A prospective study is presented of frozen section examinations (FS) performed in parallel with 265 consecutive sentinel lymph node procedures (SLNP) over a 20-month period. The final pathological study included immunohistochemistry (IHC) for keratin if the haematoxylin-eosin (HE)-stained section was tumour free. FS correctly identified node-positive or node-negative axillae in 235 cases. In 28 SLNPs the final examination gave a positive result not detected in the FS, resulting in reoperation. In 21 of these false-negative (FN) cases micrometastases (MIM) were present. There were no false-positive cases, but in two cases of lobular carcinoma the findings in the FS were equivocal, the final reports recording
metastases
in one but not in the other.
Lobular carcinoma
and other less common subtypes of carcinoma were overrepresented, ductal carcinoma not otherwise specified (NOS) being less likely to affect the FN findings.

Psoriasin (S100A7) is a member of the S100 gene family that is involved in psoriasis, which can be induced in cultured squamous epithelial cells. Previously, this molecule has been shown to be over-expressed in human ductal carcinoma in situ and invasive breast cancers. This current study examine the levels of expression of this molecule in a group of human breast tumours, with particular emphasis on the relationship with the clinical outcomes using quantitative PCR and the distribution in tumours using immunohistochemistry. Psoriasin is primarily expressed in breast cancer cells, and at very low level in normal breast epithelial cells. Quantitative analysis of psoriasin mRNA has shown that breast tumour tissues exhibited a significantly high level of psoriasin compared with normal tissues (p=0.0026). The levels do not correlate with nodal status of breast tumours, however levels in grade 2 and grade 3 tumours were significantly higher compared with that in grade 1 tumours (p=0.07 and p=0.0015, vs grade 1 respectively).

also had higher levels of psoriasin compared with ductal tumours. The most interesting observation is that levels of psoriasin were significantly higher in patients who developed
metastatic disease
and in patients who died of breast cancer (p=0.02 and p<0.001, vs disease-free, respectively). It is concluded that aberrant expression of psoriasin is commonly seen in human breast cancer and that excessively high levels are correlated with the clinical outcomes.

This study was initiated to evaluate the efficacy of luteinizing hormone-releasing hormone (LH-RH) agonists in protecting premenopausal patients against the adverse gynaecological effects induced by tamoxifen. Between January 1998 and January 2000, 85 premenopausal breast cancer patients were included in this prospective study. All were to receive LH-RH agonists and tamoxifen for a minimum of two years. All patients underwent a pretreatment gynaecological evaluation and annual follow-up. Bone density was also measured at the start of treatment and then after 2, 3 and 4 years. Pretreatment evaluation revealed 2 polyps. At one and two years of follow-up, no abnormal symptoms were noted and echographic findings were normal. At three years of follow-up, a polyp associated with adnexal masses was discovered. Histology revealed ovarian and endometrial

to the small bowel and ovaries from pleomorphic type infiltrating
lobular breast carcinoma
presenting as intestinal obstruction. Barium transit study revealed a stenosis in the jejunum and another in the terminal ileum at the ileocecal valve area. A flat formation in the area of the ileocecal valve was observed on endoscopy but the results of biopsy were negative, a fairly frequent finding due to the absence of infiltration of the intestinal mucosa. Pathological and immunohistochemical study of the intestinal and ovarian surgical specimens confirmed the diagnosis of

In planning treatment of a gastric neoplasm in a patient previously treated for
lobular breast carcinoma
, it is important to differentiate a primary gastrointestinal tract tumor from a metastatic form. We report a case of a breast lobular carcinoma metastatic to the stomach. The patient underwent a subtotal gastrectomy for symptomatic disease. Although gastric symptoms appeared 14 years after the breast carcinoma, immunohistochemical analysis of the surgical specimen helped to establish that the gastric lesion, thought to be primary, was effectively a metastatic repetition of the breast neoplasm. To better define treatment in a gastric neoplasm patient previously treated for breast carcinoma, the preoperative diagnosis should rule out a
metastatic disease
. The patient described received an adjuvant chemotherapy according to breast cancer protocol after gastric resection for symptomatic disease. The patient is still alive and undergoing chemotherapy for peritoneal carcinosis.

from a lobular carcinoma of the breast in a 73-year-old woman who had undergone a left mastectomy with axillary dissection 15 years earlier. Metastatic breast cancer involving the hepatobiliary tract or ascites secondary to peritoneal carcinomatosis has been well described. Luminal gastrointestinal tract involvement is less common and recognition of the range of possible presentations is important for early and accurate diagnosis and treatment. We report 6 patients with a variety of presentations of metastatic breast cancer of the luminal gastrointestinal tract. These include oropharyngeal and esophageal involvement presenting as dysphagia with one case of pseudoachalasia, a linitis plastica-like picture with gastric narrowing and thickened folds, small bowel obstruction and multiple strictures mimicking Crohn's disease, and a colonic neoplasm presenting with obstruction.
