Lymph node metastasis was evaluated angiographically in 151 patients with gastric cancer. In 20% of the patients with nodal metastases, abnormally staining masses were demonstrated by angiography. Superselective angiography is useful in confirming the presence of lymph node involvement.

To precisely determine the process spread in gastric cancer, in 90 patients left prescalene biopsy was performed simultaneously with the operative procedure on the stomach. In 20% of the patients the histological examination of serial sections showed gastric cancer metastases in non-enlarged supraclavicular lymph nodes. In vast majority of the observations a supraclavicular metastasis diagnosed only histologically was a manifestation of the tumor process spread. In these patients only explorative laparotomy or palliative resection was performed, followed by lethal issue (but one case) in the early postoperative period. The rationale of incuding prescalene biopsy in the scheme of examination for gastric cancer is discussed.

The leukocyte migration assay proved to be a very sensitive tool in gastric cancer diagnosis. When patients' leukocytes were exposed to a panel of 3m KCl extracts, positive reactivity was found at a rate of 92%. All patients with non-malignant gastric diseases exhibiting "positive" reactivity with gastric tumor extracts also showed a pathological migration index with a 3m KCl extract of normal gastric mucosa, while only in 16% of patients with gastric cancer leukocyte migration was influenced by this extract. Using gastric tumor extracts patients with colorectal cancer showed a cross-reactivity in 67% and patients with carcinoma of the pancreas and oesophagus in 42% of the tests. Three months after curative surgery most patients' leukocyte migration was found to be in the normal range. Positive reactivity reappeared in patients with local recurrence or metastases.

Among 16 patients admitted into the Volynsk oncological dispensary with far-advanced tumors of the stomach and colon, the tumor perforation was recognized prior to surgery only in a half of the cases. Despite marked local proliferation and distant metastases there may be a pronounced pain syndrome. The absence of parallelism between the moment of intestinal content getting into the abdominal cavity and the protective reaction of the anterior abdominal wall musculature was characteristic of these patients. While the symptoms of peritoneal irritation in perforated gastric cancer can be recognized within the period up to 2 hours, the perforation of rectal cancer is frequently not accompanied with peritoneal signs. Among gastric cancer patients the perforation was diagnosed within 6 hours in six cases. The time of recognition of perforated cancer of the transverse colon and rectum was 15--16 hours.

Gastric cancer is a common cause of death in the Netherlands. The prognosis is generally poor at the time of diagnosis. The designation early gastric cancer (EGC) refers to a gastric carcinoma which does not infiltrate beyond the submucosa. This definition is not influenced by absence or presence of metastases or by the diameter of the tumour. The 5-year survival of EGC is 90% or more. In the Netherlands too, the diagnosis can regularly be made if--in the case of persistent vague upper abdominal complaints--an optimal radiological examination of the stomach is done. At even the slightest radiological suspicion, or if complaints persist in spite of negative radiological findings, gastroscopic examination and multiple aimed biopsies should follow. Personal observations in ten cases are presented.

Records of 403 patients with gastric cancer seen between 1957 and 1966 at the Lahey Clinic Foundation have been analyzed. While the operability rate was 94 per cent and the resectability rate was 58 per cent, the cure rate was only 11 per cent and was unchanged from the previous decade. Significant correlation occurs with location, clinical type, histologic type, size of tumor, and number of lymph node metastases. The surgical approach is aggressive in exploration and in treating extensive but resectable cancers but is conservative for the average cancers and does not include resection of spleen, omentum, pancreas, and total stomach if possible. The histologic differentiation into diffuse and intestinal varieties provides additional prognostic information as well as clues to the etiology of varieties of gastric cancer.

When gastric cancer patients' leucocytes were exposed to a panel of tumour extracts, leucocyte migration reactivity was "positive" before operation (90% of cases), it declined after surgery and it reappeared in patients with local recurrence or metastases. In patients with non-malignant gastric diseases, "positive" reactivity against tumour extracts was associated almost exclusively with atrophic gastritis with and without intestinal metaplasia (24/43 cases). "Positive" patients also showed a pathological migration index with an extract of normal gastric mucosa.

Methods have been established to produce gastric cancer in rats and dogs by administration of N-methyl-N'-nitro-N-nitrosoguanidine or of the ethyl derivate. The agent is administered in drinking water or by a pellet diet soaked in the carcinogen. Histologically well differentiated and poorly differentiated types of adenocarcinoma and signet-ring cell tumors are induced in several months with greath reliability. Metastases were observed in both rats and dogs with gastric carcinoma. The carcinogenic effect could be enhanced by surface active agents, sodium chloride, iodoacetamide, insertion of plastic beads into the stomach and gastroenteroanastomosis. Follow-up studies by radiologic, endoscopic and bioptic examinations are possible in the dog. There are similarities in these experimental tumors to those in man and thus they provide means for the investigation of histogenesis, prevention, and chemotherapy of gastric cancer. An adenocarcinoma of the glandular stomach of a Wistar rat was successively transplanted to new born rats of the same strain.

The early gastric cancer can now be diagnosed more frequently by improved methods of roentgenology and endoscopy. At present 16.7 percent of the gastric resections for cancer in our clinic are for early gastric cancers. Indicative clinical symptoms were demonstrable in 25 of our own 27 cases described here. Infiltration of the gastric wall (mucosa- and submucosatype) and lymph-node metastases are important to the prognosis. Intraoperatively, it is essential to localize the tumor unobjectionably and to perform a sufficiently radical resection (generally subtotal gastrectomy and excision of the lymph-nodes). The 5-year-survival-rate totals 70 to nearly 100 percent. Postoperatively controls are necessary for the diagnosis and eventually the treatment of possible recurrence.

Metastasis to lymph nodes was seen more higher rate in protruding lesion, especially in Type IIa and IIa + IIc than in concaving lesion. In view of the metastasis to the secondary regional lymph nodes or the third regional lymph nodes, care should be taken in the eradication of lymph node in early gastric cancer of Type IIc. Five-year survival rate was 88.9% of the resected cases of early gastric cancer.
