Cloacogenic carcinoma
of the anus is a very uncommon tumor, and this is the first report of a major response to chemotherapy alone. Adriamycin and/or CDP should be considered in the therapy of this tumor.

Cloacogenic carcinoma
is a rare tumor of the anorectal region originating from a persistant remnant of the cloacal membrane of the embryo. The tumor accounts for 2-3% of anorectal carcinomas and occurs more than twice as often in women. Most tumors present as fungating or ulcerating lesions, but the tumor may arise in anal ducts and present as a submucosal mass. Wide abdominoperineal resection is the treatment of choice with a five year survival of 50%.
Metastases
occurs to the inguinal lymph nodes in more than 50% of the patients at sometime during the course of the disease with distant
metastases
occuring most commonly to liver and lungs.

A case of cloacogenic carcinoma with cutaneous
metastases
in a 74 year old man is reported. The patient presented with a six months history of two polyps of the lowest portion of the anal canal. Physical examination of the inguino-scrotal area revealed a 5 X 7 cm. hard infiltrative plaque with three ulcero-vegetative scars. Histologic examination of both polyps revealed two of the four cytologic patterns described in this neoplasm. One polyp showed undifferentiated cells, type IV and second one, type II differentiated. The biopsy of the cutaneous
metastases
showed dermal infiltrative clusters of the cytologic type II differentiated epidermal cells. Treatment with radiotherapy and chemotherapy was ineffective.

Cloacogenic carcinoma

From April 1958 through July 1976, 23 patients with transitional
cloacogenic carcinoma of the anus
were treated with curative intent by abdominoperineal resection alone (15 patients), combination abdomino-perineal resection and irradiation (6 patients), and irradiation alone (2 patients). None of these patients had been previously treated or had distant
metastases
at the time of presentation. Initial presentation survival, and complications of treatment are analyzed. Surgery alone failed to control local and pelvic disease in 1 of 6 patients with negative nodes (with a tumor greater than 2 cm in diameter) and in 6 of 9 patients with positive nodes. No local and pelvic failures occurred in patients treated with irradiation alone or preoperative or postoperative irradiation.
