Records of 685 cases of malignant tumors in the oral and maxillofacial regions were collected. Pulmonary metastases occurred in 70 of these cases. The metastatic rate was 10.2%. Roentgenographic appearances of pulmonary metastases were divided into 3 types: (1) Multiple ball-like lesion (78%); (2) Single ball-like lesion (14%); (3) Irregular lesion (8%). When metastasis was single, diagnosis was made according to the location of the metastasis and comparison between and old recent roentgenograms. Pulmonary metastasis was more frequent in tumors of the submandibular gland (34.5%), the base of the tongue (33.3%), and the sublingual gland (27.3%). Adenoid cystic carcinoma (47.8%) and adenocarcinoma (26.5%) might readily develop pulmonary metastasis. Pulmonary metastasis was rare in squamous cell carcinoma (2.5%). The metastatic rates were 3.9%, 5.4%, 6.5%, and 7.1% in stage I, II, III and IV, respectively. The period between the occurrence of pulmonary metastasis and initial treatment was from 1 month to 8 years and 5 months, with a mean of 33.8 months. The period between the occurrence of pulmonary metastasis and death of the patient was from 1 month to 8 years and 8 months, with a mean of 21.4 months.

Adenoid cystic carcinoma of the Bartholin's gland is a rare tumor. Only 41 cases, to our knowledge, have been described in literature. It is impossible, at present, to determine the best management of this disease. Local recurrence and distant metastases (mainly of the lung) frequently occur. A case of adenoid cystic carcinoma of the Bartholin's gland is looked into with respect to clinical and immunohistochemical characteristics. Radiation therapy is suggested as effective together with surgery. The efficacy of routine lymphadenectomy has not been demonstrated. The case reported is the first described with nodal metastasis as single mode of recurrences presentation.

Adenoid cystic carcinoma of the cervix, traditionally associated with a poor prognosis, occurs in postmenopausal patients in the vast majority of cases reported. Only four cases have been reported in women less than age 40, and none in women less than age 30. Three new cases of adenoid cystic carcinoma of the cervix are reported in women aged 24, 27, and 38 years. All three patients were treated with radical pelvic surgery; lymph node metastases and vascular involvement were prominent. Adjuvant chemotherapy with cisplatin was used in two patients, one of whom has had long-term survival. A review of the literature is also presented.

Adenoid cystic carcinoma of the nasopharynx is a rare disease with 34 previously reported cases in the literature. We report three additional cases with clinical presentation, treatment, and outcome. The presenting symptoms were facial pain in one case, middle ear effusion and epistaxis in another, and diplopia in the third. Radiotherapy was administered with relief of symptoms in all three cases. However, two patients with long-term follow-up developed local recurrence as well as distant metastases. The literature review showed that patients with this disease present with symptoms and signs such as insidious onset of facial pain, epistaxis, diplopia, and hearing loss. The final outcome, despite vigorous treatment, was dismal.

Adenoid cystic carcinoma of the breast is a rare neoplasm that usually has a benign biological behavior. A patient who was operated upon for removal of this tumor developed metastases to the lungs and brain 12 years after mastectomy. This is probably the first report in the literature of brain metastasis from adenoid cystic carcinoma of the breast. Radiological and histological evidence of both primary breast tumor and the metastasis to the brain are presented.

A unique case of adenoid cystic carcinoma arising in a ceruminous gland, with widespread pulmonary metastases, is presented. This is the first case in the English literature in which the diagnosis was made from material obtained by needle aspiration from a pulmonary metastasis. The literature is reviewed and the histological features and clinical course are discussed in the paper. Adenoid cystic carcinoma is the most common tumor of ceruminous gland origin, and is the only one which produces widespread metastases in the lungs. Tumors arising from ceruminous glands are slightly more prevalent in females, and pulmonary metastases occur almost three times as often in females as in males. Suggested treatment is wide local excision of the primary tumor and of local recurrences, and excision of distant metastases when possible. Irradiation is useful only for palliation. All patients, but especially females, should be followed carefully for the detection of possible pulmonary metastases.

The radiographic appearance of 16 uncommon primary esophageal tumors (4 adenoid cystic carcinomas, 3 adenoacanthomas, 4 small cell carcinomas, 2 adenopapillary carcinomas, adenocarcinoma, mucoepidermoid carcinoma and malignant melanoma), is presented. These tumors were found in a retrospective study of 270 esophageal neoplasms. Sufficient radiographic, histologic and clinical data was available to substantiate primary esophageal origin of the tumors. Cylindromas tender to have a smooth surface and either funneled or polypoid appearance. None of the tumors, however, had radiographic appearance readily distinguishable from variable patterns of the common esophageal carcinomas. The age and sex distribution, location as well as prognosis of the rate adenocarcinomas appear to be similar to squamous cell carcinomas. The small cell carcinomas of the esophagus only had an average prognosis of 4 months. The x-ray examination was of little value in predicting local tumor spread, mediastinal metastases or prognosis.

Adenoid cystic carcinoma, or cylindromas, constitute a large and distinct group of neoplasms with characteristic microscopic features recognizable in tissues as varied as the nasal passages, tongue, and bronchi. Cylindromas of the larynx are rare and only 60 cases have been reported in a review of the literature in 1977. In this report, a case of adenoid cystic carcinoma originating in the subglottic area of the larynx is discussed, with emphasis on the clinical and pathologic characteristics. In the past, the histogenesis of cylindroma has been the subject of considerable controversy, but recent light microscopic and ultrastructural studies strongly implicate the seromucinous glands of the respiratory and upper digestive tracts as the tissues from which these neoplasms derive. In general, cylindromas of the upper alimentary and respiratory passages are best treated by wide surgical excision. Some clinicians have combined surgery with postoperative radiation therapy in an attempt to prevent or delay recurrence and metastases. Chemotherapeutic agents have also been employed in the postoperative management of adenoid cystic carcinoma although the longterm results of this treatment modality remain to be determined.

Adenoid cystic carcinoma constitute around 0.4 to 1.7% of all carcinomas of cervix. It generally presents in elderly age group, however few cases in younger age have also been reported. Majority of cases present in early stage. Though concluding statement regarding the results cannot be made from the cases accumulated from the various reported series, nonetheless this review shows that radiotherapy too is effective in early stage and even in stage I results with radiotherapy appear better than surgery. In advanced stage outcome is invariably poor. In early stage main pattern of failure is distant metastasis. Both local and distant failure are observed in stage III and IV. Though lung is the commonest site of metastases (51%), abdominal cavity and brain are also found to be involved in some cases. Limited experience shows effectiveness of high doses of cisplatin and multiagent chemotherapy. Cases of early stage disease having risk for metastases and local failure should be identified and managed with multimodality treatment using surgery for bulky disease with post-operative radiotherapy and chemotherapy. In advanced stages combination of chemotherapy and radiotherapy is required. Exenterative surgery for central failure has been used effectively. Overall no evidence of disease rate is found to be about 48%, with mean follow-up of 39 months.

Adenoid cystic carcinoma of the head and neck is relatively rare and is characterized by slow evolution, multiple recurrences, protracted clinical course, and late distant metastasis. This article presents its peculiar clinical course, response to therapy, and long-term treatment results by analyzing 67 cases treated from 1979 to 1991 at the Seoul (Korea) National University Hospital. The most common primary site was the parotid gland in the major salivary glands and the nose and paranasal sinuses in the minor salivary glands. The local control rate was 71.1% at 5 years and 44.3% at 8 years. Late-occurring distant metastases did not allow a plateau in survival curves, even after 8 years. Our study also revealed that surgery combined with postoperative radiotherapy could yield better local control.
