In 14 patients with
adenocarcinoma of the colon
or rectum (Duke stages A to D), positive results for neuraminidase were obtained with the isolated leukocytes at pH 6. These were tested by a cytochemical method using a chromogenic substrate for neuraminidase. Under the same conditions, normal leukocyte neuraminidase has a pH optimum of 4. The percentage of leukocytes showing neuraminidase activity increased with the progression of Duke stage and presence of liver or peritoneal

From May 1984 through December 1986, 141 patients with metastatic
adenocarcinoma of the colon
or rectum were entered on this Hoosier Oncology Group (HOG) trial evaluating the role of cisplatin in systemic therapy. Patients were stratified by the presence or absence of hepatic
metastases
and by performance status, and were subsequently randomized to receive fluorouracil (5-FU) (15 mg/kg/wk) alone or the same dose of 5-FU plus cisplatin (60 mg/m2 every 3 weeks). The total duration of treatment was six cycles (18 weeks). In 132 fully evaluable patients the objective response rates were 19% for 5-FU and 22% for 5-FU plus cisplatin. Statistically, the median survival times of 40 and 39 weeks were not significantly different (P = .62). However, the median duration of remission (MDR) was superior (P = .05) for 5-FU alone. This study fails to confirm clinically significant synergy of 5-FU plus cisplatin in the treatment of metastatic colorectal cancer.

Malignancies of the appendix are extremely rare. Of the reported cases, carcinoid tumors are most common, with mucinous cystadenocarcinomas and adenocarcinomas next in frequency. Adenocarcinoids, which share morphologic and clinical features that are a composite of adenocarcinomas and carcinoids, have been described. Concomitant second malignancies, most often in the GI tract, occur with greater than expected frequency in patients with appendiceal tumors. Treatment and prognosis are markedly different for each of the histologic variants of appendiceal malignancy. In general, carcinoid tumors (smaller than 2 cm in diameter) may be treated by appendectomy alone and are associated with long term survival. Adenocarcinomas, unless well differentiated and extremely superficial and localized, required hemicolectomy and confer a prognosis that is, stage for stage, similar to that of
adenocarcinoma of the colon
. Mucinous cystadenocarcinomas may spread widely through the abdomen, resulting in P peritonei, but can be associated with long survival duration if surgical treatment is aggressive. There is evidence that adenocarcinoids develop from simultaneous neoplastic change in two stem cell populations.
Metastases
may show histologic features of adenocarcinomas alone, of carcinoid tumors alone, or may resemble a composite neoplasm. The prognosis is variable and treatment recommendations are not well defined.

Twenty-two patients with pulmonary
metastases
from
adenocarcinoma of the colon
and rectum had sputum samples examined for cytology. All patients had lesions demonstrated on chest roentgenography and eight patients also underwent bronchoscopy. Three or more sputum samples were examined for eight patients, two samples for seven patients, and seven patients had one sample only examined. In five of 22 patients, the sputum samples were reported to be unsatisfactory for cytodiagnosis. Of the remaining 17, nine patients had at least one positive result for metastatic adenocarcinoma and in two additional cases the sample showed atypical cells. The result was therefore positive or atypical in 11 of 17 patients (65 percent). The positive cytology rate reported (79 percent for multiple pulmonary
metastases
) represents an improvement compared with previously published work. In this study 14 patients had either one or two samples collected for cytology and it is likely that the positivity rate would have been higher if three samples had been collected.

A retrospective review of 176 patients with
adenocarcinoma of the colon
who underwent total colonoscopy preoperatively demonstrated synchronous carcinomas in 3.4 percent and synchronous polyps in 55.1 percent. Full-column barium enemas (68 patients) failed to identify cancer in 22 percent of patients and synchronous polyps in 58 percent of patients, a statistically significant (P less than 0.001) number of false-negative examinations. Double-contrast barium enemas (30 patients) failed to identify cancer in 27 percent of patients and synchronous polyps in 42 percent of patients, also a statistically significant (P less than 0.007) number of false-negative examinations. Full column and air contrast barium enemas identified all index cancers with distant
metastases
. Air-contrast barium enemas failed to identify 40 percent of "early" index cancers (confined to the bowel wall, negative nodes), and full-column barium enemas failed to identify 32 percent. The incidence of synchronous carcinoma and polyps underscores the need for total colon evaluation when a primary carcinoma is detected. Because of the poor accuracy of barium studies, total colonoscopy is the method of choice for this evaluation.

, her serum progesterone and estradiol concentrations gradually declined. The unusual hormone production in the patient was confirmed by regression of clinical symptoms and by in vitro endocrine assays performed on cells from ovarian tumor grown in tissue culture. The tumor consumed pregnenolone and produced progesterone and estradiol in large quantity.

A case of
adenocarcinoma of the colon
is reported, whose
metastases
exhibited a syncytiotrophoblastic differentiation that gradually increased with advancing distance from the primary. Immunohistochemically, syncytiotrophoblastic giant cells with strong positivity for beta-HCG could be demonstrated within the metastatic tissue. Also, there was beta-HCG positivity in a fraction of the atypical epithelial elements of the primary. The case is discussed with respect to the differential diagnosis, and the cause of the syncytiotrophoblastic differentiation is attributed to the phenomenon of genetic instability. The case shows that by virtue of genetic instability totally new differentiations may be elaborated, so that the phenotypical appearance of a given tumor does not necessarily allow conclusions as to its tissue of origin. Thus, observations such as the one presented here clearly indicate that the histogenetic principle of tumor classifications by far does not apply to all cancers and that, therefore, its value and validity are considerably limited.
