Between 1983 and 1988 197 patients underwent mammography and fine needle aspiration cytology procedure beside physical examination before operation for breast tumor in our clinic. The sensitivity of mammography and fine needle aspiration cytology were 81.9 and 66.1%, their specificities were 91.2 and 95%, the specificity of physical examination was 97.7%. When all three examinations indicated malignancy, this was confirmed by histology without exception. When the examinations showed benign lesion in concert, four of the 26 diagnoses were false, histology proved breast cancer smaller than 2 centimetres in diameter without lymph node
metastases
in each case. Those aspirations which were considered unsatisfactory because of the lack of cells supported the diagnosis of a
benign tumor
raised by physical examination and mammography. Our data indicate that the diagnosis yielded by the combination of the three examinations may support therapy more than it does in the present.

The levels of several tumor associated proteases, including plasminogen activators (PA), are elevated in many malignant tumors compared to their
benign tumor
counterparts. Extracellular matrix degradation mediated by PA may facilitate tumor cell invasion and metastasis. To assess whether PA content correlates with the aggressive phenotype in prostate cancer, we studied these activators in the PC-3 human prostate cell line and PC-3CALN, an aggressive in vivo derived variant cell line. Enzymatic assays using H-D-val-leu-lys-pNA (S-2251) as substrate and peroxidase-anti-peroxidase immunohistochemical techniques were used. In an in vitro chemoinvasion assay, the PC-3CALN variant cell line demonstrated significantly greater invasive behavior than the unselected, parental PC-3 line. The activity of PA secreted by PC-3CALN cells was 3.5 times greater than that of PC-3 cells (p less than 0.01). PC-3

obtained following intrasplenic injection of PC-3 cells had greater PA activities than the corresponding primary tumors. Immunohistochemical studies of PC-3 tumors demonstrated preferential localization of urokinase-type PA to areas of apparent tumor cell invasion. These data suggest a correlation between PA and the aggressive phenotype in this model of human prostate cancer. PA, in particular u-PA, may play a role in the migration and invasion of prostate cancer cells and provide a marker of the aggressive phenotype.

Since there is some controversy over the best way to treat this disease, and since its histogenesis is still unknown, the authors elected to report the case of a 28-year-old white male for three reasons: first, to illustrate that the tumor is of neural crest derivation; second, to show that an ultrastructural evaluation of the tumor is imperative in some cases; and lastly, to suggest that amputation is the treatment of choice in localized disease.

Ten patients underwent resection of primary or secondary cardiac tumor. Two-dimensional transthoracic echocardiography per se accurately located the endoluminal cardiac mass in nine patients, and transesophageal echocardiography demonstrated a right atrial tumor in the tenth case. The indications for urgent surgery included prior embolic events (3 cases), syncopal attacks (2) or echocardiographic evidence of a multilobulated mass (2 cases). The operative strategy was standardized for atrial tumors, but for malignant myocardial neoplasm both the anatomic site and the extent of tumor growth determined the surgical procedure. Histologic examination showed myxoma in seven cases, fibroma in one and

of malignant melanoma in two cases. The course after resection of endoluminal

A growth rate analysis of lung metastases of benign giant cell tumor (GCT) of bone, histologically indistinguishable from the primary
benign tumor
, is presented. The doubling time (DT) was 186 days in the first 13 months of growth, and 465 days in the last 4 months. These DTs are much longer than DT of lung metastases of any other tumors, and have a good correlation with a long survival time. Backward extrapolation showed that the lung metastases began to develop years before the primary tumor was diagnosed and treated. If this can be confirmed in a larger number of patients, it could be concluded that there will be no indication for amputation as a treatment for benign GCT whether or not lung metastases are present. The growth rate analysis supports the concept that these
metastases
might represent a benign process, and as such are a unique histologic and clinical phenomenon.
