Two hundred and twenty-one patients with invasive bladder cancer had routine bone scintigraphy performed as part of tumour staging. The incidence of detectable metastases was 12%. Skeletal scintigraphy at diagnosis has a sensitivity of only 38%. The predictive value of a negative examination was 92%. These results suggest that skeletal scintigraphy is not indicated as a routine staging procedure in carcinoma of the bladder.

Hypertrophic osteoarthropathy secondary to metastases from carcinoma of the bladder is reported in a seventy-year-old man. A review of the literature has not revealed an association of hypertrophic osteoarthropathy with pulmonary metastases from carcinoma of the bladder.

The authors report the preliminary results of attempts at treatment using radiotherapy and chemotherapy for inoperable carcinoma of the bladder. Chemotherapy alone (cis diamine dichloropalatinum : CDDP) used twice for metastases gave 1 regression of more than 50 % and 1 failure, used twice for non-irradiated pelvic tumours gave 1 regression of more than 50 % and finally was associated with no improvement in 7 patients already irradiated. Chemotherapy (CDDP) associated with a series of flash doses of radiotherapy was used in 6 cases and gave one regression of more than 50 % and one of less than 50 %. Bleomycin in continuous infusion associated with flash radiotherapy was used in 3 cases and gave a regression of more than 50 % of pelvic tumours in two. These poor though not completely nil results have lead the authors to develop a cooperative treatment programme involving the urologist, radiotherapist and chemotherapist.

CT staging was performed in 49 consecutive patients with known carcinoma of the bladder. The overall accuracy of CT staging in 25 patients with surgically confirmed disease stage was 64%. Most diagnostic errors in this series were related to the determination of perivesical fat involvement by tumor. The overall accuracy of CT in predicting lymph node metastasis was 92%. The sensitivity was 60% and the specificity ws 100%. Because of the current management of bladder carcinoma, the major role of CT is in the evaluation of lymph nodes for metastatic disease. Since CT cannot detect metastasis to nonenlarged lymph nodes, it has only a limited role in the staging of bladder carcinoma at this time.

In a series of 1095 Egyptian patients with carcinoma of the bladder treated by radical cystectomy, 902 cases (82.4%) contained schistosome eggs in the specimens, and 193 (17.6%) were egg-negative. The different tumor parameters were compared in these subgroups to explore any differences that could be related to schistosomal infestation. In egg-positive cases, the tumor developed at a younger age (46.7 years) than in egg-negative cases (53.2 years). Squamous cell carcinoma, commonly of low grade, predominated in the egg-positive group. No difference was observed in the frequency of tumor stages or lymph node metastases between the two subgroups. The limited tendency to distant spread in schistosomal bladder cancer, despite its advanced local stage, is accounted for by the high frequency of low grade tumors rather than the limiting effect of local schistosomal tissue reactions.

Hypertrichosis lanuginosa developed in a patient with carcinoma of the bladder. After regional lymph node metastases became evident, soft, fine, blond hair developed on his entire body, with the exception of the soles and the palms. Extensive endocrinologic evaluation demonstrated depressed testosterone levels and elevated levels of leutinizing hormone, changes not thought to be related to the hair growth. To our knowledge, this patient constitutes the second reported case of this rare syndrome in association with bladder carcinoma.

Thirty-two patients with vesical leukoplakia have been seen in the St Peter's group of hospitals during the past 20 years. Twenty-five (78%) were men and 7 (22%) women with an average at diagnosis of 55 years. Seventeen (53%) had a history of chronic or recurrent urinary infection whilst 9 (28%) had previously developed bladder stones. Nine patients (28%) have so far developed a carcinoma of the bladder in association with the leukoplakia and, of these 4 are dead, 2 having died of metastatic disease and 2 in the immediate post-operative period. Eight of the 9 malignancies were squamous carcinomas.

A clinical and pathological analysis was done on 87 autopsy cases of carcinoma of the bladder. Of the 87 cases 68 were men and 19 were women, with a mean age of 64 years. The average survival time was 39 months from the onset of symptoms to death. The patients with multiple tumors had a longer average survival time than the patients with single tumors (51 compared to 40 months), and the average survival times were definitely more favorable for men than for women (43 compared to 23 months). Histological examination revealed transitional cell carcinoma in 77 patients, squamous cell carcinoma in 4, vesical adenocarcinoma in 3 and urachal adenocarcinoma in 3. Squamous cell carcinoma and vesical adenocarcinoma seemed to be more malignant neoplasms. Metastasis was noted in 58 of 87 cases (66.7 per cent) and occurred most frequently in lymph nodes (37.9 per cent), liver (29.9 per cent), lungs (29.9 per cent) and bones (24.1 per cent). Of the 87 cases of bladder carcinoma 11 had primary carcinoma in organs other than the bladder.

Primary small cell carcinoma of the bladder is a rare condition with approximately 100 cases reported to date. An incidence of 0.7% small cell carcinoma was found in a total series of bladder tumors diagnosed in Western Sweden during a 2-year period. We describe 22 patients with primary small cell carcinoma followed to death or for at least 6 years. Three additional cases were diagnosed at autopsy. Of 18 patients with stages T2M0 to T4M0 cancer treated with locoregional therapy 5 (28%) had no evidence of disease after a median observation of 10 years (range 6 to 18) and 13 died of disease after 0.5 to 19 months (median 7.3). Two patients with metastatic disease were treated with systemic chemotherapy but died within 15 months. Two patients died shortly after the diagnostic biopsy. Our study and a review of the literature indicate that primary small cell carcinoma of the bladder is less aggressive than its pulmonary counterpart and that some patients can be cured by transurethral resection, or partial or radical cystectomy combined with radiotherapy.
