This paper presents examples of non-neoplastic lesions of the spine that produce focal or diffuse signal alterations on MR images, and that may, therefore, be confused with metastases or primary neoplasms. Examples include endplate changes associated with degenerative disk disease, hemispherical spondylosclerosis, osteoporotic compression fractures, Paget's disease, focal and diffuse fatty infiltration, osteomyelitis, and changes associated with various arthritides. Distinguishing signal intensity characteristics on T1 and T2 weighted images and distinctive morphology are emphasized.

An autopsy case of genital Paget's disease with a widespread metastases is reported. A 69-year-old man with redness, itching and swelling of the scrotum obtained dermatological consultation and underwent tumor resection for Paget's disease. At 8 months after discharge, he suffered from a costal fracture because of carcinomatosis of the bone marrow. In laboratory data the serum level of alkaline phosphatase and carcinoembryonic antigen was shown to be high. Four months later, he died of severe congestive edema of the lung with diffuse micrometastases. An autopsy revealed a widespread metastases in various organs and lymph nodes. The Paget cells were positive in carcinoembryonic antigen.

A case of extramammary Paget's disease is reported. The distinguishing features of the case are: (1) localization of the primary lesion to the scrotum, (2) development of metastases in the absence of a recognizable underlying carcinoma, and (3) response of the systemic disease to a combination of radiation therapy and chemotherapy. The histogenesis, clinical behavior, and treatment of extramammary Paget's disease are reviewed.

A 74-year-old man with the chief complaint of ulceration of penile shaft and scrotal swelling was referred to our hospital on November 7, 1985. Biopsy specimen of the local lesion histologically revealed invasive adenocarcinoma with Paget's cell. Clinical studies revealed a high serum carcinoembryonic antigen (CEA) level and regional lymph node metastasis was highly suspected. We concluded that the patient was suffering from Stage II genital Paget's disease. Emasculation, bil-inguinal and pelvic lymph node dissection, and free skin mesh grafting were performed on December 24, 1985. Histological findings of the tumor confirmed Paget's disease and bil-inguinal and pelvic lymph node metastases. CEA staining showed CEA positive malignant cells in both the primary lesion and metastatic lymph nodes. After operation, the rapid deterioration of the patient's condition was accompanied by extremely high serum CEA levels, obstructive jaundice, and evidence of metastasis to the liver. He died 24 days after operation and autopsy could not be performed. We recommend aggressive treatment when clinical studies reveal invasion to the epidermis, high serum CEA levels and lymph node metastasis.

The incidence, clinical features, histogenesis, and treatment of extramammary Paget's disease (EMPD) are reviewed. This unusual skin lesion is associated with an underlying adnexal neoplasm in about 50% of cases. Also, the incidence of distant organ malignancies of EMPD is higher than expected by chance. Even in the absence of a recognizable underlying cancer, EMPD may occasionally produce distant metastases, indicating the malignant potential of this condition. Histochemical, immunohistological, and lectin binding studies demonstrate that the cell of origin of EMPD is the exocrine cell of sweat glands. Although EMPD may arise from eccrine cells, derivation from apocrine cells appears more common. The treatment of the primary lesion, by wide margin excision, is fraught by a high recurrence rate. Chemosurgery may reduce local relapse of EMPD. The value of adjuvant radiation therapy is unestablished. Chemotherapy has induced remission in 2 cases of advanced EMPD and needs testing in clinical trials.

Thirty-four patients with known Paget's disease were examined by CT in order to characterise the appearances of the bones on transverse sections. The typical findings have been related to their anatomical localisation and frequency. The differential diagnosis from conditions with similar CT appearances, such as osteoplastic metastases, fibrous dysplasia or osteomyelosclerosis, is discussed. The CT appearances of Paget's disease are typical in the same way as conventional radiographs.

Thirteen patients with confirmed Paget's disease were examined using a 1.5 T MR tomograph. Five patients had polyostotic and eight patients monostotic forms of the disease. The results of these MR examinations were compared with the results of MR examinations performed on patients with osseous changes of different origin (metastases of breast cancer and prostate cancer, osteomyelitis, bone marrow infiltration due to acute lymphatic leukaemia and plasmocytoma). MR tomography permits evaluation of bone marrow space, osseous changes, and the surrounding soft tissue. Areas of sclerotic bone, thickened corticalis and bands of connective tissue in the marrow space were recognised as structures with no signal in T1- and T2-weighted images. The red and yellow marrows showed normal signal intensity patterns, although they were often reduced in size.

Recent findings suggested a possible role for pinhole collimator scintigraphy to differentiate between metastases, infection and fractures of the vertebrae. The aim of this study was to verify these findings and to ascertain whether we could identify any other specific scintigraphic patterns. The study group consisted of 58 patients. A metastatic pattern was obtained in 18 patients, while 16 showed the compression fracture pattern, 3 the tuberculosis pattern and 1 the pyogenic infection pattern. The study was considered normal in 4 patients and showed a nonspecific pattern in 4. Sensitivity was 85% for the metastatic pattern and 83% for the compression fracture pattern. Specificity was 94% for both patterns. Unique new patterns were identified in Paget's disease and in degenerative disease of the vertebrae (12 patients). Pinhole scintigraphy of the vertebrae is a valuable aid for detecting specific diseases of the vertebrae. Our findings suggest a valuable new role for skeletal scintigraphy, improving the specificity and obviating, in most cases of vertebral disease, the need for additional investigations.

Malignant fibrous histiocytoma is a rare primary bone tumor, and there have been conflicting reports on its grades of malignancy. We are describing the cases of eight patients who were seen between 1977 and 1982. Four had pulmonary metastases and five, involvement of the lymph nodes. Five patients had a high level of serum alkaline phosphatase. None of the patients had a pathological fracture, an associated bone infarct, or Paget's disease. Seven of the eight patients died within one year after diagnosis.

There are two forms of extramammary Paget's disease in respect of clinic, therapy and prognosis. In most cases (about 95%) extramammary Paget's disease is an intraepidermal manifestation of a cutaneous adenocarcinoma. After horizontal spreading within the epidermis, malignant growth continues along the adjacent adnexal structures with dermal spreading and subsequent metastases. In rare cases (approx. 5%), extramammary Paget's disease is an epidermal manifestation of an adenocarcinoma primarily located in the intestinal or urogenital tract. Further diagnostic and therapeutical procedures depend upon the type of extramammary Paget's disease.
