Eleven patients with measurable subcutaneous or pulmonary
metastases
were selected for a study of the effectiveness of the radiosensitizer misonidazole (MIS). Evaluable data were obtained in 6 patients and radiosensitization demonstrated in 5. Patients were irradiated either before or after MIS, and each patient acted as his own control. Response to treatment in 5 cases was assessed in terms of growth delay, and radiation doses were selected in expectation of enhancement ratios of 1.2 to 1.5. In 1 case evidence of sensitization was obtained from differential tumour clearance from 2 areas of skin irradiated before or after MIS. Results in 4/5 growth-delay studies indicated enhancement ratios ranging from 1.1 to greater than 1.5. An enhancement ratio of 1.3 was measured in a case of squamous carcinoma treated by a 10-fraction course of irradiation. Evidence of sensitization was obtained in breast carcinoma, osteosarcoma,
leiomyosarcoma
, prostatic carcinoma and synoviosarcoma. The results of this study support the view that MIS may improve the radiotherapeutic management of a wide range of tumours, although more extensive data are required to identify those categories of disease in which greatest benefit will be obtained, and to indicate the optimum radiation schedule.

Sixty-five patients operated with total pancreatectomy were reviewed with respect to factors influencing operative mortality and morbidity, long-term survival, and metabolic sequelae. The diagnoses were pancreatic cancer in 58 patients, periampullary cancer in three, cancer of the bile duct in two and

of the duodenum and cystadenocarcinoma of the pancreas in one patient, respectively. In nine of the 58 cases with cancer of the caput, the histological examination revealed multicentricity of the tumor. In 44%, there were signs of degeneration and fibrosis in the distal part of the gland. Hospital mortality was 23% for the entire series. After 1970 the hospital mortality was 17%, and among patients operated by senior surgeons especially trained in pancreatic surgery, the hospital mortality was 12% during the whole period. The peroperative bilirubin levels seemed to influence survival time. Among 24 patients operated before 1975 in whom the operating surgeon judged the operation as radical, a five year survival of 21% was recorded. In patients without detectable lymph node

, the mean survival time was 25 months. The postoperative exocrine insufficiency and diabetes were possible to control. A blood sugar level above 10 micromol/l was found to significantly decrease the frequency of hypoglycemic attacks. Thirteen cases of leiomyosarcomas of the extremities are reported. Most of the cases were operated on without information concerning benignancy or malignancy. Plain radiographs showed amorphous calcification in two cases. Three cases had angiographic study and all revealed malignant neovascularity. In no literature that we reviewed was angiography mentioned in the cases of leiomyosarcomas of the extremities. Venography alone has been done in many reported cases. The
