinoma of the bladder treated by radical cystectomy, 902 cases (82.4%) contained schistosome eggs in the specimens, and 193 (17.6%) were egg-negative. The different tumor parameters were compared in these subgroups to explore any differences that could be related to schistosomal infestation. In egg-positive cases, the tumor developed at a younger age (46.7 years) than in egg-negative cases (53.2 years). Squamous cell carcinoma, commonly of low grade, predominated in the egg-positive group. No difference was observed in the frequency of tumor stages or lymph node metastases between the two subgroups. The limited tendency to distant spread in schistosomal bladder cancer, despite its advanced local stage, is accounted for by the high frequency of low grade tumors rather than the limiting effect of local schistosomal tissue reactions.

Transcutaneous fine needle biopsy of retroperitoneal lymph nodes after lymphography minor is connected with only discomfort for the patient. It is used for clarification of a possible infestation of metastases by malignant growth, the lymphatic drainage of which is situated retroperitoneally. A new instrument for puncture is presented, the use of which permits increase in accuracy of impact and reduction of radiation exposure for both patient and examiner. These advantages are of great importance, especially in serial puncture of several lymph nodes along the lymphogenous pathway of metastases. The quality of the fluoroscopic picture is even improved by the instrument for puncture.

Cholangiocarcinoma is very common in areas endemic for the liver fluke Opisthorchis viverrini. Survival after surgical treatment of cholangiocarcinoma associated with opisthorchiasis was studied prospectively in 30 patients, all of whom resided in an endemic area. The median age was 52 (range 32-69) years and twenty-five patients were men. Seven patients had their tumours removed, four with concomitant liver resection. Twenty-two patients underwent palliative biliary bypass procedures to a segmental duct. Laparotomy with biopsy of metastatic lesions was undertaken in one patient with multiple lymph node metastases and peritoneal seedlings. No patient received postoperative chemotherapy or radiation treatment. Patients were followed for 2 years or until death. The 1-year survival rate after tumour resection was 86 per cent and the 2-year survival rate 43 per cent. After palliative procedures the 1-year survival rate was 26 per cent; no patient reached 2 years and the median survival time was 8 months. Survival after surgical treatment of cholangiocarcinoma in patients with opisthorchiasis is broadly similar to that reported for cholangiocarcinoma without liver fluke infestation.

The aim of this study was to evaluate [18F]fluorodeoxyglucose ( F-FDG) imaging of recurrent or inoperable lung cancer using a hybrid positron emission tomography (PET) device of the third generation. Examinations were compared with the results of conventional staging. Thirty-six patients suffering from recurrent or primarily inoperable lung cancer (29 men, seven women; age 64.8+/-12.0 years) were examined using hybrid PET (Marconi Axis gamma-PET ) 60 min after injection of 370 MBq F-FDG. The data obtained were reconstructed iteratively. All patients received a computed tomography (CT) scan using either the spiral or multislice technique. All lesions suspicious for primary or recurrent tumour were verified by biopsy; mediastinal lymph nodes were considered as malignant, when positive histology or a small axis diameter of greater than 1 cm measured with CT in addition to progression of clinical course was found. Distant metastases were diagnosed by CT and bone scintigraphy. Using hybrid PET all lesions showed a focally elevated glucose metabolism. Lymph node involvement of the ipsilateral peribronchial and hilar station (N1) was identified in 24/26 cases (92%), in 26/29 cases (90%) of ipsilateral central manifestation (N2) and in 11/13 (85%) cases of central contralateral or supraclavicular lymphatic infestation (N3). Pulmonary spread in hybrid PET was found in 4/8 cases (50%), whereas mainly lung metastases with a diameter of 1.5 cm and smaller were missed. Pleural involvement diagnosed by CT was verified in 4/5 patients. All four patients with bony metastases in conventional staging also presented with positive findings in hybrid PET (8/9 lesions). Concordance with conventional staging was found in 28/36 of patients (78%). In 4/36 patients (11%) unknown sites of tumour were detected leading to therapeutic consequences in three patients after radiological confirmation. Hybrid PET would have led to an understaging in four cases (11%), resulting theoretically in inefficient treatment in two patients. Hybrid PET for F-FDG imaging in the staging of recurrent or primarily inoperable lung cancer supplied equal (78%) or more information (11%) compared to conventional staging procedures. Using the information of hybrid PET alone, 11% of the patients would have been understaged. We conclude that hybrid PET has the potential for use as an additional staging tool in this subgroup of patients, providing supplementary information compared to conventional staging modalities.

