Twenty-nine consecutive patients 2-35 years old underwent serial thoracic CT evaluations for
metastatic disease
. Thymic volumes were determined for each patient during cycles of chemotherapy and were compared with the patient's clinical status. This group included patients with Hodgkin's disease (13 patients), osteogenic sarcoma (five),
testicular neoplasm
(four), Wilms' tumor (three), rhabdomyosarcoma (two), malignant fibrous histiocytoma (one), and Ewing's sarcoma (one). Seven patients with mediastinal lymphoma had tumor involvement of the thymus and therefore were excluded. The 22 remaining patients showed cyclic thymic volume changes in response to chemotherapy or its discontinuance. During the first course of chemotherapy the thymic volume decreased by an average of 43% in 20 of 22 patients. Between the first and second course, regrowth was observed in all 20 of these patients. Among the six patients who received a second course of therapy, an average volume decrease of 36% was observed during the second course with regrowth again occurring during recovery from chemotherapy. Thymic rebound (regrowth 50% greater than baseline volume) occurred in five patients, three of whom were in clinical remission. The thymus appears to atrophy during the administration of chemotherapy and regrow during the recovery phase of chemotherapy in 90% of the patients studied. Thymic hyperplasia or rebound is a relatively common phenomenon occurring in 25% of patients. The size of the thymus appears to be extremely sensitive to chemotherapy.

, orchiectomy was performed on May 14,1984. PVB chemotherapy (Cis-diamminedichloro-platinum, Vinblastine and Pepleomycin) was administered. Because he got worse, however, he was treated with another combination chemotherapy, consisting of Methotrexate (MTX, 100 mg/m2 intravenous push (i.v.), 200 mg/m2 12-h infusion, day 1. The dose of MTX was increased with each course. Maximum dose of MTX was 900 mg/m2/day), Vincristine (1.0 mg/m2 i.v. day 1.) Actinomycin D (10 micrograms/kg i.v. days 3.4.5), Cyclophosphamide (600 mg/m2 i.v. day 3.), Adriamycin (30 mg/m2 i.v. day 8.) and Melphalan (6 mg/m2 p.o. day 8.). After 6 courses of this regimen, distant
metastases

Ten cases of a distinctive
testicular neoplasm
characterized by large cells with abundant eosinophilic cytoplasm and calcification are reported together with two similar cases from the literature. A Sertoli cell origin of these tumors was established by their architectural and cytologic characteristics, as well as their frequent growth within seminiferous tubules. A high association was found with unusual features, including bilaterality (six cases), multifocality (six cases), complex endocrine disorders, (three cases), and familial occurrence and cardiac myxoma (two cases). Eleven of the 12 cases had benign courses, but one was associated with

Spermatocytic seminoma is an uncommon variety of
testicular neoplasm
, differing from its classical counterpart at both clinical and pathological levels and having a low propensity to
metastasize
. This is a retrospective review of ten patients with pathologically confirmed and reviewed disease who were seen at the Royal Marsden Hospital. All patients presented as Stage I. Five were treated with radiotherapy and five have undergone surveillance. The median age was 56.5 years. The median follow-up is 8 years and no patient has relapsed. Two have died of intercurrent disease. Our series supports others of a similar size in the literature with respect to both the rarity and the good prognosis of spermatocytic seminoma. With only one case of relapse confirmed in the literature, this is a subgroup of patients in whom radiotherapy can safely be omitted.

We report the case of a 20-year old male with a right testicular tumor. Bilateral orchidectomy was practised considering the synchronous clinical, ultrasonographical and histological (intraoperative biopsy) findings of the left testis. The definitive pathological report showed a right embryonal carcinoma with wide intratubular germ cell neoplasia (IGCN) of the contralateral testis. IGCN (formerly carcinoma in situ) is present in about 5% of cases in the contralateral gonad of those patients with a
testicular neoplasm
. More than 50% will develop cancer in that testis. Clinical and physical examination findings are usually unspecific. The diagnosis of IGCN is based on biopsy, although ultrasonography could give some data too, as some authors report. We analyze the therapy options for IGCN: (orchidectomy, chemotherapy, radiotherapy, or "wait and see"). In our case, the first one was made. Chemotherapy was used due to existence of retroperitoneal lymph node

The spermatocytic seminoma is a distinct
testicular neoplasm
with a low tendency to
metastasize
. Two own cases with the diagnosis of a spermatocytic seminoma are presented. A third patient is described, where the initial diagnosis of a spermatocytic seminoma was retrospectively changed to classical seminoma after developing retroperitoneal relapse during surveillance. A literature review revealed distinct histopathological characteristics and a remarkably good prognosis for spermatocytic seminoma. With only one case of relapse confirmed in the literature, in these patients postoperative treatment can safely be omitted in favor of surveillance. In case of spermatocytic seminoma diagnosis should be confirmed by a second pathologist.
