The Cavitron Ultrasonic Surgical Aspirator was used to reduce the volume of diaphragmatic
metastases
in 33 patients. Thirty patients had epithelial carcinoma of the ovary and one each had tubal adenocarcinoma, papillary peritoneal tumor, and
mesothelioma
. Twenty-seven patients had Stage IIIC disease, and six had Stage IV. Initially, 13 patients had diaphragmatic disease greater than 15 mm diameter (of any single nodule), and 2 had disease 6-15 mm diameter. After cytoreduction using standard means, 11 patients had disease greater than 15 mm diameter and 2 patients had disease from 6 to 15 mm in greatest diameter. After using the Cavitron, 1 patient had no gross diaphragmatic residual disease, and 30 patients had disease 1-5 mm diameter. A 43-year-old alcohol-dependent man had sustained three acute episodes of chronic pancreatitis. At the third hospital admission enlarged axillary and supraclavicular lymph nodes, widening of the mediastinum and bone metastases were noted. Cytological examination of a needle biopsy of the supraclavicular lymph node revealed a poorly differentiated adenocarcinoma. Because of the marked enlargement of the pancreas and the history, a rapidly and unusually metastasizing carcinoma of the pancreas was diagnosed. In view of the rapid deterioration of the patient no chemotherapy was begun and he died 4 weeks after admission. Autopsy confirmed the chronic pancreatitis but no carcinoma of the pancreas. Instead there was a peritoneal
mesothelioma
with extensive lymphogenous and haematogenous
metastases
. The incidence of this tumour is ever increasing. It should be included in the differential diagnosis, because survival time can be increased if the correct diagnosis is made very early.

A series of almost 25,000 thyroids examined by fine needle aspiration (FNA) biopsy was reviewed to ascertain the incidence and presentation of metastatic cancers in thyroid FNA samples. Metastatic cancers in FNA samples from the thyroid were identified in 25 cases (0.1%); the primary tumors were carcinomas of the kidney (8 cases), lung (7 cases), breast (5 cases), cervix uteri (1 case) and colon (1 case) and 1 case each of malignant melanoma, malignant pleural
mesothelioma
and rhabdomyosarcoma. FNA cytology was positive in all 25 cases. In 11 cases, the primary tumor was clinically known at the time of FNA biopsy; of the other 14 cases, cytology suggested that the malignancy was metastatic in only 5.
Metastases
of renal and mammary adenocarcinomas were almost indistinguishable from follicular and papillary thyroid carcinomas on cytologic grounds. The results demonstrate the rarity of this finding and the difficulty of diagnosing a metastatic tumor in the thyroid by FNA biopsy, in the absence of a clinical history of a prior primary neoplasm.

Opinions differ widely as to the value of morphometric techniques for differentiating between mesothelial cells,
mesothelioma
cells and metastatic adenocarcinoma cells in pleural effusions. The significance of the changes in the chromatin structure to differentiate between these three cell types was stressed by various scientific groups only qualitatively, and until now no group has used digital methods to quantify chromatin structures. The controversy about the value of morphometric methods was the reason for conducting the following study. We wanted to investigate (i) the importance of various planimetric and texture parameters as well as invariant moments as morphometric discriminators; and (ii) to set up a data structure to make possible the further diagnostic use of the multivariate morphometric analyses method for other applications. , and 16 cases of secondary cardiac tumors. It was found that the nature of primary cardiac tumors could be speculated by two-dimensional echocardiography based on their pathological features. Most patients with large left atrial myxoma had obstructive symptoms of mitral valves, abnormal ECG and enlarged left atrium, whereas patients with small atrial myxoma, embolic phenomenon was liable to occur. Echocardiography of secondary cardiac tumors showed that the tumors usually invaded most frequently both the myocardium and pericardium as single or multiple nodular echoes in the myocardium under pericardium or within the pericardial cavity, with profuse pericardial effusion. . After a course of chemotherapy, the patient developed lower back pain. Plain X-ray and bone scan were negative for

, but magnetic resonance imaging (MRI) revealed multiple metastatic lesions to the thoracolumbar vertebrae. Subsequent lumbar spine biopsy confirmed the presence of malignant mesothelioma. The patient then received radiation therapy. This report illustrates an unusual presentation for malignant mesothelioma and shows that, in addition, when bone metastases are clinically suspected, MRI may be a useful adjunct to more conventional imaging approach.

Two hundred consecutive patients with malignant pleural effusion were reviewed. The pathologic etiology of malignant pleurisy was: primary lung cancer in 123 cases; five,
mesothelioma
; and 72 cases secondary to metastatic tumors. Adenocarcinoma of the lung and mammary cancer were the most frequent tumors causing malignant pleural effusion. The modalities employed in local treatment consisted of thoracocentesis in 62 patients, tube thoracotomy in 111 cases with local instillation of adriamycin, MMC, CQ, 5FU, OK432 or talc. Surgical procedures including pleuropneumonectomy or reduction surgery of the tumor with decortication were performed in ten patients. Tube drainage with local instillation of drugs was more effective than thoracocentesis with or without local therapy. Excellent initial results were obtained in patients who received reduction surgery with decortication and pleurodesis. Results of cytologic investigation were positive in 157 cases (78.5 percent). The tumor cells disappeared in 79.4 percent of primary cancer pleurisy cases and 81.1 percent of patients with
metastases
while disappearance or significant decrease in pleural effusion following treatment was obtained in 75.2 and 77.8 percent respectively. The median survival was 11.3 months in primary cases, and 11.7 months in patients with