Echinococcus alveolaris (E. alveolaris) (multi-localaris) is a parasitic agent that commonly affects the liver and poses as a solid mass. In contrast to Echnicoccus granulosus, E. alveolaris tends to invade adjacent structures rather than displace and compress them. Haematogenous dissemination to other organs such as the lungs, brain, or bone is rare. In this case, we report on a 34-year old woman with a history of liver segmentectomy due to E. alveolaris infestation and the disease disseminated to the lungs. Computed tomography and magnetic resonance imaging showed multiple opacities and mixed signal intensities in the lungs, mimicking metastatic disease. The biopsy proved that the lesions were a result of metastasis of E. alveolaris.

In the treatment of osteoporosis, M. Kahler and bone metastases from prostate and breast cancer bisphosphonates play a major role. Not all patients respond well to bisphosphonate treatment. Since a few years adverse effects of these drugs have been reported. A new drug, denosumab, a fully human monoclonal antibody to RANKL, has recently been developed. This case reports a 74-year-old male patient with a medical history of diabetes mellitus, angina pectoris, coronary bypasses, hypertension, and prostate cancer with multiple metastases to lymph nodes, bone and lungs. The prostate cancer was treated according to the protocol. But he was never treated with bisphosphonates. Instead he was included in a phase III randomized double blind multicenter trial, testing the efficacy of denosumab compared to zoledronic acid in the treatment of bone metastases of hormone resistant prostate cancer. Only 7 months after start of denosumab infectious symptoms developed, followed by infestation of the mandible. Despite surgical treatment fistula and exposed bone remained. This case illustrates that use of denosumab can lead to a type of osteonecrosis resembling bisphosphonate related osteonecrosis of the jaws.

Despite all the progress in clinical diagnostics and multimodal therapy, the prediction of oropharyngeal carcinoma remains to be limited. Therefore, it is important to further improve clinical staging to set therapy modalities based on that.In a retrospective pseudonymized study over a 13 years period of time the clinical classification (cTNM) was compared with the histopathologic classification (pTNM) of 84 patients with oropharyngeal squamous cell carcinoma and who were treated primarily with surgery. Furthermore, the existence of occult metastases as well as overall survival as a function of histopathologic extent of the tumor and regional infestation of lymph nodes were considered.The highest agreement between cTNM and pTNM was observed with computer tomography. A definite evaluation with histopathological reliability is not possible with cTNM. MRT examinations showed the tendency to estimate T -, N- and UICC-stage compared with the histopathologic findings. In the context of the examinations of survival probabilities, separated by sex and histopathologic T- and N-categories, a lower overall survival probability was seen for the higher T- and N-category.Despite all the progress in imaging diagnostics for patients with oropharyngeal carcinoma the cTNM, compared with pTNM, is only partially able to determine the TNM classification of the cancer with requested clinical accuracy. With the trend of decreasing predictions and in spite of present-day multimodal therapy it is essential to improve and optimize the pre-therapeutic clinical staging.

A patient who underwent mastectomy of the left breast owing to breast cancer was referred to our department for abdominal ultrasonography during her routine check-up. Radiological examinations demonstrated multiple masses that tended to form clusters in the liver parenchyma, and the lesions were initially thought to represent metastases from the breast cancer. Multisite biopsies and serological tests confirmed the diagnosis of Fasciola hepatica infestation. To our knowledge, this is a unique case report of a patient with a known malignant neoplasm. We also present an overview of the literature about human fascioliasis in Turkey.
