Since prolactin has several modes of action on prostatic growth and physiology, the effect of the antiprolactin bromocriptine on plasma kinetics and intraprostatic metabolism of testosterone was studied in patients with untreated prostatic cancer; a therapy protocol was deduced which was controlled in 27 patients with advanced inoperable

. Bromocriptine resulted in a significant suppression of prolactin and testosterone as well and favored testosterone elimination from the plasma pool. Prostatic androgen uptake was enhanced and the intraprostatic metabolism altered in relation to tumor grade. Adjunctive administration of bromocriptine to 27 patients, mostly in the state of hormone resistance, resulted in an overall objective regression of 22.2% and in stable disease in 55.6% of the patients. In half of the individuals a prompt relief of bone pain from osseous

was observed as well as improvement of micturition and decline of phosphatase activity. This preliminary data justify further investigations under controlled and randomized conditions.

The brain isoenzyme of creatine kinase (CK BB) occurs in trace amounts in normal serum and is moderately increased in only a small number of non-oncological conditions. Although many tissues and tumors contain CK BB, we observed serum elevations only in certain carcinomas. Eleven patients with tissue-proven small cell anaplastic carcinoma (SCAC) of the lung had striking elevations of serum CK BB and no evidence of central nervous system (CNS)

After the injection of the potent carcinogen 9,10-dimethyl-1,2-benzanthracene (DMBA) male rats (Fisher/Furth), mice (Strong A/J), and Golden hamsters, which had been previously conditioned by castration and in which the prostate had become histologically atrophic, developed tumors consistent with
prostatic adenocarcinoma
. One month after castration, intravenous injections of DMBA were given in the vena cava or jugular vein once a month for 3 months. Three to four months later, histologic evidence of prostate adenocarcinoma was consistently found in each of the groups of castrated animals (11 rats, 22 mice, and 11 hamsters) which survived the experiment. The glands were enlarged grossly and ureteral obstructions were also noted.

Creatine kinase isoenzyme BB (CK-BB) is found in the serum of patients with various types of cancer. Using both radioimmunoassay and agarose electrophoresis, we found abnormal amounts of this isoenzyme in the serum of 15 of 17 patients with untreated prostatic carcinoma. Among patients with other types of adenocarcinomas and
metastatic disease
, serum CK-BB was increased in most of those who were unresponsive to therapy. In benign or malignant prostate tissue and in malignant pleural effusions, cytoplasmic CK-BB as determined by immunoperoxidase staining was found in epithelial cells. Prostatic fluid had high concentrations of CK-BB, as did malignant, but not benign, pleural fluid supernates. We conclude that glandular epithelial cells contain CK-BB, which is released in benign and malignant states and may appear in higher concentrations in the circulation in malignant states. These conclusions are consistent with predictions we have made from a model experimental system concerning characteristics of possible tumor markers. A progress report on the treatment of 116 patients with
adenocarcinoma of the prostate
treated by external irradiation is presented. Fifteen, 60, and 41 patients presented initially with Stages A, B, and C, respectively. The majority of the patients received 7,000-7,500 rads during a period of seven to eight weeks, locally to the prostate gland. The five-year actuarial survival were 90, 70, and 40 per cent for Stages A, B, and C, respectively. The five-year survival rates for patients with well and moderately differentiated tumors were significantly better (45 per cent) than for those with poorly differentiated tumors (24 per cent). The five-year survival rate in the patients who received prior or concomitant hormone manipulation was lower (33 per cent) as compared with those receiving radiation therapy alone (43 per cent). All of the above differences were statistically significant (0.01 level). Of the patients failing, distant
metastases
have developed in 88 per cent suggesting that subclinical distant
metastases
might have been present prior to initiation of radiotherapy. The local control rate was 82 per cent. The morbidity of this mode of therapy was found to be acceptable. Radical radiation therapy is an effective method for control of carcinoma of the prostate and is potentially curative.

causing urinary retention is reported. The unique findings of the urethrogram are described. Transurethral resection provided symptomatic relief. No local recurrence of tumor was observed during the follow-up period of sixteen months.

We believe that the probable "estrogen-free" site which is present in the liver while a patient is on estrogens gives a conductive environment for local growth of prostate malignancy. We report 1 patient who improved with local irradiation and 2 others who stabilized and symptomatically improved with chemotherapy. Multiple agent combination chemotherapy was used to treat 16 patients with Stage D
adenocarcinoma of the prostate
in relapse. Agents employed were cyclophosphamide (Cytoxan), methotrexate, 5-fluorouracil, vincristine, and prednisone. Results were encouraging. Six of 16 patients achieved an objective response (2 complete, 3 partial, 1 stabilization) for a 37.5 per cent response rate. Eleven of 16 patients achieved a subjective response for a 69% response rate. Soft tissue
metastases
may be more responsive than bony lesions. Employment of vincristine made no obvious difference in response rate in those patients in whom it was used. Toxicity was only moderate. Further studies are warranted.
