The clinical beginning of diffuse pleural
mesothelioma
is a pleurisy. Initially pleura may be normal at X-ray examination, then either a pachypleuritis or a tumorous pleura appears. Thery serious since there is in the literature no cured case. The average survival time from diagnosis to death is 15 months. At terminal stage
mesothelioma
does not invade the other coelomic cavities. It may remain limited to the hemithorax where it has started, invading then the organs by contiguity.
Metastasis
may occur but their frequency is very variable, from 30 to 70% according the literature. On a therapeutic point of view all was tried: Surgery is rarely curative but may reduce temporarily effusion pain and dyspnea. Radiotherapy at high doses produces a partial effect in some series, but results are variable. Chemotherapy: adriamycin could be the most promising agent. At present time according to many authors no treatment seems to change the course of the disease.

to the pleural cavities, lungs, and spleen. These tumor cells contained a large amount of stable of stable glycogen-like carbohydrate insoluble in formalin and tolerating postmortem changes. Electron microscopy revealed features similar to those mesotheliomas arising from body cavities.

What effects such changes will have on morbidity and mortality rates is incompletely understood.

(one case). In addition, four of 29 leukaemic patients and six of 28 with non-Hodgkin's lymphoma showed testicular involvement. The

Microscopically, these were represented by tumour cells within the interstitial tissue without involvement of the seminiferous tubules; interstitial tissue and tubular involvement, and tumour confined to the seminiferous tubules respectively.

Ffty cases of malignant pleural
mesothelioma
(47 men and 3 women) have been diagnosed during the period 1966-76 in Barrow-in-Furness, all of them histologically proved and accepted by the Pneumoconiosis Panel. At the time of writing only three survive. There was a history of industrial exposure to asbestos in all 47 men, who had worked in various trades in the shipyard, as had one of the women. One of the women was married to a shipyard plumber, who may have brought home asbestos dust on his clothes. Only one patient, a housewife, aged 28 at diagnosis, had no known asbestos contact. The asbestos content of the lungs has been measured in the last 20 cases, in 18 of whom it was found to be substantially greater than the accepted levels for the general population. In a small number studied by electron microscopy, the predominant fibre was crocidolite. A positive histological diagnosis was achieved in 39 subjects during life in the expectation of securing industrial compensation, but, in spite of this, less than half of the dependants are currently receiving payments.
Metastases

The gas sterilized bronchofiberscope has been utilized as a pleuroscope for visual exploration of the pleural space and forceps biopsy of abnormal lesions in 16 patients with undiagnosed pleural disease. The instrument was inserted through a small (1-2 cm) incision. Aspiration of pleural fluid and instillation or aspiration of air to produce a controlled pneumothorax are easily accomplished through the instrument's suction channel. In eight patients with undiagnosed pleural effusion, biopsy of visualized nodules established the diagnosis of carcinoma. Three patients with bronchogenic carcinoma and pleural effusion had no pleural
metastases
at pleuroscopy which was confirmed in two patients who had thoracotomy and lung resection. Parenchymal subpleural nodules of metastatic carcinoma were confirmed by pleuroscopic biopsy in one patient. Biopsy via the pleuroscope was unsuccessful in two patients, one with pleural fibrosis, probably related to asbestosis, and one with
mesothelioma
. Both required open surgical biopsy of the pleura. There has been minimal patient discomfort and no serious complications.

All of the analyzed large cell anaplastic carcinomas and the majority of the epidermoid carcinoma (89%), adenocarcinoma (79%), and

(82%) displayed binding capacities for EPO. Five out of the eleven small cell lung carcinomas, the analyzed

and lymphocytic interstitial pneumonia revealed definite staining, too. Binding sites could, in addition, be seen in air dried, non-fixed, acetone-fixed, and ether-ethanol-fixed cytological specimens.
